Elon Musk said excess liquid oxygen vented from Starship’s upper stage caused a fire and explosion late in its November 18, 2023 test flight. That is Musk’s explanation—not, on the available reporting, an independently confirmed finding about the failure.
What happened during Starship Flight 2?
SpaceX’s second Starship integrated flight test ended in two separate explosions during an approximately eight-minute flight, according to Gizmodo’s 2024 account. The Super Heavy booster failed shortly after stage separation; about five minutes later, the Starship upper stage exploded.

What did Musk say caused the upper-stage explosion?
At a January 11, 2024 all-hands meeting at Starbase, Musk said Starship had no cargo and therefore carried excess propellant. He said the vehicle vented liquid oxygen near the end of its engine burn, and that the venting ultimately led to a fire and explosion.
“Flight 2 actually almost made it to orbit,” Musk said. Had Starship been filled with cargo, “it would have made it to orbit because the reason that it actually didn’t quite make it to orbit was we vented the liquid oxygen, and the liquid oxygen ultimately led to a fire and an explosion.”

The quotation and explanation were reported by Gizmodo. They describe Musk’s account of the upper-stage loss; they do not specify the precise sequence by which vented oxygen ignited or caused the fire.
Was Starship supposed to reach orbit?
No. The planned Flight 2 profile was a long suborbital test, according to Gizmodo’s report. Musk’s phrase “almost made it to orbit” was his characterization of how far the vehicle got, not a description of the mission’s planned objective.

Is Musk’s explanation a confirmed cause?
The cited account reports Musk’s explanation, but does not provide a detailed, FAA-accepted final report establishing the upper-stage event’s root cause. The exact ignition pathway remains unresolved in the material available here. The booster’s failure was a separate event, and its mechanism is not established by Musk’s explanation of the upper-stage explosion.

The FAA says mishap investigations identify root causes and corrective actions; return to flight depends on public-safety findings and implementation of relevant measures. The FAA archive also records investigations into later Starship flights, including Flights 7, 8, and 9. Those are separate mishaps and should not be treated as findings about Flight 2.
