If an AI tutorial project works in a demo but becomes brittle or disappointing in real use, don’t add another technique at random. First identify which layer is failing: the information available to the model, retrieval, model behavior, evaluation, or the permissions around its tools. These five dead ends explain why common tutorial shortcuts fall short—and how to diagnose the actual problem.
1. Treating prompts, RAG, and fine-tuning as a recipe ladder
A common tutorial path is to improve the prompt, add retrieval-augmented generation (RAG), then fine-tune. That sequence is not a universal progression: each method addresses a different kind of failure. OpenAI’s Optimizing LLM Accuracy describes optimization as iterative: establish a baseline, inspect incorrect examples, choose an intervention based on a hypothesis, and evaluate again.
- The model lacks the right information: Improve the context it receives. Retrieval or another context source may help when knowledge is missing, stale, or proprietary.
- The model has the information but behaves inconsistently: Work on instructions or model behavior, including format, tone, or task-specific responses.
- A recurring task needs learned behavior or greater efficiency: Fine-tuning may be relevant, but it is not a general-purpose way to make a model “smarter.” Use examples that represent real inputs and keep a separate hold-out set to check for overfitting.
Before choosing a fix, collect representative failures and state a testable hypothesis: for example, “answers are wrong because the relevant policy is missing from the context,” not simply “the model needs more tuning.”

2. Assuming RAG is a hallucination switch
RAG can provide external or domain-specific information, but adding a vector database does not by itself make answers accurate. There are at least two distinct questions to debug: did retrieval find concise, relevant evidence, and did the model use that evidence correctly?

An integrated 350W power supply ensures stable and reliable output for demanding computing tasks and fully loaded extended configurations.
- Inspect retrieval: For failed examples, check whether the returned passages contain the information needed to answer. Look for missing evidence, irrelevant results, or so much context that useful material is buried.
- Inspect generation: If the retrieved passages are suitable, check whether the model followed instructions and grounded its answer in them.
- Change one layer at a time: Adjust retrieval settings or the answer instructions according to the diagnosed failure, then rerun the same evaluation cases.
Keeping retrieval and generation failures separate makes it easier to tell whether a change helped or merely moved the problem.
3. Stopping when the tutorial demo works
A successful demonstration shows that a path through the application can work; it does not establish that the application will behave reliably on the range of inputs people will actually send. Evaluation needs examples representative of the intended task, a baseline, and review of failed cases. The cost of an error matters: a wrong draft and a wrong consequential decision are not equivalent.

Build an evaluation that reflects the use case
- Include ordinary inputs, edge cases, and the formats or ambiguity users are likely to produce.
- Define task-specific measures and the minimum acceptable performance before testing. Google’s Safety evaluation guidance cautions that generic datasets may not cover an application’s particular risks.
- Test adversarial and unusual cases, not only typical examples. Outputs can vary between runs, so repeat tests when variability could affect the result.
- When training or fine-tuning, reserve hold-out examples that are not used to fit the model.
Review failures, not just a single score
OpenAI recommends red-teaming with representative and deliberately adversarial inputs, and human review where practical—especially for high-stakes uses and code generation. Its Safety best practices also support making source material available to reviewers so they can verify generated summaries and other outputs.

1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsThere is no universal accuracy or safety percentage that establishes readiness. “Good enough” depends on the task, who may be affected, and the consequences of errors.

4. Letting untrusted text act like an instruction
Webpages, uploaded files, retrieved documents, and connector results are data—but they may contain text designed to redirect a model. If a tool-using application follows those instructions, it might take an unintended action or expose private information through a downstream call. This is why prompt injection is not merely a wording problem: it can become an action or data-access risk.
OpenAI’s Agent Builder safety guidance and Safety best practices describe mitigations including trusted or audited connectors, constrained tool arguments, and review of tool activity. Practical safeguards include:

These controls reduce exposure; they do not guarantee that every attack will be detected.
5. Treating a prompt as an access-control system
A model’s instruction is not an enforceable permission boundary. As the US Department of Energy’s GEAR AI Security and Safety guidance puts it: “A prompt such as ‘never delete files’ is not an access control.” Enforce authorization in the application and the systems it uses—such as the operating system, database, scheduler, gateway, or policy layer—not only in natural-language instructions.

Grant only the authority the task needs
Start at the lowest capability level that can complete the task. Give the model only the data, tools, and autonomy it needs, and restrict what each tool can read or change. A system that only drafts a proposed database update should not also have unrestricted authority to apply it.
Put limits and recovery outside the model
Set enforceable limits for time, cost, tokens, retries, jobs, and actions. Provide a way to cancel work and, where possible, roll back changes. Require a human approval point before consequential operations such as writing shared data, deleting information, installing software, sending external communications, or using outputs in consequential decisions.
A practical way to get unstuck
- Capture the failure: Save representative inputs, outputs, retrieved context, and tool calls where applicable.
- Classify the failing layer: Is needed information absent or stale, retrieval poor, model behavior inconsistent, input unsafe, or tool authority excessive?
- Choose one intervention: Write down what you expect it to change and why that addresses the observed failure.
- Re-run the evaluation: Compare results on the same representative cases and inspect new failures, not just the headline score.
- Check the risk boundary: Confirm that permissions, limits, approvals, and recovery remain effective even if the model produces an unexpected response.
When comparing possible fixes, weigh the diagnosed failure first, then consider likely effect, implementation and operating effort, data access, reversibility, and the consequences of getting it wrong. The right next step is the one that addresses the demonstrated failure—not simply the next tool in a tutorial sequence.
