Vision-language models (VLMs) misread images for several different reasons, not one generic “hallucination” problem. They can mistake an object or attribute, invent a relationship between visible items, misread text, or produce reasoning that is not sufficiently grounded in what the image shows. Fine visual details may be missed, language expectations can outweigh image evidence, and degraded images can make a precise answer impossible.
The most reliable response is to identify the failure mode first, then match the mitigation to it: provide clearer or cropped visual evidence, ask for observable details, use a specialist recognition system, change decoding, or require an uncertainty statement or abstention. No single technique fixes every kind of visual error, and benchmark results from one model or dataset do not predict performance in every application.
What “misreading an image” actually includes
In a VLM, an incorrect answer can originate at different stages. The model may fail to perceive a visual detail, connect correctly perceived details in the wrong way, misread pixels as text, or answer a reasoning question with language that is only loosely tied to the image. Treating all of these as the same problem makes diagnosis and improvement harder.
Object and attribute mistakes
An object error is a wrong identification or an invented item. A model might call a bicycle a motorcycle, report a baseball when only a bat is visible, or describe a person, sign, or tool that is not present. Attribute errors are more specific: the object is real, but its color, material, size, state, or count is wrong. Fine-grained visual similarity is a major source of these mistakes. A bicycle and motorcycle can share a silhouette, while a bat and baseball can occur in the same familiar scene even when only one is visible.

Relationship mistakes
Relationship errors concern how objects are connected. The model may say that one person is holding an item when the hand is merely near it, place an object in front of another when it is behind it, or assign an action to the wrong person. Some questions ask whether a relationship exists at all; others ask which particular instance participates in it. Spatial context, occlusion, and crowded scenes make these distinctions difficult.
OCR failures
Optical character recognition errors occur when the model reads text that is blurred, partly hidden, low contrast, angled, tiny, stylized, or visually cluttered. The output may contain substitutions, missing characters, incorrect numbers, or a completely invented word. A document question can therefore fail even when the model recognizes that the image is an invoice, identity card, or prescription.
Reasoning that is not grounded in the image
A reasoning answer can be wrong even when the individual visual elements are recognized. The model may infer a cause, comparison, quantity, or outcome from common language patterns rather than from the pictured evidence. For example, it may complete a familiar story about a scene while overlooking an unusual object or an exception that changes the answer. Reasoning-intensive prompts expose a gap between recognizing visual elements and using them correctly in context.

Why VLMs make these errors
Fine-grained visual confusion
The 2025 ACL paper on Visual Evidence Prompting links hallucinations to weak fine-grained visual comprehension, particularly when objects or scenes are visually or semantically similar. Generalist models may have enough information to produce a fluent answer but not enough reliable detail to distinguish near-neighbors. The paper’s approach supplies outputs from specialist visual models as prompts, giving the generalist model additional evidence before it generates a response.
This explains why broad labels can be correct while a specific answer is wrong. “Vehicle” may be defensible when “motorcycle” is not; “sports equipment” may be safer than choosing between a bat and another object from a small, partially occluded region. The error is perceptual before it is linguistic.
Language priors and missed context
The 2024 R-Bench paper reports that evaluated LVLMs can overlook visible content, rely too heavily on large-language-model common sense, and struggle with spatial relationships that depend on context. Visual instruction-tuning data also contain recurring co-occurrence patterns and long-tail relationships. Those patterns can make a likely relationship seem more plausible than the relationship actually shown.
In practice, a model may answer what normally happens in a familiar scene instead of checking which object is where. This is why a prompt that asks for a justification based on visible evidence can reveal a different problem from a prompt that asks only for a label. The model’s language fluency is not proof that its visual grounding is strong.

