Make sure the face stays uncovered and the nose and mouth remain clear. Recheck after putting the baby in, after any movement or adjustment, and regularly while carrying; if you cannot maintain a safe position, take the baby out and reset the wrap.
Check your baby’s position with TICKS
Australia’s ACCC Product Safety guidance summarizes the checks as: “Keep your baby: Tight, In view at all times, Close enough to kiss, Keep chin off chest and Supported back.” Use each point to check the baby’s position, not as a guarantee that a wrap is safe in every circumstance. The UK NHS and US American Academy of Pediatrics (AAP) give similar airway and positioning advice.
Tight
The wrap should hold your baby close enough to prevent slumping. Loose fabric can allow a baby to slump into a position that restricts breathing. Make sure the wrap is secured as its manufacturer instructs.
In view
You should be able to see your baby’s face by looking down. Keep the face uncovered, and ensure neither fabric nor your body covers the nose or mouth. Do not rely on sounds or obvious movement to tell you that the baby is comfortable: the ACCC warns that a baby in distress may not make noise or move noticeably.

Close enough to kiss
Your baby’s head should be high on your chest and close enough to kiss by tipping your head forward. If the baby is too low to see and kiss easily, take them out and adjust the wrap.
Keep chin off chest
Keep the neck straight and the chin away from the chest. A chin curled down against the chest can restrict the airway. Check the position again if your baby shifts or the wrap is adjusted.

Supported back
The wrap should support your baby’s back in a natural position and hold them close to you. In a practical check described by Royal Cornwall Hospitals NHS Trust for its neonatal procedure, a caregiver gently presses a hand against the baby’s back to check that the baby does not uncurl or move closer. This is that trust’s practical guidance, not a universal diagnostic test; follow your wrap’s instructions and seek clinical advice if you have concerns.
When and how often to recheck
- After placing your baby in the wrap: Check all five TICKS points and confirm the nose and mouth are unobstructed.
- After movement or adjustment: Recheck the baby’s face, chin, support and position whenever either of you moves or you alter the wrap.
- Regularly while carrying: Keep checking that the baby remains visible and breathing freely. Do not assume a quiet or still baby is necessarily fine.
- If a check fails: Take your baby out and reset the wrap. Do not continue carrying with the face covered, airway obstructed, chin against the chest or body unsupported.
Use the wrap only as directed for its specific model. If you cannot keep the baby’s face visible, airway clear and body supported, stop and get help with the fit rather than accepting a compromised position.

When a newborn may need individual advice
The ACCC identifies babies younger than four months, premature babies, babies with low birth weight and babies with breathing difficulties as having greater risk in carriers, wraps and slings. It advises consulting a paediatrician before using one with a premature baby. The AAP cautions that premature infants and infants with respiratory problems should not be placed in backpacks or other upright positioning devices because the position may make breathing harder.
If your baby was premature or has a medical or breathing concern, ask your baby’s clinician for individualized guidance before using a wrap. For every baby, check the exact product’s age, weight and developmental requirements; do not assume all wraps have the same limits.

Feeding and sleep in a wrap
Take your baby out to feed
The NHS advises: “If you need to feed your baby, take them out of the sling.” Do not feed your baby while they remain in the wrap, and do not let the material cover the baby’s head.
Move a sleeping baby to a safe sleep environment
A wrap, carrier or sling is not a safe sleep environment, according to the ACCC. If your baby falls asleep in it, keep checking their breathing and chin position, then move them to a safe sleep environment as soon as possible.

Check the wrap and its instructions
Before using a wrap, confirm that it is made of strong material, is suitable for your baby’s age, size and development, and comes with instructions. The ACCC recommends getting a demonstration and having help the first time you use one. A demonstration can help you learn the wrap’s setup, but it does not replace repeated checks of your baby’s position.
These general checks do not assess a particular wrap, a specific baby’s fit or medical status, or the condition of a product. Follow the manufacturer’s instructions for the exact model, and seek help if the wrap cannot keep your baby’s face visible and airway clear while providing support.
