Measure test coverage beyond code coverage by defining what needs to be tested, listing those coverage items, and linking them to tests and results. Track requirements, high-risk scenarios, modeled behavior, input partitions, security work, and test sensitivity as separate measures. Each answers a different question; none turns a percentage into proof that software is correct.
Start by defining what “covered” means
Coverage is meaningful only in relation to a defined test basis: the requirements, specifications, workflows, risks, state model, interfaces, or quality attributes against which tests are designed. ISO/IEC/IEEE 29119-1:2022 defines test coverage in terms of specified coverage items exercised by test cases; examples include equivalence partitions, state transitions, and executable statements (ISO/IEC/IEEE 29119-1:2022).
For each measure, make the denominator inspectable. A practical calculation is covered in-scope items / total in-scope items. Publish the numerator and denominator, what counts as covered, any exclusions, the test level, and the reporting window. This is a useful way to apply the definition; it is not a reason to combine unlike measures into one score.
For example, “92% covered” is hard to interpret without knowing whether the items are acceptance criteria, transitions in a state model, or lines of code. A useful report lets a reader inspect the items and understand what evidence a passing test supplies.

Measure requirement and acceptance-criteria coverage
For every requirement or acceptance criterion in scope, record one or more linked tests and the latest status: passed, failed, blocked, or not run. Count an item as covered only under a stated rule—for example, when a test exercises it and has a recorded result. Keep failures visible: a failed test is evidence that the item was exercised, not that the requirement is satisfied.
- List the current requirements and acceptance criteria in scope.
- Link each item to its tests, including the relevant test level.
- Report unlinked items and items whose tests are blocked or not run.
- Review the basis with stakeholders so the measure does not silently omit expectations.
Formal requirements can also be assessed with criteria applied to their structure. A NASA report discusses requirements coverage, antecedent coverage, and Unique First Cause coverage for Linear Temporal Logic properties (NASA Technical Reports Server: Coverage Metrics for Requirements-Based Testing). These are specialized criteria, not interchangeable with a simple count of acceptance criteria linked to tests.
Track risk-scenario coverage separately
Identify plausible failure scenarios, assess their impact and likelihood using a scale appropriate to the application, and link important scenarios to tests or other verification activities. Report covered high-consequence scenarios and uncovered ones separately. A single percentage can make many low-risk items look reassuring while concealing one severe gap.
Risk-based testing uses analyzed risk to guide test selection and resources. The scoring scale and acceptable residual risk are application-specific; there is no universal scale or threshold in the available standards guidance. State how the team ranks risk and who accepts any remaining exposure. ISO/IEC/IEEE 29119-1:2022 describes risk-based testing as an approach grounded in analyzed risk (ISO/IEC/IEEE 29119-1:2022).

Model user-visible behavior, states, and transitions
For systems where behavior changes with state, enumerate meaningful states and transitions, then connect tests to the transitions they exercise. Examples might include an account moving from pending to active, or a payment moving from authorized to settled or failed. The count is only as useful as the model: a transition omitted from the model cannot appear as a coverage gap.
Validate the model against requirements and real workflows, and revise it as the product changes. ISO’s testing concepts include state-transition testing and specification-based testing of external inputs and outputs (ISO/IEC/IEEE 29119-1:2022). Keep state or behavior coverage distinct from code coverage: executing a line does not show that every meaningful user-visible path through the system was exercised.
Measure input partitions, boundaries, and combinations
Inputs can be partitioned into groups expected to behave similarly, with tests at representative values and important boundaries. For interactions among multiple inputs, decision tables or pairwise combinations may expose cases that single-variable tests miss. Define the model and its scope before calculating a percentage—for example, which input dimensions and values are included and which combinations are deliberately excluded.
ISO/IEC/IEEE 29119-1:2022 includes equivalence partitioning and pairwise testing concepts, while IEEE/ISO/IEC 29119-4-2021 describes test techniques (ISO/IEC/IEEE 29119-1:2022; IEEE/ISO/IEC 29119-4-2021). A high percentage over a narrow or outdated input model is not evidence about values or combinations left outside it.

Use mutation testing to probe test sensitivity
Mutation testing makes small deliberate changes to code or specifications and checks whether the tests distinguish the modified version from the original. NIST gives changing < to >= as an example. A mutation that is detected is often called killed; one that survives needs investigation. Report the mutation scope and operators, and explain how equivalent or otherwise non-meaningful changes are handled.
Interpret the result narrowly: it indicates how the suite responds to the selected changes, not the percentage of real defects it will find. NIST includes mutation testing among developer verification techniques in NIST IR 8397, Guidelines on Minimum Standards for Developer Verification of Software, published October 6, 2021.
Include security testing and exploratory work
Security evidence should extend beyond structural coverage. Track threat-model scenarios and the tests that address them; include fuzzing scope, such as targets, duration, or input space; and record exploratory charters or scenarios completed and any findings. NIST recommends threat modeling, black-box test cases, fuzzing, and attention to included libraries, packages, and services (NIST IR 8397).
Fuzzing generally needs a harness, can be computationally intensive, and often benefits from scale. Exploratory testing can seek hidden properties or behaviors that create failure risk; ISO describes it as a testing approach rather than a substitute for a defined test basis (ISO/IEC/IEEE 29119-1:2022). Report what was actually exercised and for how long, rather than turning exploratory effort into an unsupported coverage percentage.

Keep code coverage as one structural signal
Code coverage helps identify structural elements that tests did not execute, but it does not establish that executed code behaved correctly or that all requirements have tests. NASA’s Software Engineering Handbook says, “Merely achieving 100% code coverage isn’t enough,” explaining that this does not by itself show complete requirements testing or correctness (NASA Software Engineering Handbook, SWE-066: Perform Testing).
The criterion matters. NASA notes that even 100% function coverage does not mean every statement in each function was covered. Report the coverage type and level rather than just a headline number, and read it alongside requirement, risk, and behavior evidence. As the handbook also reproduces the principle, “testing shows the presence of bugs, not their absence.”

A practical report can show separate rows for each dimension. Include scope, numerator and denominator where applicable, status, and limitations so a reader can tell what the measure says.

For every row, state the test level and reporting window. Do not average dimensions into an overall “quality” figure unless a defensible, context-specific method explains the weights and meaning. A model can omit behavior; requirements can be incomplete; mutation results depend on the selected changes; and structural coverage cannot establish correctness. Those limitations belong next to the evidence they qualify.

Decide whether coverage is sufficient for a release
There is no universal percentage for overall test adequacy established by the cited standards and guidance. Set completion criteria for the application: which items must be exercised, which failures block release, which high-risk gaps require mitigation or explicit acceptance, and what exclusions are permitted. Make the criteria visible before reporting results so a team cannot redefine “covered” after seeing the numbers.
- Choose the release’s test basis and keep its version or reporting window identifiable.
- Set measurable, dimension-specific completion criteria and the rule for passing each item.
- Review failed, blocked, untested, and high-risk uncovered items with their owners.
- Record accepted residual risks and exclusions, with rationale.
- Update the basis when requirements, models, interfaces, or included dependencies change.
ISO/IEC/IEEE 29119-1:2022 is informative; ISO says Parts 2, 3, and 4 are normative for those claiming conformance. It also describes tailored conformance where tailoring and its rationale are described and agreed (ISO/IEC/IEEE 29119-1:2022). A team should not imply formal conformance merely because it uses some of the standard’s concepts.
