Yes. Copying clinical-note text can put patients at risk when old, inaccurate, irrelevant, or wrong-patient information is carried forward without careful review. Repeated text can also hide current findings from the clinicians who need them. These are recognized hazards, but the available evidence does not establish a universal rate of harm or show that every copied note causes injury.
How copying a note can affect patient safety
Electronic health records (EHRs) make it easy to reuse text from earlier notes. Often called “copy and paste,” “copy-forward,” or “cloned notes,” the practice can reduce repetitive typing. The safety issue is not reuse by itself: it is whether the copied material is accurate, relevant, clearly attributed, and reviewed for the current patient and encounter.
Old details can appear current
A diagnosis, medication, examination finding, or treatment plan may have changed since it was first documented. If that text is copied forward without checking and editing it, a later note can present an outdated detail as though it describes the patient now. NIST identifies failure to properly review and edit copied information as a common error and discusses interruptions as one circumstance that can contribute to it. NIST’s 2017 human-factors report examines the copy-and-paste function in EHR use.
Missing context makes text harder to judge
A later reader may not be able to tell where copied information came from, who entered it, when it was recorded, or whether it was changed. Without that provenance, it is harder to decide whether the information still applies to the patient or to the current encounter. Both the NIST report and a 2017 systematic review identify attribution and source context as important concerns.

Repeated text can bury the current picture
Copying without a clear purpose can make notes long, redundant, or difficult to follow. Important current findings may be buried among repeated material, making it harder for a clinician to locate timely information and maintain an accurate picture of the patient’s situation. NIST and the Joint Commission identify note bloat and obscured information as safety concerns.
Contradictions and mistakes can spread
Old and new statements may conflict within a note, or an inaccurate statement may be repeated in later documentation. A mistake can also be copied into the wrong patient’s chart, where another clinician might rely on it as true. The systematic review and Joint Commission guidance, updated July 2021, describe internal inconsistency, error propagation, and wrong-chart documentation as hazards.

What the evidence does—and does not—show
Copying clinical notes is common, and research has documented potential hazards. But evidence directly measuring the overall causal effect on patient safety remains limited. The systematic review authors searched 51 publications and concluded that direct evidence of patient-safety risk was sparse and constrained by limitations in study design.

Safeguards for clinicians and health-care organizations
The Partnership for Health IT Patient Safety recommends a set of organizational safeguards: make copied material identifiable, make its provenance readily available, educate staff, and regularly monitor, measure, and assess copying practices. The systematic review presents these recommendations alongside a model for health IT collaboration.
- Identify reused text. Clinicians and later readers should be able to recognize which material was copied rather than newly documented.
- Preserve provenance. Make the source, author, date, and any subsequent changes accessible so readers can judge whether the text applies.
- Train staff in practical review. Guidance should help clinicians verify relevant details and edit or remove information that no longer belongs in the note.
- Monitor practice. Organizations should assess copying patterns and investigate whether local workflows or controls are allowing risky repetition.
NIST also highlights interface design: selected content should be visible to reduce incomplete copying, source and editing information should be exposed, and review and editing should fit efficiently into clinical work. These capabilities and procedures are EHR- and organization-specific; they should not be assumed to exist in every system.

A practical check before reusing note text
- Confirm the chart and encounter. Verify that the destination record belongs to the correct patient and that the text is appropriate for this encounter.
- Check each clinical detail against the present situation. Review diagnoses, medications, findings, and plans rather than treating previously documented text as automatically current.
- Remove what no longer applies. Edit or delete outdated, irrelevant, or conflicting statements so the note reflects the current clinical picture.
- Make the source clear. Preserve or provide enough attribution for later readers to determine where the information came from and when it was recorded or updated.
- Review the finished note as a whole. Check for contradictions, repeated material that obscures important current information, and any signs that text belongs to another patient or context.
These checks reflect the risks and safeguards identified by NIST, the systematic review, and the Joint Commission. They do not substitute for local documentation procedures or EHR-specific controls.
