A travel site absorbed 178 newly published trip reports in one afternoon by queuing moderation and place-extraction work instead of making model calls inside the publishing request. The queue handled the burst at its configured pace—but an older batch of rate-limited image calls had left 98 photos marked “needs review” without any model judgment. The incident, described by site author Corneliu Croitoru, shows why an LLM pipeline must distinguish a moderation verdict from a failed attempt to obtain one.

How the site handled the publishing burst

In a first-person account published on September 20, 2026, Corneliu Croitoru described a traveller importing a two-year Polarsteps diary and publishing 178 trip reports in one afternoon. The site had nine reports beforehand. A publish-status trigger queued work for text moderation, place extraction, and image moderation; model calls ran asynchronously, outside the publishing request.

The figures below are Croitoru’s account of one site, not an independent benchmark. He did not name the model provider, service tier, exact rate limit, or billing records.

A shared queue set the pace

The worker woke once per minute and claimed the ten oldest jobs. Each claim received a five-minute lease. Croitoru said the lease served both as retry backoff and as crash recovery if a worker stopped mid-job. After three unsuccessful attempts, a job was marked failed and its related item sent to human review. He said the ten-jobs-per-minute pace was chosen to fit the shared per-minute model budget on his tier.

Six minutes after publishing, Croitoru reported these counts:

Job type Done Pending
Place extraction 28 151
Text moderation 27 152
Image moderation 0 18
Total 55 321

The 321 pending jobs amount to roughly 32 minutes of work at ten jobs per minute, if that pace continued and no new work entered the queue. Croitoru reported 179 rows in each report-level category despite 178 reports; he did not explain the extra row. The 18 image jobs are listed separately in his count.

That backlog did not mean the publishing request itself had to wait for model responses. It did mean a later report arriving during the backlog could wait behind older work because the queue was shared and oldest-first.

Why 98 photos were awaiting review without a model verdict

The next morning, Croitoru found about a hundred photos marked “needs review.” Ninety-eight carried the reason “Image moderation service unavailable.” He said those rows reflected three unsuccessful attempts and a final pipeline failure—not a model assessment of the images. Two other photos had model-generated reasons.

The cause, in his account, predated the new queue. Before it existed, photo inserts called image moderation directly. The bulk import was rate-limited, and those failed photos were saved as “needs review.” Weeks later, the publish trigger selected only photos still marked pending, so it skipped the failed rows. Completed job records were purged after seven days, which meant Croitoru could not use those records to date the failures.

The underlying design problem was that the same status field represented two different things: a moderation decision and a failure to get a decision. Once the transport failure had been stored as “needs review,” the pending-only trigger treated the photo as already handled.

Re-queueing separated the failure from the verdict

Croitoru’s repair was to re-queue failures identified by their stored reason while leaving genuine model decisions alone. He said he manually re-queued 98 photos. In his subsequent account, 465 photos had been processed: 432 approved, nine rejected, and 24 still needing review with model reasons. Those are reported outcomes, not independently verified counts.

How the apparent worker timeout was diagnosed

Croitoru also described four jobs with one attempt that remained pending. He interpreted them as in flight under a live lease, rather than as jobs that had never started. Separately, the HTTP log showed a five-second timeout each minute, even though the worker could run for up to 60 seconds: the cron caller had stopped waiting. He checked the cron’s own run log and said it showed a successful run every minute.

In this incident, the useful diagnostic was to compare the request-facing timeout with the worker’s lease and the cron’s own run record. A caller timing out did not, by itself, establish that the worker had failed. That is Croitoru’s diagnostic sequence for this system, not a universal rule about cron or HTTP timeouts.

What the shared queue traded away

A single oldest-first queue kept all job types within the same stated rate budget. Its trade-off was that new work—whether a new report or a different kind of job—could wait behind the existing backlog.

Croitoru named two possible changes, neither of which he had built:

  • Add a second worker. The account does not establish how this would affect queue wait times or whether it would stay within the same model rate budget.
  • Reserve part of each tick for each job kind. The account does not measure the effect on wait times, fairness, or total throughput.

These are design options, not tested recommendations. Choosing between them would require checking how quickly new job types need service, how shared rate limits are enforced, how work is allocated fairly, and how much extra operational complexity the system can support. Croitoru did not report a comparison or benchmark.

What the incident says about LLM pipeline monitoring

Croitoru said he added an admin Pipeline panel showing counts for waiting, in-progress, recently completed, and failed jobs, recent failure text, and a status verdict when work was not progressing. The panel addressed more than throughput: it made it easier to distinguish work still under a live lease from work that had reached a terminal failure.

For a pipeline that triggers consequential actions such as human review, the incident points to a practical monitoring need: operators should be able to see job state and failure reason separately from the model’s decision about the underlying content. Otherwise a failed request can look like a completed moderation judgment, and retries that select only pending items can leave it behind.

Croitoru reported that the day’s model calls and retries cost twenty-nine cents. Because he did not identify the provider, tier, or billing evidence, that figure describes his account only; it cannot establish a general cost per report or a reproducible price for another pipeline.