Uncertainty caused by image degradation
The 2025 NeurIPS KIE-HVQA work studies OCR hallucination in degraded documents with blur, occlusion, and low contrast. Its central concern is uncertainty: when the pixels do not support a precise reading, a system that is unable to recognize that limitation may confidently invent text. The same principle applies outside documents. A tiny object, a dark region, glare, compression artifacts, or an obstruction can remove the evidence needed for a reliable answer.
A dependable system should sometimes say that a detail cannot be read or distinguished. Refusal is not a failure when the image is genuinely ambiguous; an unqualified guess can be more harmful than an explicit request for a clearer crop.
A gap between recognition and reasoning
The ICLR 2025 Visual Description Grounded Decoding (VDGD) paper reports that methods aimed mainly at visual recognition can fall short on reasoning-intensive prompts. The authors describe a gap in contextualizing recognized visual information with the question. Their result comes from the paper’s experimental setup and does not establish that every current system uses the same mechanism, but it illustrates why better object recognition alone may not solve a multi-step question.
How to diagnose the failure mode
Start with the smallest claim that could be wrong. Ask whether the model failed to see an item, connect items, read characters, or apply the question’s logic. The following symptoms provide a practical triage guide.
| Observed symptom | Likely failure mode | Useful check |
|---|---|---|
| An absent item is described, or a similar item is substituted | Object or attribute error | Ask for a list of visible objects and distinguishing features before requesting the final label |
| The objects are present but their positions, actions, or ownership are wrong | Relationship error | Ask who or what is connected to which specific instance, including location and direction |
| Words or numbers change between attempts, especially in a small or damaged region | OCR error | Provide a higher-resolution crop and ask the system to mark unreadable characters rather than guess |
| The answer is fluent but ignores an exception, count, or visual condition | Grounding or reasoning error | Request the relevant observations first, then require each inference to cite one of them |
Run these checks independently. If an answer changes when you ask for a description, the original response may have been driven by a language prior. If the description itself changes, the bottleneck is more likely visual perception. If both descriptions agree but the conclusion is wrong, investigate reasoning or prompt interpretation.
Reduce errors before generation
1. Ask for observable evidence before interpretation
When exact grounding matters, split the task into two turns or two explicit stages. First request only what can be seen: objects, text regions, colors, positions, counts, and visible actions. Then ask the model to answer the higher-level question using that inventory. This reduces the chance that a familiar narrative will replace an inspection of the pixels.
First list only directly observable details. For each object, state its location, visible attributes, and whether any part is occluded. Do not infer intent or identity beyond what the image supports. Then answer the question using only that list.
For a relationship question, name the instances rather than using vague pronouns: “Is the person on the left holding the red object, or is the hand merely beside it?” For an attribute question, ask what evidence distinguishes the candidate classes.

2. Keep the question narrow
Bundling detection, OCR, counting, spatial reasoning, and explanation in one request increases the number of opportunities for an error. Ask one visual operation at a time when the result matters. A sequence such as “locate the label,” “transcribe the readable characters,” and “calculate the total” is easier to audit than a single request to interpret an entire document.

Supply a crop of the relevant region while retaining the original image for context. A close view can make a small label or object legible; the full image can preserve relationships and scene layout. Compare the crop-based result with the context-based result rather than treating either one as ground truth. Cropping may remove information needed to identify who is holding an item or where a sign is located.
4. Improve the image instead of forcing precision
Use the clearest available source, adequate resolution, even lighting, and a straight camera angle. For documents, avoid glare and compression, and capture the entire text line when possible. Upscaling can make a crop easier to inspect but cannot recreate characters that were never recorded. If blur, occlusion, or low contrast remains, ask the system to identify uncertain positions and leave them unresolved.
5. Require calibrated uncertainty
Tell the model what to do when evidence is insufficient. A useful instruction is: “If a character, object, or relationship cannot be determined from the image, say ‘uncertain,’ identify the obstructing condition, and do not fill it in from context.” This does not guarantee calibrated probabilities, but it makes abstention an allowed outcome and gives reviewers a visible reason for uncertainty.
6. Add a task-specific evidence source when stakes justify it
A specialist detector, OCR engine, or visual classifier can provide another view of a difficult region. Treat its output as evidence to verify, not as an unquestionable label. Disagreement between the specialist and the generalist is a signal to inspect the crop or involve a person, not a reason to select whichever answer sounds more confident.

