To keep AI-generated changes from merging without human review, configure merge protection on the destination branch: require a pull request or merge request, require approval from at least one eligible human, and separately require the CI checks you need. Then restrict direct pushes and bypass permissions so the rules actually apply.
CI checks and human approval are different gates. A passing test or security scan does not count as a person reviewing the changes. The exact controls depend on your repository host.
Which settings make human review mandatory?
Set the rule on every destination branch an AI agent or contributor could update. A review requirement on a branch does not protect another branch, and direct pushes can sidestep a pull-request or merge-request review workflow if they are allowed.
- Require a pull request or merge request. Prevent ordinary contributors and agents from pushing directly to the protected destination branch.
- Require at least one eligible human approval. Choose reviewers with the permissions and project knowledge needed to review the change. For sensitive areas, require a designated team or Code Owner instead of relying on any available reviewer.
- Require CI checks separately. Select the status checks or pipeline condition that must pass before merge. A green CI result is not approval.
- Choose how new commits affect approval. Decide whether a changed diff invalidates previous approval or whether approval must come from someone other than the latest person to push.
- Restrict exceptions and rule changes. Review who can bypass the merge rule, push to the branch, dismiss reviews, or edit protections.
GitHub describes the purpose of its required-review rule this way: “If you enable required reviews, collaborators can only push changes to a protected branch via a pull request that is approved by the required number of reviewers with write permissions.” GitHub Docs: About protected branches.

Configure the gate in GitHub
Use a branch protection rule or a ruleset targeting the destination branch. GitHub’s branch-protection setup is under the repository’s branch protection settings. Require a pull request before merging and set a nonzero approval count. Rulesets provide overlapping controls and can target repositories or organizations.
Choose review requirements
For broader review coverage, enable review from Code Owners when the repository defines them. You can also configure required status checks, conversation resolution, or a merge queue as separate merge conditions. Select only the checks and controls that fit the repository’s workflow.

Decide what happens when someone pushes new commits
GitHub offers two distinct safeguards. Dismissing stale approvals requires another review after commits change the pull request. Requiring approval of the latest reviewable push instead requires someone other than the latest pusher to approve, while earlier approvals may remain. If the concern is that unreviewed content could be added after approval, GitHub says dismissing stale approvals is the safer choice.
Account for Copilot cloud-agent behavior without assuming all agents behave alike
GitHub documents safeguards for Copilot cloud-agent pull requests: the agent cannot mark its pull request ready for review or approve or merge its own pull request. In the documented case, the person who assigned the task cannot count their own approval toward the required approval. When Copilot opens a pull request under its own app identity, GitHub documents one additional approval if the repository already requires at least one. Corresponding ruleset behavior is documented as public preview and may change.

GitHub also documents a separate Copilot code-review feature that can allow AI approvals to satisfy merge requirements; that feature is optional and documented as public preview. If the policy requires a human approval, make sure AI review approvals cannot substitute for it. Do not assume Copilot-specific safeguards apply to other agents.
Configure the gate in GitLab
In project settings, configure merge-request approval rules: set an approval count greater than zero, select eligible people or groups, and target the rule to the relevant branch. Code Owners can provide file-aware review. GitLab Ultimate also supports security approvals tied to vulnerability findings. These review rules can coexist with a failed-pipeline blocker, so approval and successful CI/CD can both be required.

Separate the author and committers from approvers
Check whether the project should prevent approval by the merge-request creator and by users who added commits. GitLab also documents that authors may otherwise edit approval rules on individual merge requests unless rule overrides are disabled. Available settings and tiers vary among GitLab.com, Self-Managed, and Dedicated offerings, so confirm the plan and instance-level policy in use.
Remove direct-push routes around review
GitLab warns that users allowed to push to a protected branch can skip merge-request approval rules. Restrict protected-branch push permissions if review must be mandatory. The reviewed GitLab documentation establishes general merge-request controls, not a special trigger for AI authorship; those controls apply to an AI-authored request only when it is subject to the rules and the agent cannot bypass them.

Verify that the rules block the routes you intend
After configuration, use a test pull request or merge request to verify the expected merge behavior. Exercise the missing-approval and failing-check cases, then change the diff after approval to confirm the selected stale-approval or latest-push behavior. Also check whether any configured bypass path allows a merge. This is a suggested verification procedure, not a report of a test performed here.
