CT uses X-rays and ionizing radiation; MRI uses magnetic fields and radiofrequency energy, not X-rays. CT is often chosen when speed matters or for particular diagnostic questions, while MRI can provide useful detail for selected soft tissues. Neither is universally better: the suspected condition, body area, urgency, patient history, implants, pregnancy status and possible use of contrast all affect the choice. A scan ordered to investigate a specific concern is also different from whole-body imaging offered to screen someone without symptoms.

How whole-body MRI and CT differ

Feature CT MRI
How images are made X-rays, which are ionizing radiation. A magnetic field and radiofrequency energy; it does not use X-rays or ionizing radiation.
Common advantage Fast imaging can be useful when a prompt answer is needed, including evaluation of internal injury or bleeding in emergencies. Can be useful for selected soft-tissue questions, including some involving internal pelvic organs and certain joints.
Key safety consideration Radiation exposure varies by exam and patient; pregnancy possibility and iodinated contrast should be discussed with the care team. Implants and metal require safety screening; gadolinium contrast and sedation have additional considerations.

When a clinician may use CT or MRI

Why CT may be selected

CT is used for a range of diagnostic problems, including trauma, infection, cancer and vascular conditions. It is fast and less sensitive to patient movement than MRI, which can matter when images are needed promptly or a patient has difficulty remaining still. Why MRI may be selected

MRI is used to assess organs and tissues in the chest, abdomen and pelvis, and it can be valuable when the question concerns soft-tissue detail. Some internal pelvic organs and certain joints may be better evaluated with MRI. That does not mean MRI is always preferable for those areas; the specific indication determines which exam is suitable.

Risks and preparation to discuss

CT: radiation and contrast

  • CT uses ionizing radiation, and the dose varies by exam and patient. If you are pregnant or could be pregnant, tell your care team before the scan. Radiation concerns should be weighed against the reason for an indicated exam, rather than used to assume that CT should be avoided.
  • Some CT exams use iodinated contrast. Serious allergic reactions are very rare, but tell the care team about allergies and relevant medical conditions so they can consider the risks and plan accordingly.

MRI: implants, contrast and sedation

  • MRI does not use ionizing radiation, but its strong magnetic field can affect some implanted devices. Tell the imaging team about implants, devices and metal in or on your body; they need to screen these for MRI safety before the scan.
  • Some MRI exams use gadolinium contrast. It has rare risks, particularly for people with serious kidney disease, so share relevant medical history with the care team.
  • If sedation is needed, it has its own risks and requires appropriate monitoring.

Whole-body screening is different from diagnostic imaging

A whole-body scan offered to a person without symptoms is a screening exam; a scan ordered to investigate a known symptom or concern is diagnostic imaging. Evidence about screening should not be used to judge every clinically indicated CT or targeted screening program.

RadiologyInfo’s ACR–RSNA-reviewed guidance on screening examinations, last reviewed June 15, 2026, says of whole-body CT screening for the general public: “The benefits do not appear to justify the radiation exposure or potential for unnecessary downstream follow-ups at this time.” This statement is specifically about whole-body CT screening of the general public, not CT used to answer a clinical question. Read the guidance on whether screening examinations are worthwhile.

Screening images can also show false-positive findings or abnormalities that would never cause harm. Follow-up may include additional tests, biopsies, operations or other treatment. Finding something on a scan is not by itself proof that screening improves health outcomes; screening programs need a defined population and evidence that benefits outweigh harms. How to discuss which scan fits a specific question

Ask the clinician who ordered the exam what condition or body area they are evaluating and what information the scan needs to provide. Useful factors to clarify include:

  • Whether a prompt scan or tolerance of movement is important.
  • Whether the question calls for soft-tissue detail.
  • How radiation exposure applies to the proposed CT exam.
  • Whether an implant, device or metal could affect MRI safety screening.
  • Whether contrast or sedation is planned, and how your medical history affects those choices.
  • Whether the scan is intended to investigate a specific concern or screen someone without symptoms.

These questions help explain the reasoning behind a recommendation; they do not replace the clinician’s assessment.
