A virtual medical visit is a clinical encounter in which a patient and a healthcare professional communicate from different locations using electronic technology. It may happen live by video or phone, or through information a patient sends for a clinician to review later.
What counts as a virtual medical visit?
The defining feature is remote communication for a health concern or care—not a particular device or app. A video appointment is one familiar example, but a remote encounter may also use audio, messaging, or submitted images. The U.S. Department of Health and Human Services (HHS) describes telehealth technologies as including videoconferencing, internet, store-and-forward imaging, streaming media, and landline or wireless communications. Its broader definition also covers health-related education, public health, and administration, so not every activity called telehealth is itself a medical visit. HHS: What is telehealth?
Does a virtual visit have to be on video?
No. The format depends on how the patient and clinician exchange information. HHS notes that telehealth can use audio, text messaging, or video communication, including videoconferencing software. Whether a particular provider or payer accepts a given format is a separate question from whether the interaction is virtual care. HHS: Getting started with telehealth
Live visits (synchronous)
In a synchronous visit, the patient and clinician interact at the same time. Examples include a live video appointment or an audio-only call. The clinician can ask questions and respond during the conversation.

Delayed exchanges (asynchronous)
In an asynchronous exchange, the patient and clinician do not communicate at the same time. A patient might send information or images, which the clinician reviews later. This is often described as “store and forward.” It is remote care, but it is not a live appointment.
Virtual visit, telehealth, and telemedicine: what is the difference?
These terms overlap, and their meaning can depend on the context. The American Medical Association describes telemedicine as historically centered on remote clinical services, while telehealth can refer to a broader range of synchronous and asynchronous services. In everyday patient-facing use, “virtual medical visit” usually means a remote clinical encounter; “telehealth” may cover that visit and other technology-supported health services. Is remote monitoring itself a virtual visit?
Not necessarily. Remote patient monitoring involves collecting and transmitting patient data from outside a clinic so it can inform clinical decisions or follow-up. Monitoring may be part of a broader care plan, but monitoring by itself is not automatically a visit. The AMA notes that CMS does not currently classify remote monitoring as telehealth for its purposes; that program-specific terminology should not be mistaken for the broader everyday meaning of telehealth.

HHS describes direct-to-consumer telehealth examples that include acute-care assessment, primary care, and behavioral health. These examples show that virtual visits can serve different kinds of care; they do not mean every condition is appropriate for remote assessment. Does “virtual visit” mean Medicare will cover it?
No. A general definition of a virtual medical visit is not the same as a payer’s billing or coverage category. CMS materials distinguish Medicare telehealth visits—which they describe as real-time interactive communications with audio and visual capability—from services such as virtual check-ins and remote evaluation of submitted images. These are Medicare-specific service and payment distinctions, not a universal definition of virtual care. How to identify the kind of virtual care being offered
- Timing: Is the interaction live, or will the clinician review information later?
- Communication method: Is it by audio, video, messaging, or submitted images?
- Service type: Is it a clinical encounter, a brief check-in, or ongoing monitoring?
- Coverage: Which current payer and jurisdiction rules apply to this specific service?
