Ketamine can make movement or speech feel impossible because it changes how the brain integrates sensation, awareness and purposeful action. Its dissociative state may make you feel separated from your body or surroundings, so commands you intend to carry out do not translate into coordinated movement or words. That experience is not automatically paralysis: U.S. Food and Drug Administration labeling says skeletal-muscle tone is usually normal or slightly increased, and spontaneous limb movements can occur.
The risk and meaning depend on the formulation, dose, setting and supervision. Clinician-administered ketamine and supervised esketamine treatment include observation, while an excessive or nonmedical exposure can produce profound detachment and loss of movement. Trouble breathing, collapse, a seizure or inability to wake someone is an emergency.
What ketamine is doing when movement feels impossible
Ketamine is a dissociative anesthetic. The FDA describes its primary action as antagonism of N-methyl-D-aspartate (NMDA) receptors in the central nervous system. Rather than simply switching muscles off, this changes how the brain receives and interprets incoming signals, including touch, sound, pain and information about where the body is in space. The result can be a powerful separation between awareness and normal interaction with the body.

The KETALAR prescribing information calls ketamine “a rapidly-acting general anesthetic producing a dissociative anesthetic state characterized by profound analgesia, normal pharyngeal-laryngeal reflexes, normal or slightly enhanced skeletal muscle tone, cardiovascular and respiratory stimulation, and occasionally a transient and minimal respiratory depression.” This is a drug-label description, not a prediction of what every patient will experience. Dissociation breaks the expected connection
In ordinary movement, the brain combines intention (“raise my hand”), sensory feedback and motor coordination. Dissociation can disrupt that integration. You may know what you want to do but feel that the instruction is distant, delayed or disconnected from your limbs. Visual or auditory information may also seem unreal or difficult to interpret, making a simple action harder to organize.
An NHS procedural-sedation guideline describes dissociation as a disconnection between cortical and limbic processing that prevents higher centers from fully perceiving visual, auditory or painful stimuli. It also lists random purposeless movements and muscle twitching as possible effects. That guidance concerns pediatric procedural sedation, so it helps explain the phenomenon without proving the precise cause of an individual adult’s symptoms. Why this is not necessarily paralysis
Feeling unable to move and having no muscle activity are different things. The FDA label says muscle tone may remain normal or become slightly enhanced, and it records nystagmus, salivation, tearing and spontaneous limb movements associated with ketamine’s effects. A person may therefore be unable to perform a purposeful action while still moving, twitching or resisting passively.
That distinction matters clinically. A dissociated person might not respond coherently, follow instructions or coordinate a reach, yet still have intact reflexes and muscle tone. The sensation can be frightening, especially if the person is conscious enough to notice it. It does not, however, establish that the brain, spinal cord or muscles have been permanently damaged.

Conversely, apparent immobility should never be dismissed as “just dissociation.” A serious overdose, a second substance, low oxygen, a seizure or another medical problem can also impair movement or consciousness. A new, severe, persistent or worsening inability to move needs professional assessment rather than a remote diagnosis.
Why speaking can become difficult
Speech requires attention, language planning, breathing, precise movements of the tongue and lips, hearing your own voice and feedback from the people around you. Dissociation can interfere with several of those steps at once. Words may be hard to retrieve, your voice may feel far away, or you may understand a question without being able to form a response. Poor coordination and altered hearing or time perception can make speech sound slowed, slurred or unrelated to what you intended.
Difficulty speaking is specifically listed among possible side effects of esketamine nasal spray in MedlinePlus drug information. That information applies to esketamine treatment; it is not a prevalence estimate for every ketamine product or nonmedical exposure. The symptom can occur alongside confusion, hallucinations, agitation or involuntary movements.

Ketamine, esketamine and nonmedical exposure are different contexts
The word “ketamine” can refer to several situations. Route, formulation, amount, co-administered substances and monitoring change both the experience and the risk. The following comparison keeps the available evidence within its proper boundaries.

