A dropped frame becomes a clinical problem when choppy, frozen, or interrupted video—or accompanying audio problems—keeps a clinician from reliably observing or communicating information needed for that particular assessment. There is no established universal frame-loss count, percentage, or duration that marks the threshold. The practical test is whether the connection still supports the clinical task.
Why the clinical task matters more than the frame count
A brief stutter during a conversation that does not depend on continuous visual observation may have little consequence. The same disruption can make an assessment inadequate if it obscures movement, appearance, or another detail the clinician needs to evaluate. This is a task-dependent judgment, not a measured frame-loss threshold.
Video is one part of an assessment, not a replacement for every element of an in-person examination. The Agency for Healthcare Research and Quality (AHRQ) identifies remote-care limits such as physical findings that may be difficult to see and reduced participation by onsite team members. It also notes potential benefits, including easier access to clinicians, family involvement, and the ability to bring multiple clinicians into a difficult case. Those are telehealth considerations generally; AHRQ does not attribute them specifically to dropped frames. AHRQ’s telediagnosis material describes these trade-offs.
What dropped frames can do to a telehealth visit
Frame loss is one possible sign of a degraded connection, and it may occur alongside other problems. The National Telehealth Technology Assessment Center defines packet loss as missing data packets that may result in frozen video, garbled audio, or dropped calls. Its 2026 Clinician’s Guide to Video Platforms recommends evaluating more than resolution: frame rate, motion handling, color, compression, latency, jitter, stability, reconnection behavior, and quality monitoring can all affect usability.

Consequently, a video image that looks sharp when still may not be adequate when the clinician needs to assess motion. Audio matters too: if speech becomes garbled, out of sync, or unavailable during a visual assessment, the encounter may no longer support clear communication.
What to do when the video degrades during an encounter
- Check whether the information needed for the assessment is still clear. Consider whether the clinician can reliably see the relevant movement or appearance and hear the patient or care team.
- Stop relying on an impaired observation. If the needed information cannot be assessed, pause and repeat the observation after the connection recovers, or move to an appropriate alternative channel or in-person workflow.
- Escalate time-sensitive care through the service’s established plan. Do not assume that degraded video is sufficient when the assessment cannot be completed reliably.
- Document the disruption for follow-up. Note whether it was isolated or recurring, whether audio was affected, whether motion could still be assessed, and the network conditions at the time.
This is a practical synthesis of the guidance, not a universal protocol. Clinical services should define their own protocols, monitor outcomes and complaints, test equipment, and plan for downtime.

How services can test and prepare for connection problems
The 2026 National Telehealth Technology Assessment Center guide recommends testing where and how the service will actually be used. That can include wired Ethernet, typical Wi-Fi, mobile or mixed-use networks, and locations with known coverage challenges. Testing should examine whether the platform maintains a session through brief interruptions, reconnects, signals degraded quality, and preserves audio if video quality must be reduced. Services can also monitor call quality and drop rates.
Wired connections may be more stable and predictable in some clinical environments, particularly where many devices share the network. A wired-connection test may therefore be useful where equipment and organizational policy permit, but a cable is not a guaranteed fix for a particular disruption.

System selection should be based on the clinical tasks it must support. Useful evaluation points include image and motion clarity, audio intelligibility and synchronization, latency and behavior under packet loss, recovery from interruption, quality monitoring and reporting, bandwidth and device compatibility, security, and documented downtime workflows.
Additional requirements for telestroke
Acute telestroke has specific technical guidance: the American Telemedicine Association calls for real-time, two-way audiovisual solutions with minimal latency, sufficient bandwidth tested during expected hours, and redundant connectivity. It identifies high-quality pan-tilt-zoom cameras as ideal for acute care interactions. These recommendations are specific to telestroke, rather than universal requirements for every telehealth visit. The guideline does not set a numerical acceptable frame-loss threshold. See the American Telemedicine Association telestroke guideline.

What the safety evidence can—and cannot—tell you
Evidence about telehealth safety overall is not direct evidence about harm caused by frame loss. AHRQ’s 2023 rapid evidence review screened 7,155 citations and included 23 studies. Findings on preventable hospitalizations and emergency department visits were mixed, and the review found no studies evaluating specific patient-safety practices intended to reduce harms associated with real-time telehealth. Those figures describe the review and its broader evidence base; they are not estimates of frame-related clinical harm. Read AHRQ’s 2023 rapid evidence review.
