A finding is a false positive only after review shows that the reported issue does not apply in context; a clean scan is not proof that a site is accessible. The reliable approach is to reproduce the reported state, check the relevant criterion, verify the experience manually, and report what the evaluation did—and did not—cover.
What is an accessibility testing false positive?
The UK Department for Education defines false positives as issues flagged by testing tools that are not actually issues when reviewed manually. That does not mean a report can be dismissed because it looks inconvenient or because a developer disagrees with the tool. Confirm the affected element, the page and interface state, and the applicable accessibility criterion first. See the Department for Education’s guidance on false positives.
It is also important to distinguish a false positive from false assurance. A false positive is an alert that review determines is not an issue. False assurance occurs when a check passes even though a meaningful problem remains—for example, when a simplistic test checks only whether an image has an alt attribute, not whether its text alternative is useful.
Why do accessibility checkers report false positives?
They cannot infer all meaning from context
A tool can identify image alt text or link text, but it may not judge whether that text communicates the relevant information in context. The Department for Education notes that link purpose and image meaning can require human review. A report about an element’s text is therefore a prompt to assess its actual purpose and surrounding content, not a verdict based on text presence alone.

A criterion may have exceptions
A contrast checker might flag a logo’s low contrast. Before treating that as a violation, check the criterion and its exceptions: the cited contrast criterion exempts logos and branding. An exception should be applied only when the element genuinely qualifies; it is not a reason to ignore contrast problems elsewhere.
The scan may evaluate the wrong interface state
Automated engines inspect rendered content. If a menu, dialog, or other region is inactive or not rendered when the scan runs, that state is not evaluated. The axe-core API documentation advises exposing inactive or non-rendered regions before analysis. Test each important state after activating it.
The rules or configuration may not match the evaluation
A tool’s rules, exclusions, supported file types, severity labels, and version may not align with the standards and expectations used in a particular evaluation. Section508.gov advises evaluating how a vendor defines and quantifies rules, including file-type fidelity, customization, ruleset version control, exclusions, severity, context, and workflow integration. Check that fit before interpreting a report as a complete account of compliance.
Why can a scan pass while an accessibility problem remains?
A presence check can confirm that an alt attribute exists without determining whether its value is appropriate. The Department for Education gives examples such as classroom art labeled alt="car" or alt="image123": these can pass an attribute-presence check but fail to provide useful context. By contrast, a decorative image can correctly use an empty alternative, alt="". Whether text is useful depends on the image’s role and the information users need.

