Organizations can reduce insider risk by controlling what people can access, how they authenticate, and how long elevated permissions remain active. That does not make intent irrelevant or explain every incident: it makes access a practical place to limit opportunity and potential impact while people, reporting, analysis, and response remain essential.
Why access is a practical starting point
Employees, contractors, and other insiders may have legitimate access to facilities, systems, information, and other organizational assets. That access creates exposure regardless of whether it was granted for sound business reasons or whether anyone has harmful intent. A person’s identity or behavior alone cannot tell an organization everything it needs to know about risk; understanding what that person can reach, and why, makes exposure more concrete and governable.
CISA’s Insider Threat Mitigation Guide calls identifying, tracking, and monitoring critical assets “the cornerstone to any effective insider threat program.” In practice, that means beginning with the assets that matter most—such as sensitive information, business-critical systems, or important facilities—and mapping which accounts and roles can access them.
How to put access governance into practice
1. Identify critical assets and map access
Keep an inventory of critical assets and the accounts, roles, and third parties that can reach them. For each permission, record the work need it serves. This gives managers and security teams a basis for spotting access that is broader than necessary, difficult to justify, or no longer appropriate after a person changes roles.

2. Grant only the permissions a job requires
Use role-based permissions, need-to-know, and least privilege to restrict access to the information, networks, hardware, and applications required for a person’s work. CISA’s Cyber Essentials Toolkit gives examples of questions an organization can ask: does marketing need access to financial transaction systems, or does a junior analyst need management-level privileges? The toolkit is dated September 29, 2020, so organizations should check for newer applicable policy or guidance.
Separate ordinary user accounts from administrative accounts where practical, and avoid giving routine accounts powerful permissions. When a person changes roles, reassess access against the new responsibilities rather than carrying forward permissions by default.

3. Make elevated access temporary when possible
Standing administrative access is available even when no administrative task is underway. CISA’s #StopRansomware Guide describes just-in-time access as granting administrative privileges for a specified timeframe when needed for a task. A workable process should define how a request is made, who approves it, when access expires, and how the activity is recorded. Without reliable approvals and expiration, time-limited access can become standing access in practice.
4. Require strong authentication for sensitive access
Use multifactor authentication for sensitive systems, with particular attention to privileged accounts. CISA recommends phishing-resistant MFA and identifies hardware-based PKI or FIDO authentication as options in its MFA guidance and hardening guidance. A FIDO2 security key is one generic example; confirm that a chosen key works with the organization’s systems and that recovery is planned. Strong authentication protects the sign-in step, but it does not narrow excessive permissions or replace monitoring.

5. Review, remove, and monitor permissions
Periodically review accounts and permissions with the relevant managers or data owners. Remove access that is no longer needed, including after role changes or when third-party work ends. Monitor account activity so that unusual or unauthorized use can be investigated. CISA’s hardening guidance supports role and privilege management, account reviews, and monitoring as part of reducing exposure.
6. Apply the same discipline to third parties
Contractors and other external providers may need access to organizational systems or information, but that need does not justify broad or indefinite permissions. Apply least privilege and separation of duties, and define the scope and duration of access. CISA’s red-team findings support limiting third-party access through these controls.

Compare access controls by what they constrain
These controls address different dimensions of exposure. The comparison below is an implementation lens, not a formal CISA scoring framework.

Access controls need a broader insider-risk program
Restricting access can reduce opportunity and limit the potential impact of misuse, but it cannot prevent or explain every insider incident. CISA’s Insider Threat Mitigation Guide says that “Information, technology, people, reporting paths, and skilled analysts or investigators are the fundamental elements for an operational insider threat mitigation program.” A mature approach therefore pairs access governance with clear ways to report concerns, appropriate analysis, and a defined response process.

Intent can matter when an organization assesses a specific concern, but it is not a substitute for managing exposure. A sound program does not need to presume bad intent to ask whether a permission is necessary, review who holds it, and respond to activity that warrants investigation.
