To add image choices to a WordPress form, use a form plugin that supports them, then add or configure a choice field and assign an image and text label to each option. The steps depend on the plugin: some provide a dedicated Image Choice field, while others add images to radio-button or checkbox choices.
Choose the right kind of image-choice field
First decide how visitors should answer. A form may allow one selection, several selections, or a specific number or range. Then check whether your existing form plugin supports that behavior with images; the feature is part of the form plugin, not a WordPress core setting established by these product guides.

These are product-specific behaviors, not universal WordPress rules. If you need to retain a particular crop or layout, check how that plugin handles images before preparing the full set.
Add image choices in WPForms
- Install and activate WPForms, and verify the license provides the feature.
- Create a form or open one to edit in the WPForms builder.
- Add a supported Multiple Choice, Checkboxes, Multiple Items, Checkbox Items, or Ranking field.
- Open the field options and enable image choices.
- For each choice, use Upload Image to select an image from the Media Library or upload one, and enter the option’s text label.
- Choose a layout: One Column, Two Columns, Three Columns, or Inline. Inline is the default when image choices are enabled; WPForms recommends it in most cases because it adapts to different screen sizes.
- Preview the published form to confirm the actual images, labels, selection behavior, and layout look right. The editor’s placeholder images are not shown on the published form.
WPForms says choice images are not cropped or resized when added and recommends keeping them consistently sized, at 250 × 250 pixels or smaller. If the field is used to take payments, configure a payment integration such as Stripe, PayPal, Authorize.Net, or Square; selecting an image option alone does not process a payment. Add image choices in Gravity Forms
- Add the Image Choice field from Standard Fields.
- Open Edit Choices, then add each choice’s label and image.
- Set whether visitors can choose one option, multiple options, an exact number, or a range.
- Keep a descriptive text label for each image. If you hide choice labels, account for the accessibility warning in the field documentation.
- If labels contain HTML or special characters and the form uses calculations, conditional logic, dynamic population, or validation, enable Show Values and give each choice a simple, unique value.
- Preview the form and verify the crops and labels. Images uploaded through this field’s interface are cropped to a 1:1 aspect ratio. For images already in the Media Library, or images not uploaded through that interface, the documentation says a WordPress plugin may be needed to resize them.

Gravity Forms also lists a separate Image Choices add-on for adding images to radio and checkbox fields in contexts including Survey, Quiz, Poll, Product, and Options. It is supported by its developer and is purchased or downloaded from that developer; do not assume it is the same as the dedicated field or included with every license.
Add image choices in Formidable Forms
- Add or edit a Radio Button or Checkbox field.
- Set Display Format to Images.
- Choose an image size, then upload or select an image for each option and provide its text label.
- Adjust styling if needed, including border, selected or hover color, border width, and corner radius.
- Preview the form and check the choices on the front end. Labels are required for each option even if the form hides them from visitors.
Formidable documents square image sizes of Small 150 px (the default), Medium 250 px, Large 320 px, and Extra large 400 px. Image choices are a Premium Feature. The feature is unavailable in Lookup and Dynamic fields, does not support the bulk editor, and is incompatible with Add Other options. In plain-text emails, the selected labels appear without images. Check accessibility and behavior before publishing
An image can make a choice easier to recognize, but it should not be the only way a visitor can understand the option. Use a concise, meaningful text label for every choice and keep it associated with the image. This is especially important for visitors who use assistive technology and for anyone who cannot distinguish an image clearly.

- Check that each image corresponds unambiguously to its text label.
- Preview the published form on a narrow screen as well as a desktop; editor previews and placeholders may differ from the live form.
- Verify image crop, sizing, and layout in the plugin you use. Do not assume another plugin’s resizing behavior applies.