Research approaches and what each one targets
Published methods operate at different points in the generation process. They should be compared by the error they target, the model and data they require, and the extra computation they add.
| Approach | Mechanism reported by the cited work | Questions to ask before adopting it |
|---|---|---|
| Visual Evidence Prompting | Uses outputs from small or specialist visual models as prompts so a generalist LVLM receives fine-grained evidence. The 2025 ACL paper focuses on visual and semantic confusions. | Which specialist model is needed? Does it cover the confusing classes and the target image domain? Does the added evidence improve the task rather than introduce another error? |
| Visual Description Grounded Decoding (VDGD) | Generates an image description and uses it during decoding to ground the response. The ICLR 2025 paper presents it as training-free and evaluates it on eight benchmarks. | What additional inference passes are required? Does the description help the target reasoning task? Does it preserve recognition and OCR performance? |
| Visual Contrastive Decoding (VCD) | The CVPR 2024 paper reports object-hallucination mitigation without additional training or external tools by changing the decoding process. | Which object-hallucination cases improve? How does the decoding change affect latency and other task metrics? |
| Assembly of Global and Local Attention (AGLA) | The CVPR 2025 paper uses image-prompt matching to emphasize prompt-relevant local features and suppress irrelevant distractions. | What preprocessing or attention modifications are needed? Does relevance highlighting help small objects and cluttered scenes? |
| Uncertainty-aware OCR response behavior | The NeurIPS 2025 KIE-HVQA work trains and evaluates uncertainty awareness and refusal behavior for degraded documents. | Can the system recognize ambiguity? Is abstention calibrated and useful for the application? What happens on clean documents and on non-OCR tasks? |
Visual Evidence Prompting: add specialist visual evidence
This method is most directly aligned with fine-grained recognition problems. A specialist model can focus on distinctions that a broad LVLM misses, and its output can be inserted into the prompt as evidence. The trade-off is architectural: you need a suitable specialist, a way to format and validate its output, and a policy for conflicts between specialist and generalist predictions. It is not a universal relationship or reasoning solution.

VDGD: ground decoding in an explicit description
VDGD creates an intermediate description and uses it while generating the answer. The design addresses the gap between recognizing visual elements and using them to answer a reasoning prompt. Because it is described as training-free, it can be applied without updating model weights, but it still adds inference work. Its reported gains are tied to the authors’ setup and benchmarks, not a guaranteed gain for an arbitrary deployment.
VCD: alter decoding to reduce object hallucination
Visual Contrastive Decoding changes how candidate tokens are selected so that image-supported content is favored over patterns associated with hallucinated objects. The CVPR 2024 report describes this as requiring no additional training or external tools. The relevant question is whether the deployment’s error profile resembles the object-hallucination cases studied; a decoding improvement for object presence does not automatically fix OCR or multi-step reasoning.
AGLA: emphasize prompt-relevant local regions
AGLA combines global and local attention using image-prompt matching. The intended benefit is to highlight regions relevant to the question while reducing distracting features in cluttered scenes. This is useful to consider when a small object is present but surrounded by visually dominant content. Implementation details, preprocessing needs, and effects on latency must be measured for the target model.
Uncertainty-aware OCR: allow a defensible refusal
The KIE-HVQA work treats refusal and uncertainty as explicit behaviors for degraded documents. This is a different objective from squeezing out another guessed character. A production workflow can route uncertain fields to a clearer capture or human review, preserve the original crop, and record which characters were unreadable. Such a policy is especially important for identity documents, invoices, and prescriptions, where an invented digit can change the meaning.
Match the mitigation to the task
| Primary task | First interventions | Evidence that should be checked |
|---|---|---|
| Distinguish similar objects | Crop the object, request distinguishing visual features, and consider specialist visual evidence | Class confusion rate, performance on near-neighbor classes, and whether the crop preserves context |
| Determine who did what or where an item is | Ask for instance-level positions and relationships; consider local-attention methods | Correct subject-object pairings, direction, depth, and performance in crowded scenes |
| Read a damaged document | Use a high-quality crop, separate transcription from interpretation, and permit uncertain characters or refusal | Character-level accuracy, field-level accuracy, abstention quality, and behavior under blur or occlusion |
| Answer a multi-step visual question | Require an observation stage, then an inference stage; consider description-grounded decoding | Whether each conclusion follows from visible evidence, not just the final answer string |
| Reduce invented objects in open-ended captions | Consider contrastive decoding and test on images with unusual or sparse content | Object hallucination rate, retained description quality, and latency |
Methods are not interchangeable. A specialist detector may improve a class distinction but add its own blind spots. An explicit description can make reasoning more traceable but may propagate an incorrect intermediate observation. A decoding change may reduce one category of hallucination while affecting speed or another task metric. Compare alternatives on the exact image domain and prompt style you will deploy.