Clinician-administered ketamine
For injection, MedlinePlus says a specially trained provider administers ketamine intravenously or intramuscularly and monitors the patient during treatment and for a period afterward. The provider can observe breathing, circulation, consciousness and unusual movements instead of relying on the patient to report symptoms. Details are in MedlinePlus: Ketamine Injection.
Supervised esketamine
A 2025 patient leaflet from Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust says acute effects in its supervised treatment setting are usually short-lived and generally subside within 90 minutes to two hours. Patients remain in the clinic until the effects pass. The same leaflet reports approximate frequencies of around one in three for dizziness and around one in four for nausea, headache or dissociation in that treatment context. Those figures do not measure inability to move or speak and must not be extrapolated to all ketamine use. Read the NHS esketamine patient information leaflet.

NHS Greater Glasgow and Clyde and FRANK describe a “k-hole” as a state of detachment in which a person may be unable to move and may struggle to make sense of what is happening. This is public-health information, not a clinical definition or a universal dose rule. Mixing ketamine with alcohol, sedatives, opioids or other drugs can make impairment and breathing risk less predictable.

How long can the problem last?
There is no single duration for every route, dose or person. In supervised esketamine treatment, the cited NHS leaflet says acute effects usually settle within 90 minutes to two hours under clinic observation. Injection effects depend on the medicine, amount, route and other medicines used. A nonmedical exposure may last longer or fluctuate, particularly when another substance is involved.
Duration alone does not prove safety. A person who remains unable to speak or move normally after the expected observation period, or whose symptoms are worsening rather than settling, should be assessed by a healthcare professional. Do not drive, operate equipment or leave a sedated person alone; the treating service’s instructions take priority.

MedlinePlus’ esketamine guidance advises calling emergency services if the person has collapsed, had a seizure, has trouble breathing or cannot be awakened. These are emergencies regardless of whether ketamine was prescribed or obtained elsewhere.
- Call emergency services immediately for trouble breathing, blue or gray lips, collapse, a seizure, or inability to wake the person.
- Seek urgent medical assessment for severe or persistent inability to move or speak, rapidly worsening confusion, repeated vomiting, chest symptoms, or new weakness that does not improve as the acute effects wear off.
- Tell responders exactly what was taken, including ketamine or esketamine, the route if known, the approximate time, and any alcohol, opioids, sedatives or other drugs. Accurate information helps clinicians protect breathing and choose appropriate monitoring.
Do not force food or drink on someone who is drowsy, leave them unobserved, or assume that a lack of movement is harmless. If the person is in a supervised clinic, alert staff immediately; if not, contact emergency services.

What clinicians monitor during supervised treatment
Observation is part of the safety design, not an optional extra. For ketamine injection, the trained provider monitors during administration and afterward. In supervised esketamine programs, specialist nurses observe the patient until acute effects pass. Monitoring allows staff to respond to changes in alertness, breathing, circulation, coordination, agitation or involuntary movements while the dissociative state resolves.
Patients should report symptoms rather than trying to hide them. Tell the clinician if you cannot understand instructions, cannot speak clearly, feel detached from your body, have unusual twitching, or notice breathing changes. The team can distinguish an expected transient effect from a complication only by assessing the person in context.
Why one person may be affected more than another
Responses vary with the specific medicine, dose, route, prior exposure, other medicines or substances, underlying health and environment. The same person may have different experiences on different occasions. A quiet, monitored clinical setting also differs fundamentally from an unobserved or mixed-substance exposure.
Because the available sources do not establish one mechanism for every episode, it is safest to treat the explanation as a synthesis: altered sensory integration and dissociation can impair purposeful movement and communication while muscle tone and some reflexes remain present. The symptom pattern still needs context and, when severe or prolonged, an examination.
Key distinction to remember
Ketamine can make you feel unable to move or speak because your conscious intention, sensory feedback and motor coordination no longer line up normally. That is consistent with dissociation, not proof of paralysis. However, breathing difficulty, collapse, seizure, inability to wake, or severe and persistent impairment is not something to watch at home. Use the emergency guidance above and obtain professional help.
