If VirtualBox says it is unable to insert a virtual optical disk, first check the full error message, the selected VM, and the ISO’s path. The same phrase can point to different issues: an image that is unavailable, an optical drive that is already occupied or locked, or a VM configuration problem. These checks apply to Windows 11 hosts, but the reported error cases are not established as Windows 11-specific.
What the error does—and does not—tell you
“Unable to insert the virtual optical disk” is not a diagnosis by itself. Note the complete message and any error code before changing the VM. A VirtualBox support-forum report includes VERR_PDM_MEDIA_LOCKED and “Could not mount the media/drive”; a separate VirtualBox project ticket describes a duplicate Guest Additions ISO UUID conflict. Record your VirtualBox version and guest operating system as well as the Windows 11 host. The available documentation describes general VirtualBox media workflows; it does not establish that the forum and ticket cases occur only on Windows 11.
Work through the least-disruptive checks first

Capture the complete error
Write down the whole message and code, if shown. Check whether it names an ISO path, says the medium is locked, or mentions a UUID. Those details determine which branch to investigate.

Confirm the VM and optical-drive state
Make sure you selected the intended VM, then inspect its storage settings or Virtual Media Manager to see whether an ISO is already attached or the optical drive is empty. Verify that the ISO is still available
Check that the host file exists at the path VirtualBox is trying to use and that Windows can access it. If the ISO was moved, renamed, or stored on an unavailable location, select its current location through VirtualBox’s image chooser.

For Guest Additions, use the documented menu
With the intended VM selected, choose Devices > Insert Guest Additions CD Image from the VM menu bar. Oracle’s VirtualBox 7.0.0 manual says this action mounts the Guest Additions ISO inside the VM. On a Windows host, that manual says the ISO is usually in the VirtualBox installation directory, commonly
C:Program FilesOracleVirtualBox. If the menu action is unavailable or fails, check whetherVBoxGuestAdditions.isois present there, then select it using the VM’s optical-drive image chooser. If the message says the medium is locked, inspect its state
VERR_PDM_MEDIA_LOCKEDappeared in a reported Guest Additions insertion problem, but that report does not establish one cause or a guaranteed remedy. Check whether the ISO is already mounted or the virtual optical device is in use. Only after confirming its state should you try detaching and reinserting the image. If the message mentions a duplicate UUID, check VM-file access
Confirm that any external, removable, or encrypted location containing the VM configuration is mounted and accessible to Windows. A VirtualBox project ticket documents a particular duplicate Guest Additions ISO UUID scenario involving temporarily inaccessible VM configuration files, with restoring access described as a workaround. Check the optical-drive slot before using VBoxManage
Command-line media attachment depends on the VM’s storage controller and the optical device’s port and device values. Identify those settings and the exact VM name and ISO path before making a change. The older VBoxManage manual excerpt describes an empty drive, the Guest Additions image, or a filename for an existing image as media choices; it also notes that some removable-media changes can be made while a VM runs, while other storage changes require it to be powered off. Do not assume its syntax or runtime behavior applies unchanged to your installed version. Consult the current manual and verify the VM’s storage configuration before using a command. Collect diagnostics before considering manual configuration edits
If these checks do not explain the error, preserve the complete message and inspect the VM’s configuration and logs before attempting a manual edit. Do not delete VM files or edit the
