Colorlib’s 10 free HTML survey form templates cover customer ratings, employee pulses, event feedback, course evaluations, and other common survey patterns. They use plain HTML, CSS, and vanilla JavaScript, so Bootstrap and jQuery are not prerequisites; “Bootstrap-ready” means the styling can be used in Bootstrap projects, not that Bootstrap is bundled. The important limitation: the forms are front-end demos, and answers are not collected until you connect a submission endpoint.

What the ten templates are designed to do

Colorlib’s roundup, published September 25, 2026, describes ten templates identified as V01 through V10. These are different interaction patterns, not a tested ranking: the page reports no controlled comparison or measured completion-rate results.

V02 — Conditional NPS follow-up A 0–10 score reveals a follow-up whose wording, color, and label vary for detractors, passives, and promoters. In-app prompts, SaaS dashboards, and relationship surveys.
V03 — Multistep survey Four pages, progress display, back and next navigation, validation, retained answers, and a review page with edit links. Longer research questionnaires or onboarding.
V04 — Product feedback Emoji reaction scale, feature checkboxes, notes, and optional screenshot upload with file-type and size checks and a preview. Beta testing and in-app product feedback.
V05 — Event feedback matrix Rating columns for each session, a “Didn’t attend” choice, and mobile layouts that turn rows into labelled cards. Conference, workshop, and course feedback across several sessions.
V06 — Employee pulse Five Likert statements, anonymity enabled by default, optional identity fields when anonymity is switched off, and an answered-item counter. HR pulse checks, retrospectives, and engagement surveys.
V07 — Instant poll One question followed by result bars and counts; the displayed counts are simulated in the script. Blogs, newsletters, and community pages where a demo poll is sufficient.
V08 — Conversational survey One question at a time, keyboard shortcuts, a progress display, and up/down navigation. Short onboarding, lead qualification, or informal check-ins.
V09 — Course evaluation Four range sliders with live value badges and spoken aria-valuetext, open questions, and a completion recap. Course and training feedback.
V10 — Exit-intent feedback A checkout feedback card with reason checkboxes, an optional note, and focus-return behavior. Checkout, pricing, cancellation, and abandoned-cart research.

Colorlib describes the mobile and accessibility behaviors as implementation details; they should not be read as independent usability or accessibility test results. For example, V03’s navigation panel becomes a compact row on phones, V05 changes matrix rows into cards, V01 and V02 use radio groups for ratings, and V08 includes keyboard controls.

How to choose a template

Start with the question you need answered and the moment you will ask it. A single score with a tailored follow-up points to V02; a long questionnaire with review and editing points to V03; ratings across multiple event sessions fit V05; and a short interruption at checkout is the purpose of V10. Match the audience as well: V01 targets customers after a purchase, V06 employees, V09 course participants, and V04 product users.

  • Choose a short, focused interaction when a person has only one clear response to give: V01, V02, V07, or V10.
  • Choose a longer or structured interaction when you need multiple sections, session-by-session ratings, or several kinds of answers: V03, V05, or V09.
  • Choose by context when the audience determines the language and controls: V04 for product feedback, V06 for employee pulse checks, and V09 for courses.

These descriptions explain the intended use of each pattern, not which one will produce better response rates. Colorlib publishes no comparative performance measurements.

Do these survey forms need Bootstrap or jQuery?

No. Colorlib describes the templates as plain HTML, CSS, and vanilla JavaScript, without a bundled Bootstrap file, jQuery, or plugin. They can run on their own or be incorporated into projects using Bootstrap 4, 5, or 6. In this context, “Bootstrap-ready” means they can be styled or integrated into those projects; it does not mean Bootstrap is included or required.

Where do the answers go?

They do not go anywhere by default. Colorlib says the templates are front-end only: nothing is sent when a visitor submits one. A thank-you screen is a display state, not evidence that the answers were saved. V07’s bars and counts are also demo behavior: its script simulates the counts rather than aggregating votes from real visitors.

To collect responses, connect the form to a backend or form service, then validate the submitted data on the server. Decide what information to store, who can access it, and how long to retain it before collecting sensitive feedback. Treat client-side checks—such as V04’s file type and size checks—as a convenience for users, not a substitute for server-side validation.

FormBold’s separate repository contains an html-survey-form.html example and describes its examples as vanilla HTML/CSS. It presents FormBold as an optional way to handle submissions dynamically; that is a separate project and endpoint, not a built-in part of Colorlib’s downloads. Its repository describes its own examples as usable and modifiable for personal or commercial work without attribution, but those terms do not change the license for Colorlib templates: FormBold HTML survey form repository.

What license applies to Colorlib’s templates?

Colorlib states that its snippets use the Creative Commons Attribution 3.0 license (CC BY 3.0), allow personal and commercial use, and require keeping the credit link. Check the notice in the downloaded file if you plan to redistribute or substantially adapt a template, because the individual archives were not independently inspected for this article. The separate terms of another repository or form service do not override Colorlib’s stated attribution requirement.
