Quiz and Survey Master (QSM), Quiz Maker by AYS, and form plugins such as Fluent Forms Pro support different combinations of question formats, scoring, reporting, and data collection.
1. Decide what the quiz must do
Choose the quiz’s purpose before choosing a plugin. A right-answer knowledge test needs correct-answer scoring and explanations; an exam may also need a pass mark; a personality quiz needs outcome bands or branching; a survey may need no right answers; and a lead-generation quiz needs a deliberate contact-capture step.
- Knowledge quiz: mark correct answers and show a score, explanations, or both.
- Exam or assessment: define points, a pass threshold, and pass/fail messages.
- Personality or outcome quiz: map score ranges or answer patterns to result messages.
- Trivia: combine fixed answers with immediate or final feedback.
- Survey: collect opinions without treating responses as right or wrong.
- Lead-generation quiz: request only the contact details needed for the stated follow-up.
This decision determines the question formats, result rules, and data features you actually need.
2. Choose a WordPress quiz tool
Compare plugins on the capabilities that affect your quiz rather than on a universal “best” label. Feature availability and free-versus-paid boundaries can change, so confirm the current plugin listing before committing.

The WordPress.org quiz directory also lists H5P and Opinion Stage, among other options. A hosted service can reduce WordPress-side setup, but it creates a different workflow from a native plugin and may involve a separate account or service terms.
3. Install and open the plugin
- In WordPress, open Plugins → Add New.
- Search for the selected plugin, review its current compatibility, update history, feature boundaries, and documentation, then install and activate it.
- Open the plugin’s menu and identify where it creates questions, quizzes, results, entries, and settings.
QSM’s WordPress.org listing describes a free core with advanced capabilities available through Pro. AYS likewise separates some reporting and custom-field capabilities into premium features. Do not assume that a basic quiz requires a paid plan.

4. Create the questions and answers
Quiz Maker by AYS workflow
- Open Questions in the AYS interface.
- Add a question and select its question type.
- Enter the possible answers.
- For a scored question, mark the correct answer or answers and assign the appropriate value when the interface provides that control.
- Save the question before adding the next one.
Design questions that work interactively
- Write one unambiguous task per question and keep answer choices parallel.
- Use radio buttons when exactly one answer is correct; use checkboxes only when multiple answers should be accepted.
- Use text, number, date, matching, or ranking formats only when you can define how responses will be evaluated.
- Add answer explanations or feedback when the quiz is intended to teach, not merely classify.
- Check keyboard navigation, labels, and readable contrast in the published quiz.
5. Assemble the quiz and configure results
Create the quiz record, give it a clear title and description, and select the saved questions. Then configure the result behavior that matches the quiz purpose.
Scored tests and exams
- Set the points assigned to each question.
- Choose whether the result is shown as points, a percentage, a grade, or a pass/fail state.
- Define the pass mark and write separate pass and fail messages if the plugin supports them.
- Decide whether participants see correct answers or explanations immediately, after submission, or not at all.
Personality and outcome quizzes
- Write distinct result bands or messages rather than displaying an unexplained number.
- Make the score ranges non-overlapping and ensure every possible score has an outcome.
- Explain what each result means and provide a useful next step instead of a bare label.
QSM documents score-range results. AYS documents percentage results, pass/fail options, and answer feedback. The exact control names and available rules depend on the plugin version.

6. Add the quiz to a WordPress page or post
Using the QSM block
- Open the target page or post in the WordPress editor.
- Insert the QSM block.
- Select the quiz you assembled and save or publish the page.
QSM’s listing states that a quiz or survey can be added to a post or page with its block; use the plugin’s current interface if labels differ.
Using the AYS shortcode
- Open the AYS quiz list and copy the shortcode for the finished quiz.
- Edit the target page or post.
- Paste the shortcode into a Shortcode block or the location supported by your editor.
- Save or publish, then open the public URL to review the complete flow.
Do not judge the quiz only from the editor. Check the start screen, every question type, validation messages, submission behavior, result display, and the layout on a phone-sized screen.

7. Decide what data to collect
Contact capture is optional, not a requirement for an interactive quiz. QSM documents lead capture and reporting. AYS documents optional name, email, and phone fields in its free version, with more custom fields and reporting in Pro.
- Ask for a field only when it supports a stated purpose, such as sending a requested result or follow-up.
- Tell participants what will happen to their responses and how long the site retains them.
- Review the selected plugin’s current privacy documentation and your site’s privacy policy before enabling storage or email notifications.
- Limit administrator access to response data and remove exports or entries that are no longer needed.
Plugin listings describe features; they do not determine the privacy or data-protection obligations that apply to your organization or jurisdiction.

8. Plan for reporting, integrations, and paid features
A simple self-scoring quiz may work with a free plugin. Advanced reporting, integrations, certificates, custom fields, or other premium functions may require an upgrade. QSM identifies Pro as the route to advanced reporting and integrations; AYS describes premium reporting and custom fields. Check the live feature list and license terms because these boundaries can change.
Use reporting only when you have a clear decision to make from the data—for example, identifying missed topics or measuring completion. If you need a hosted quiz, evaluate the service’s account, embedding, export, and retention model separately from WordPress.

The quiz does not appear
- Confirm the plugin is activated and the block or shortcode references the correct quiz.
- Update the page and test its public URL while logged out.

Scores or outcomes are wrong