Automation also has limits beyond text alternatives. A scan covers the content and states it reaches, and automated checks cannot apply all the human judgment required to evaluate accessibility. The Department for Education says tools identify around 30% to 40% of issues; that figure describes issues identified, not a false-positive rate and not the performance of any particular tool. A blank report should therefore be read as “no findings from these checks in the evaluated scope,” not “the site is accessible.”
How to triage a reported finding
- Record what the scan actually evaluated. Capture the rule identifier, affected element, page or user flow, scan configuration and version, and interface state at the time. This makes the result reproducible and supports a clear evaluation report.
- Reproduce the same state. Open the page and perform the interaction that reveals the content. Activate menus, dialogs, or other relevant controls, then rerun the scan against the rendered state. A scan taken before activation cannot establish what is present afterward.
- Read the relevant criterion and its scope. Decide whether it applies to this element and purpose, and check for a genuine exception. For example, verify whether a reported logo is covered by the logo exemption before classifying a contrast alert.
- Check both the code and the user outcome. For an image, determine whether its alternative text conveys the information needed in context—or whether it is decorative and should have an empty alternative. For a link, decide whether users can understand its purpose in its surrounding context.
- Classify and document the result. Mark it false positive only when review supports that conclusion. Otherwise, fix it or record that it needs further evaluation. If a rule repeatedly raises an inapplicable alert, consider a centrally managed configuration change, but document exclusions and monitor them so recurring results are not silently lost.
- Check for problems the scan may not reveal. Include manual review and assistive-technology testing as distinct parts of the evaluation, not merely a final check of the scanner’s report.
- Report scope and limitations. State what pages, flows, samples, methods, and findings were evaluated, along with gaps that remain. This prevents a clean scan from being presented as proof of total conformance.
How to reduce false alarms without hiding real issues
The goal is a defensible balance between useful precision and meaningful coverage—not a report with zero findings at any cost. Section508.gov warns that automated tools can produce excessive false positives, or, when configured to eliminate them, test only a small portion of requirements. A suppression that makes reports quieter can also make them less informative.
- Align rules to the evaluation. Confirm which standard and ruleset version apply, how the tool defines each rule, and whether supported content types are evaluated faithfully.
- Use exclusions sparingly and transparently. Record why an exclusion is valid, who approved it, and how it will be reviewed. Avoid broad exclusions that hide unrelated elements or recurring issues.
- Include dynamic and authenticated states. Ensure the test process can reach the content and interactions users encounter, and run checks after important states are exposed.
- Prefer actionable reports. Consider whether results include useful context, severity information, and remediation guidance, and whether the tool fits development and CI workflows.
- Use automation alongside human evaluation. Fast, repeatable checks help identify potential issues early; manual and assistive-technology evaluation address questions that a rule cannot settle.
These are tool-selection and evaluation considerations, not a vendor ranking. The reviewed authoritative guidance does not establish a comparable, tool-by-tool false-positive-rate benchmark under a shared corpus and method. Detection coverage and false-positive rate are different measures; do not treat one as evidence for the other.
Use a structured evaluation, not a single scan
W3C’s WCAG Evaluation Methodology (WCAG-EM) 2.0 describes a technology-agnostic process: define the scope, explore the product, select representative samples, evaluate those samples, and report the results. WCAG-EM 2.0 is a W3C Group Note with informative guidance, not a new normative requirement or replacement for WCAG.

The UK Department for Work and Pensions’ guidance likewise describes automated checks as useful early checks alongside manual and assistive-software testing. That is the DWP’s guidance for its context, not a claim that one identical process is a universal legal requirement. Together, these approaches help teams explain what was checked and what a scan cannot establish.
Capture visual evidence of a tested state
A screenshot can help a team document what a page or interaction looked like when a finding was reviewed. It cannot determine whether alt text is meaningful, whether a control works with a keyboard or screen reader, or whether a WCAG criterion is met. Treat it as supporting evidence, not as an accessibility test or conformance verdict.
For a manual workflow, reproduce the issue in the browser, expose the relevant menu or dialog, and capture the rendered state alongside the finding’s rule identifier and page context. Keep the screenshot tied to the state actually evaluated; a static image cannot show every interaction or assistive-technology outcome.

Dismissal based on the alert’s wording
Problem: A team labels a result a false positive without checking the element or criterion. Fix: Reproduce the target state and verify the criterion’s scope and any applicable exception before changing the finding’s status.
Scanning before a menu or dialog is exposed
Problem: The report omits a region that was inactive or not rendered at scan time. Fix: Activate the interaction and rerun the scan on the exposed state, as recommended in the axe-core API documentation.

Suppressing recurring alerts without tracking exclusions
Problem: A broad rule change removes noise but also hides findings that should be reviewed. Fix: Make exclusions specific, documented, centrally managed, and monitored for recurring results.
Claiming conformance from a clean scan
Problem: A report says the site is accessible based only on automated findings. Fix: Describe the actual scope and methods, and add manual and assistive-technology evaluation. A scan result is evidence about the checks performed, not a complete verdict.
Choosing an evaluation tool: questions to ask
- Which standards and ruleset versions does it implement, and how does the vendor define and quantify its rules?
- Which content types does it support, and does it evaluate them in a faithful form?
- Can it reach authenticated pages and dynamic states such as menus and dialogs?
- Are customization, version control, severity labels, and exclusions transparent?
- Does it provide enough context and remediation guidance for a reviewer to assess a result?
- Can it fit into the team’s acceptance tests and CI workflow without replacing manual evaluation?
These criteria follow the considerations in Section508.gov’s overview of testing methods and the state-related guidance in axe-core’s API documentation. They support a reasoned evaluation; they do not establish which vendor has the lowest false-positive rate.
