If your repositories already live on GitHub, start by assessing GitHub Copilot code review before you commit engineering time to a custom first-pass reviewer. It can be requested on a pull request or configured to review automatically, it gathers repository context for its agentic features, and it returns comments and suggested fixes. It is an aid to review, not a replacement for human approval. GitHub’s own documentation keeps architectural judgment, final approval, and accountability with the team. No source establishes a market-wide winner among build, buy, and platform options, so the right choice depends on your repository host, the context the reviewer can see, where its runners live, what it costs in total, and how your governance works.

What “first-pass” review means in practice

A first-pass reviewer reads a pull request before a human reviewer does. It can flag likely bugs, style violations, missing tests, and risky changes so that people spend their time on design and intent. It does not decide whether code merges. That decision still belongs to the team that owns the code, and the approval step in your branch protection rules should reflect that.

Three distinct approaches are worth comparing. They solve overlapping problems, and it is easy to conflate them.

Build: you own every moving part

A custom pipeline gives you control over which model runs, what code and history it can read, which secrets it can touch, and how results are logged. The cost is that you also own the failure modes. Someone must keep prompts current as models change, measure whether the reviewer’s comments are useful, handle rate limits and outages, and make sure the reviewer cannot leak repository content to places it should not go. Building makes the most sense when your constraints (regulated code, unusual hosts, or strict data residency) rule out the platform options, or when you have a platform team with spare capacity and a clear evaluation plan. Price the build honestly: include engineering time for maintenance, not only the initial setup.

Buy: a managed service trades control for operations

A managed review service can reduce the work of running the pipeline. Before you sign anything, check the things a vendor’s sales page rarely emphasizes: which repository hosts and permission models it supports, what code and related context it can inspect, how it is triggered, whether it needs its own runners or CI access, and what happens to your code and data. The evidence gathered for this article did not verify any competing vendor’s current feature set or terms, so treat any comparison you read elsewhere as something to check directly.

Use your coding agent’s cloud: the GitHub example

GitHub is the one concrete, documented example in this comparison. Copilot code review evaluates an existing pull request. Copilot cloud agent is a different function: it researches and implements a task in an ephemeral cloud development environment, can explore code, edit files, and run tests and linters, and works toward a pull request. The two are related, and they can be used together, but they are separate stages. Reviewing a change is not the same as producing one, and a team should evaluate each on its own terms.

Can the reviewer look at every pull request before a human does?

It can be configured to, and that is the main reason teams adopt it. GitHub says a Copilot review can be requested when a pull request is opened, or configured for automatic review on every pull request. Whether it is useful on every pull request is a different question. Large or unusual changes, generated code, and pull requests that depend on code outside the diff are where you should look at its output most carefully. Automatic review on all traffic can also raise the cost, which is covered below.

What the reviewer can see

GitHub’s documentation says its agentic review capabilities gather full-project context, and the product page describes review across the changeset and the wider repository. That matters because a reviewer that reads only the diff misses how a change interacts with the rest of the system. Before you rely on it, ask what it can inspect in your intended setup, and verify that with a real pull request that touches shared code. Repository instructions and custom agent skills, along with any connected tools, shape what it checks and how it reports. Write these instructions down, version them with the repository, and review them the way you would review a linter configuration.

Runners, Actions, and what happens when they fail

Agentic review features depend on GitHub Actions. The rules are specific, so plan for them:

  • Standard GitHub-hosted runners are the default for these workflows.
  • Larger GitHub-hosted runners are billed at a higher per-minute rate than standard runners.
  • Self-hosted runners do not consume GitHub Actions minutes, though you then operate the machines yourself.
  • Disabling GitHub-hosted runners makes the agentic capabilities unavailable unless your organization uses self-hosted runners.
  • If Actions is unavailable or the relevant workflow fails, GitHub says Copilot still generates a review, but without the additional agentic capabilities.

The practical lesson is that a failed workflow degrades the review rather than stopping it. Your team should know which version of the review it is looking at. If a reviewer’s comments seem shallow on a given day, check the Actions run before you conclude the model is at fault.

The real cost of automated review

Cost has two components that come from different budgets. GitHub describes AI credits for model interaction and Actions minutes for agentic capabilities. Treat them as separate lines in your forecast.

GitHub’s current documentation, accessed in 2026, estimates that a review typically consumes about $0.05 to $1 USD worth of AI credits at Lite effort, and about $0.25 to $5 USD worth of AI credits at Balanced effort. These figures are estimates, they exclude Actions minutes, and they may change as models change. Consumption generally rises with pull request size and with the length of your repository’s custom instructions. Multiply the per-review range by your monthly pull request volume, then add runner minutes for the agentic work. The total will be uneven, so set a budget and watch actual usage for a month before committing.

Who pays also depends on how the review was started. GitHub attributes usage from automatic review to the pull request author, and usage from a manually requested review to the user who requested it. Rules differ for pull requests created by the cloud agent, for other bots, and for users without a qualifying license. Copilot Free does not include Copilot code review. Business and Enterprise organizations can enable review for members who lack a Copilot license under specific policies, and the resulting AI-credit use is paid additional usage charged to the organization or enterprise. Confirm the current plan and policy details with GitHub before you roll this out, since they are the most likely items to change.

Limits of the cloud agent

If you plan to hand work from review to the cloud agent, know its boundaries. GitHub documents the following for Copilot cloud agent:

  • It works only with GitHub-hosted repositories.
  • It works on one repository, one branch, and one pull request per task.
  • A session lasts at most 59 minutes.
  • Incompatible repository rules can block it from running.

GitHub also describes the handoff from review suggestions to the cloud agent as a public preview feature that is subject to change. Do not build a release process that depends on it without a fallback path.

Accountability does not move to the tool

GitHub’s feature page makes a point that deserves to be taken literally. It says the reviewer “Brings architectural judgment, design perspective, and system context that only comes from building the software together.” The page attributes this to “Your team” and does not name an individual or role. It also says the team “owns final approval and accountability.” Automated comments and suggested fixes are inputs to a human decision. If a bad change merges after a clean automated review, the process failed at the approval step, and no tool change alters that.

A decision framework

  1. Check the host first. If your repositories are not on GitHub, the GitHub cloud agent is off the table, and you are choosing between a build and a vendor that supports your host.
  2. Test context on real pull requests. Run the reviewer on changes that touch shared code and see whether it reasons beyond the diff.
  3. Price the full stack. Add AI credits, runner minutes, and the engineering time to maintain a build or to administer a vendor.
  4. Decide on runners. Choose between hosted and self-hosted runners, knowing the cost and availability trade-offs in each case.
  5. Write the governance rules. Define who may enable automatic review, which instructions the reviewer follows, and which approvals remain human-only.
  6. Pilot before scaling. Run a bounded set of repositories for a month, record the cost and the review quality you observe, and decide from that record.

Bottom line for most teams on GitHub

If your code is on GitHub and your governance allows it, Copilot code review is the sensible first option to evaluate, because it is documented, integrated, and cheap to trial relative to a build. Reserve a custom build for cases where hosting, data, or control requirements clearly rule out a platform option. Whichever path you choose, keep the human approval step, budget AI credits and runner minutes separately, and do not treat the reviewer’s output as a measured guarantee of quality.
