Creatine monohydrate helps muscles rapidly recycle energy during short, intense efforts, and it may support training adaptations. In a female-focused review, researchers found no statistically significant increase in pooled adverse events, gastrointestinal events, or weight change—but the evidence is limited and does not prove that side effects are impossible. The most practical expectation is that some people see the scale rise, often because of water retention; that alone does not mean fat gain.

What creatine does

Creatine is a compound stored largely in muscle. There, it helps replenish phosphocreatine, which supports the rapid regeneration of ATP, the cells’ immediate energy source. This system matters most during brief, repeated high-intensity efforts such as sprinting or lifting. Supplementing can increase muscle creatine stores and may help with repeated efforts and the training adaptations that follow.

Most of the evidence and guidance discussed here concerns creatine monohydrate. The NIH Office of Dietary Supplements says other forms have not been shown superior to monohydrate for increasing muscle creatine levels, digestibility, stability, or safety (NIH ODS fact sheet). Creatine is not a substitute for training, adequate food, or recovery, and this evidence does not establish benefits for every kind of exercise or health outcome.

What the evidence says about side effects in women

A 2020 systematic review and meta-analysis examined adverse outcomes in women taking oral creatine monohydrate. It included 29 studies and 951 participants. The review found no statistically significant difference between creatine and comparison groups in pooled total adverse events (RR 1.24; 95% CI 0.51–2.98), gastrointestinal events (RR 1.09; 95% CI 0.53–2.24), or weight change (de Guingand et al., 2020).

Those results are reassuring, but they are not proof of zero risk. The confidence intervals are broad: for example, the interval for total adverse events is compatible with outcomes ranging from lower risk to a potentially meaningful increase. The review also found that only 58 of 656 studies identified were female-only studies; that figure describes the literature identified, not the number of studies eligible for the adverse-outcome analysis. The authors called for better adverse-event monitoring and reporting.

Weight gain and water retention

The review’s pooled estimates for weight change were 1.24 kg before intervention (95% CI −0.34 to 2.82) and 1.37 kg after intervention (95% CI −0.50 to 3.23); neither was statistically significant. The estimates do not establish that creatine causes a particular amount of weight gain in women. Individual responses vary, and a change on the scale does not by itself show whether the change is water, muscle, or fat.

The NIH ODS notes that studies have not found a consistent set of side effects apart from weight gain, commonly attributed to water retention and possibly changes in muscle protein synthesis. It also describes anecdotal reports of nausea, diarrhea, cramps, and heat intolerance. These reports should not be mistaken for effects consistently demonstrated in controlled studies, nor do the cited sources establish dehydration, cramps, or kidney damage as common effects of creatine.

Kidney and liver measurements

In the female-focused review, measured renal and hepatic outcomes did not change significantly. This finding in the studied participants does not establish personal safety for someone with kidney disease or another medical condition. If you have a health condition or take medicines that may affect kidney function, discuss supplementation with a clinician before starting.

How much is commonly used

General protocols in the NIH ODS fact sheet include daily doses around 3–6 g without a loading phase. The International Society of Sports Nutrition’s position stand on female athletes discusses 3–5 g per day in its context (ISSN, 2023). These are general reference points, not personalized instructions or a guarantee that a given dose is suitable for you.

Loading is not required to use a daily protocol without loading. The appropriate choice can depend on a person’s goals and circumstances; the sources cited here do not establish a women-specific dose for every person or life stage. Check the serving size and ingredient list so you know how much creatine monohydrate each serving contains.

Women’s health, pregnancy, and evidence gaps

Female-specific evidence remains limited. The ISSN’s 2023 position stand identifies creatine as one of the supplements with the most evidence for female athletes while noting the broader shortage of female-specific supplement research. A 2025 review spanning menstruation, pregnancy, and menopause calls for further work on dosing and longer-term effects (2025 review).

Healthy-adult findings do not answer whether creatine is suitable during pregnancy. The sources here do not establish a direct human pregnancy-safety conclusion, so discuss use with a qualified clinician rather than extrapolating from studies in other populations. These sources also do not justify promising hormonal, reproductive, cognitive, bone, or menopause-specific benefits.

Choosing a creatine product

If you decide to buy creatine, use the label to compare the details that affect what you are actually taking:

  • Form: Plain creatine monohydrate is the form covered by most of the evidence here.
  • Amount per serving: Check the grams of creatine per serving rather than relying on a scoop description.
  • Other ingredients: Note added ingredients if you want to avoid them or are tracking total intake.
  • Quality checks: Look for independent quality testing only when you can verify it; the sources here do not verify a particular brand or certification.
  • Cost and convenience: Compare serving cost and whether the format fits your routine.

The evidence discussed here does not establish that a particular brand or formulation is superior. Choosing a different creatine form or a more elaborate blend is not, on this evidence, a proven way to improve safety or results.
