Short answer: choose an MCP server when an AI host should reuse tools or data across compatible clients such as Claude Code, VS Code, Cursor, or a custom application. Choose a code extension when the feature must use one editor’s APIs, native interface, settings, or marketplace distribution. They are complementary: a VS Code extension can register and configure an MCP server while adding editor-specific UI.

What is the difference?

MCP and extensions operate at different layers. The Model Context Protocol (MCP) is an open standard that lets AI applications interact with external tools and services through a unified interface. An MCP server implements that protocol and publishes capabilities; an MCP host (an AI application or editor integration) connects to it.

A code extension is a package installed into a particular editor. It uses that editor’s APIs to add commands, views, language features, settings, authentication flows, or other native behavior. Its installation, update, permission, and distribution model belong to the host editor.

Portability is an advantage, not a guarantee. The MCP TypeScript SDK lists Claude Code, VS Code, Cursor, and custom applications as compatible hosts, but each client may implement a different subset of the specification.

When an MCP server is the better choice

One integration, several AI clients

Build an MCP server when the same capability should be available from more than one host. A database-query server, repository operation service, ticketing connector, or external API can then be consumed by every client that supports the relevant MCP features instead of being rewritten for each editor.

Access to systems outside the editor

MCP is a natural boundary for services that are not inherently editor features: querying data, retrieving records, calling an API, or running a controlled repository operation. The server can expose narrowly scoped tools and return structured results to the model.

Tools, resources, and prompts belong together

Depending on host support, an MCP server can expose callable tools as well as resources and reusable prompts. More advanced implementations may use elicitation for user input, sampling, OAuth authentication, workspace roots, or MCP Apps. Verify support in the target client before depending on any of these capabilities.

Example server configuration concept

Client configuration differs, but an MCP entry normally identifies a command for a local server or an HTTP endpoint for a remote one. Treat the following as a shape to adapt to the host’s current documentation, not a universal file format:

Keep secrets in the host’s supported secret store or environment mechanism; do not commit tokens to a workspace file.

When a code extension is the better choice

Deep editor APIs and native UI

Use an extension when the feature depends on editor state or interfaces: custom views, commands in the command palette, language-server integration, decorations, inline actions, workspace events, or tightly integrated settings. An extension can react to editor events and render controls that a protocol-only integration cannot provide.

Marketplace distribution

If your goal is to package updates, documentation, activation behavior, and discoverability through the Visual Studio Marketplace, VS Code’s guidance points toward an extension. The marketplace also gives users an editor-native installation and update path.

Editor-specific language-model features

VS Code provides a Language Model API for tools that need close integration with VS Code. That option is appropriate when the value depends on VS Code’s own context, UI, or lifecycle rather than on a generally reusable external service.

They are not mutually exclusive

A common architecture places reusable business logic behind an MCP server and ships a thin extension that improves installation and editor experience. VS Code supports extension providers that register local stdio or Streamable HTTP MCP server definitions. An extension can collect configuration, show status, initiate authentication or other user interaction during resolution, and then let the host invoke the server.

  1. Implement the service and tool contracts in an MCP server.
  2. Choose a transport: local standard input/output (stdio) for a process on the user’s machine, or Streamable HTTP for a network service. Legacy server-sent events (SSE) may still appear in host documentation.
  3. Build an extension only for editor commands, views, setup, policy, or distribution that MCP itself does not provide.
  4. Register the server through the editor’s supported provider API and document which capabilities the target clients actually support.

This split avoids rewriting the core integration for every host while preserving a polished editor experience.

Transport, deployment, and security decisions

Where does the code run?

In VS Code, a configured server runs where it is configured. User-profile servers normally run locally; remote development can require configuration in workspace or remote-user settings so the process runs in the intended environment. Confirm this before granting filesystem or network access.

Local stdio controls

For locally running stdio servers on macOS and Linux, VS Code documents sandbox configuration that can restrict filesystem and network access. Use the narrowest paths and connections required by the server.

Authentication and confirmation

Remote servers may use OAuth. Review the authorization flow, token storage, scopes, and redirect behavior. VS Code also prompts for confirmation before tools that are not marked read-only; do not treat that prompt as a substitute for reviewing the server’s implementation.

Extension privilege is different

An extension can access whatever the editor’s extension API and permissions allow, while an MCP server exposes the tools its author implements. Audit both layers when they are combined: commands, network destinations, workspace files, credentials, and actions that can change data.

Practical choice checklist

  • Choose MCP first if the capability is an external service, must serve several AI hosts, or should remain independent of one editor.
  • Choose an extension first if it needs native panels, editor events, language features, marketplace delivery, or a deeply editor-specific workflow.
  • Choose both when users need a reusable service plus guided setup, configuration UI, or editor commands.
  • Check the host for supported transports, tools, resources, prompts, OAuth, sampling, confirmation behavior, and workspace-root semantics before promising a feature.

Common implementation and troubleshooting problems

The server does not appear

Check the configuration scope (workspace versus user profile), command path, working directory, and host logs. In remote development, verify that the configuration is on the remote side where the process should run.

Connection or transport errors

For stdio, ensure the process writes protocol messages to stdout and sends diagnostic logging elsewhere. For Streamable HTTP, verify the endpoint, TLS certificate, firewall, and authentication flow. Do not assume an SSE example works in a client that has moved to Streamable HTTP.

A tool is missing

The client may not support that capability, or the server may not advertise it. Inspect the client’s current MCP implementation and the server’s initialization response. Resources, prompts, sampling, elicitation, and MCP Apps are not universally available.

Authentication loops or denied actions

Recheck OAuth redirect configuration and scopes, then inspect whether the host requires confirmation for a write-capable tool. Marking a tool read-only should reflect its real behavior, not merely suppress prompts.

The extension works locally but not for teammates

Document required runtimes, environment variables, workspace settings, network access, and platform differences. If the server is local, each user needs the runtime and permissions; if remote, define ownership, authentication, and update procedures.

Performance, reliability, and maintenance

Keep tools narrow and predictable: validate inputs, return bounded results, and expose pagination or filters for large datasets. Set timeouts around external calls and report actionable errors. A server shared by several hosts needs versioned tool schemas and backward-compatible changes. An extension needs activation-time discipline so it does not slow editor startup.

Test the same server with every target client. Differences in streaming, confirmation, OAuth, resource handling, and prompt support can change the user experience even when the protocol connection succeeds. Use MCP for portable access to external capabilities, extensions for editor-native behavior, and both when setup and UI belong in the editor but the underlying service should remain reusable. Decide from the integration boundary, then verify the exact host’s support, deployment location, authentication, and safety controls.