HallusionBench
HallusionBench is presented as a diagnostic suite for image-context reasoning and visual illusions. Its authors report that the advanced LVLMs they tested found the tasks challenging. The suite is useful for probing whether a model remains tied to image evidence, but a result on it is not a universal measure of OCR, object detection, or every form of visual understanding.
R-Bench
R-Bench evaluates relationship hallucination with image-level questions about whether a relationship exists and instance-level questions about local visual comprehension. It identifies relationship-relationship, subject-relationship, and relationship-object co-occurrences as relevant patterns. This makes it more informative for spatial and interaction errors than a benchmark that only asks for object names.

KIE-HVQA
KIE-HVQA covers degraded document examples including identity cards, invoices, and prescriptions. It uses simulated degradations and pixel-level annotations for OCR reliability. The paper authors report approximately a 28% absolute improvement in hallucination-free accuracy over GPT-4o on KIE-HVQA for their 7B Qwen2.5-VL-based model. That figure belongs to the named model, method, benchmark, and 2025 publication; it is not a general expected improvement or a current performance rate for every OCR system.
VDGD’s eight-benchmark result
The VDGD paper authors report performance improvements ranging from 2% to 33% across eight benchmarks in their 2025 evaluation. The range describes that evaluation and should not be converted into an expected gain for an unrelated model, dataset, or application. Report the benchmark, model setup, prompt type, image quality, and metric whenever you publish a number.
Build an application-specific test set
Include examples that isolate the errors your users care about: similar objects, small items, overlapping people, directional relationships, low-contrast text, partial occlusion, unusual layouts, and questions requiring more than one inference. Label not only the final answer but also whether the relevant evidence is visible and whether abstention is appropriate. Keep clean and degraded versions separate so that a system is not rewarded for guessing through unreadable inputs.
Measure the targeted failure directly. For OCR, character or field accuracy and the rate of fabricated text are more useful than a caption score. For relationships, score the correct instances and direction. For reasoning, inspect whether intermediate observations support the conclusion. A benchmark score for one category cannot stand in for all image understanding.
A practical review workflow
- Define the claim. Decide whether the system must identify an object, read text, establish a relationship, count items, or infer an outcome.
- Check visibility. Confirm that the relevant pixels are present, large enough, sufficiently contrasted, and not blocked. If not, capture a better image or plan for abstention.
- Ask for observations. Request a concise inventory of visible objects, text regions, positions, and uncertainty without asking for the final interpretation yet.
- Inspect the critical crop. Give the model a focused crop for tiny text or objects while retaining the full frame for context.
- Separate transcription from reasoning. For documents, transcribe only readable characters before calculating, classifying, or summarizing.
- Cross-check high-risk details. Use a specialist visual or OCR system, a second image, or human verification for information that changes a decision.
- Require an evidence-linked answer. Ask the model to state which visible observation supports each conclusion and to mark unsupported parts as uncertain.
- Log the failure mode. Record whether the problem was object, attribute, relationship, OCR, grounding, or reasoning. This makes later model comparisons meaningful.
Common mistakes when trying to reduce hallucinations
Assuming confidence means visual certainty
Fluent wording and a definite tone can come from language generation rather than strong visual evidence. Require the model to identify what it can and cannot see, especially when the image is degraded.
Using a larger prompt instead of better evidence
Long instructions cannot restore pixels lost to blur or occlusion. Extra context can even strengthen a misleading expectation. Improve the image, isolate the region, or permit an abstention.
Treating a specialist output as ground truth
A detector or OCR engine is another fallible component. Verify its output against the crop and preserve disagreements for review.

Published improvements are conditional on a model, dataset, prompt format, image quality, and metric. They should guide hypotheses, not serve as a universal error rate or product guarantee.
Optimizing only the final answer
A correct answer obtained for the wrong visual reason may fail on a small change in the image. Evaluate the observations and relationships that support the answer, and test near-identical images where one detail changes.
What a reliable system should communicate
A useful VLM does more than produce a label. It distinguishes visible evidence from inference, identifies the region supporting a claim, marks unreadable or ambiguous details, and declines precision when the image cannot support it. For high-impact workflows, retain the original image, the relevant crop, the model version and prompt, any specialist outputs, and the reason for human review. This record lets you determine whether a later error came from capture quality, perception, OCR, relationship assignment, or reasoning rather than treating every mistake as an undifferentiated hallucination.
