Brain-computer interfaces (BCIs) turn measured brain activity into commands for a computer, speech system, or assistive device. In research, some systems have supported communication or robotic-limb control for people with severe disabilities. But they do not read unrestricted thoughts or offer effortless control of arbitrary machines: each system is designed for specific tasks, and performance, setup, training, and medical burden all matter.

What is a brain-computer interface?

A BCI measures patterns of neural activity and maps them to a limited set of commands. The output might select an item on a screen, operate an assistive device, or help produce speech. The interface is the full chain—from sensing brain activity to interpreting it and acting on the result—not a direct window into everything a person thinks.

Some systems use implanted electrodes to record signals from inside or near the brain. Others use non-invasive sensors, including scalp electrodes that record electroencephalography (EEG). These approaches differ in how they collect signals, the medical burden involved, and the tasks they are designed to support. There is no controlled quantitative head-to-head comparison in the cited sources that establishes one approach as universally better.

What can BCIs do in research?

Communication and computer access

Clinical research has explored BCIs as a communication pathway for people who cannot reliably speak or move, including systems that let users select or generate outputs through a computer. A 2024 U.S. Government Accountability Office (GAO) assessment described clinical-trial systems intended to help people with severe disabilities communicate and control robotic limbs. GAO reported that these clinical-trial systems were not yet on the market when its assessment was published on December 17, 2024; that date-bound finding is not a live inventory of device availability in 2026.

Robotic limbs and rehabilitation

Other research investigates control of robotic limbs and rehabilitation. These are distinct goals: restoring a communication pathway is not the same as controlling movement, and results for one task do not establish that a system can perform another. GAO also described possible nonmedical settings such as workplaces, defense, and entertainment, but those possibilities should not be confused with demonstrated clinical benefits.

Attempted speech and inner speech

A Stanford-led study summarized by the U.S. National Institutes of Health (NIH) on September 9, 2025 examined four people with speech impairment due to ALS or stroke. Researchers recorded motor-cortex activity while participants attempted to speak or imagined words. NIH reported that the two kinds of speech produced similar patterns, with stronger average signals during attempted speech. As the NIH summary puts it, “The findings suggest that attempted speech and inner speech are similarly represented in the brain’s motor cortex.” This describes that study, not a universal rule for every person or BCI.

In real-time inner-speech decoding, the study reported error rates of 14%–33% with a 50-word vocabulary and 26%–54% with a 125,000-word vocabulary. Those figures apply to the four participants and the study’s particular conditions; they do not show that a general-purpose speech product is ready. In one strategy tested to limit unintended decoding, an “unlock” keyword was recognized more than 98% of the time. The study, published online in Cell on August 14, 2025, also explored suppressing inner speech while decoding attempted speech. These are experimental safeguards, not proof that the privacy problem is solved.

How do implanted and non-invasive BCIs differ?

The comparison is qualitative, not a controlled performance ranking. The FDA-NIH workshop on implanted BCI outcomes, held September 19–20, 2024, emphasized robust, standardized measures that generalize to home environments and reflect meaningful communication or motor control—not just performance under laboratory conditions.

Can a brain-computer interface help someone who cannot speak?

That is one of the goals of clinical BCI research, and speech neuroprostheses are an active area of investigation. The 2025 inner-speech result is promising as a research finding, but it involved four participants and substantial decoding errors that changed with vocabulary size. It should not be read as evidence that an off-the-shelf system can reliably translate a person’s thoughts into fluent speech.

Whether a BCI is useful also depends on the person’s priorities and the work required to use it. A systematic review of 28 studies covering preferences from 1,701 patients found that people with motor impairments prioritized accuracy; in the four studies that ranked performance characteristics, accuracy ranked first each time. ALS participants typically emphasized communication, while people with spinal cord injury emphasized limb control and sphincteric functions. The review cautioned that reported speed and accuracy depended on training and setup burdens that most patients would not tolerate.

Can you control a computer with your thoughts?

In a limited, task-specific sense, research BCIs can map measured brain activity to computer selections or device commands. That is different from unrestricted “thought control.” Systems are trained or configured around particular signals and actions; the cited inner-speech study used defined vocabularies and reported meaningful error rates. A command interface should not be mistaken for a system that can decode any thought on demand.

Are consumer EEG headsets the same as clinical BCIs?

No. Consumer EEG products may be marketed for control, wellness, or focus, but they are not equivalent to implanted systems being studied for severe disability. In a 2024 presentation, the National Institute of Mental Health (NIMH) discussed reliability concerns and limited evidence for wellness benefits in consumer applications, as well as privacy issues and gaps between some company claims and available evidence. Those observations describe that presentation’s context and should not be generalized to every current product.

A headset that detects signals for a demonstration or hobby project is not thereby a validated medical communication aid or treatment. For any product, assess the specific intended task, the evidence supporting the claim, the setup required, and how the company handles neural data.

Are brain-computer interfaces safe?

There is no single safety answer for every BCI. An implanted system entails surgery and ongoing device-related medical considerations; a scalp EEG system avoids implantation but still raises questions about task reliability, usability, and data handling. Safety and benefit need to be evaluated for the particular device, user, and intended task rather than inferred from the label “BCI.”

For implanted systems, the U.S. Food and Drug Administration (FDA) issued final guidance on May 20, 2021, addressing implanted BCI devices for patients with paralysis or amputation. Guidance explains a regulatory framework; it does not mean that a named device is approved for general sale. The FDA-NIH 2024 workshop separately focused on how to assess clinical benefit using outcomes relevant to real-world use.

What practical and privacy issues remain?

Training, setup, and meaningful outcomes

A system’s laboratory performance may not reflect whether someone can use it reliably at home. Training time, setup burden, accuracy for the task the person values, and the support needed to operate the system are part of its real-world usefulness. The patient-preference review’s findings make accuracy and tolerable setup especially important considerations, rather than secondary usability details.

Neural data and unintended decoding

GAO’s 2024 assessment identified unresolved questions about who owns and controls brain data. Inner-speech research makes the concern concrete: decoding imagined words could reveal speech a person did not intend to say aloud. The 2025 study tested an unlock-keyword strategy and an approach that suppresses inner speech while decoding attempted speech, but neither should be treated as a complete solution.

Maintenance and support after a trial

GAO also highlighted uncertainty about long-term maintenance, post-trial support, and Medicare or private-insurance coverage. If a trial ends without funding or medical support, a participant could lose access to the device’s benefits. These questions affect whether a promising research system can remain useful beyond the study period.
