Freezing can damage red blood cells because ice removes water from the liquid around them, concentrating salts and other solutes. The cells shrink, their membranes experience osmotic and permeability stress, and additional injury can occur from the cryoprotectant and from thawing. It is not simply a matter of keeping blood very cold.

Blood banks reduce that injury with a validated sequence: add glycerol before freezing, store the component at −65 °C or colder, thaw it under controlled conditions, wash the cells through progressively lower sodium-chloride concentrations, and verify recovery and residual glycerol before transfusion. These are professional blood-component procedures, not instructions for freezing blood in a household freezer.

What happens to a red cell as blood freezes?

Red blood cells are bags of water and dissolved substances bounded by a flexible membrane. When extracellular water begins to freeze, relatively pure ice forms first. Salts, sugars and proteins remain in the unfrozen fraction, so the remaining liquid becomes progressively more concentrated. The physical environment around each cell therefore changes while the temperature falls.

D. E. Pegg and M. P. Diaper described slowly frozen erythrocytes this way: “When cells are frozen slowly in aqueous suspensions, the solutes in the suspending solution concentrate as the amount of ice increases; the cells undergo osmotic dehydration and are sequestered in ever-narrowing liquid-filled channels.” The cells lose water to the concentrated outside solution and shrink. As the liquid channels narrow, cells can also be exposed to mechanical crowding and steep local chemical gradients.

Osmotic dehydration is only one part of the injury

Concentrated solutes can disturb proteins and the lipid structure of the membrane. Membrane permeability may change, allowing an abnormal movement of water and ions during both freezing and thawing. A cell that survives the frozen interval can therefore rupture later, when water rapidly re-enters it or when a damaged membrane can no longer control its contents.

Freezing injury is consequently multifactorial. Ice-related effects, solution concentration, osmotic changes, membrane permeability, the rate and temperature of freezing, exposure to the cryoprotective solution, thawing conditions and glycerol removal all influence post-thaw hemolysis. Ice crystals alone do not explain every damaged cell, and keeping a unit cold without controlling those variables does not make freezing harmless.

Why laboratory blood is different from household-frozen blood

A blood-bank component is collected, processed, labeled and stored under regulated procedures. A household freezer has uncontrolled temperature changes, slow and uneven cooling, no validated container system and no process for removing cryoprotectant after thawing. Those differences matter because the survival of a red-cell unit depends on the complete process, not merely on reaching a low temperature.

Frozen red cells are generally prepared for a specific inventory or clinical need, such as preserving an uncommon antigen phenotype. The unit is handled with equipment and procedures validated by the blood center, the applicable standards and the manufacturer of any processing system. A person should never attempt to freeze, thaw or wash a transfusion unit outside that setting.

How glycerol protects cells during freezing

Glycerol is a cryoprotective agent. Added before freezing, it changes how water and solutes behave as ice forms and helps reduce the damaging osmotic and membrane stresses that would otherwise occur. The protection is a trade-off: glycerol itself is present at a high concentration and cannot remain in the component that will be infused.

The AABB Circular of Information states that glycerol must be removed from the thawed component before infusion. If a glycerol-rich unit were given directly, the sudden change in extracellular osmolality could draw water into red cells and contribute to osmotic lysis. Deglycerolization is therefore a required processing step, not an optional refinement.

The process is a chain, not a single setting

Adding glycerol, equilibrating the cells, freezing at a validated rate, storing at the required temperature, thawing, washing and resuspending the cells each affect recovery. A change in one stage can alter the stress imposed at the next stage. For that reason, a blood center follows its validated procedure and the instructions for its specific processing device rather than substituting a generic recipe.

Storage temperature and the controlled workflow

The AABB Circular describes frozen red blood cells as prepared by adding glycerol before freezing at −65 °C or colder. The temperature is a product and labeling requirement in that description; it is not a recommendation for an ordinary freezer. Blood centers also control the container, monitoring, inventory records and thawing method.

  1. Select and prepare the unit. The laboratory confirms the component identity, testing and intended use, then adds glycerol using its validated equipment and procedure.
  2. Freeze and document. The unit is frozen at the required temperature, with continuous records and controls appropriate to the facility and product.
  3. Thaw in the blood bank. Thawing is performed with controlled equipment and timing so that the cells are not exposed to an abrupt, unmanaged change in temperature or solution composition.
  4. Deglycerolize by washing. The thawed cells are washed with progressively lower sodium-chloride concentrations. The final product is placed in 0.9% sodium chloride, with or without small amounts of dextrose, as specified by the controlled procedure.
  5. Inspect, test and label. The laboratory checks the component against its validated quality requirements, records the system type and expiry, and releases it only when the product meets those requirements.

This sequence is a high-level description. It is not a stand-alone glycerolization or deglycerolization protocol: concentrations, volumes, centrifugation settings, temperatures, hold times and acceptance tests must come from the current controlled procedure and device instructions.

How deglycerolization limits post-thaw injury

Washing removes glycerol and replaces the surrounding solution gradually rather than exposing the cells to an immediate, large osmotic jump. AABB describes progressively lower sodium-chloride concentrations followed by suspension in 0.9% sodium chloride, with or without small amounts of dextrose. The progression gives water and ions time to redistribute while reducing the risk of sudden swelling and lysis.

Centrifugation has long been used to add and remove glycerol. Commercial cell washers are also common in U.S. practice. A cell washer is professional blood-bank equipment with a validated program; its existence does not make home processing safe or acceptable. The device, tubing set, software and operator procedure must match the component and the facility’s validation.

Expected recovery

The June 2024 AABB Circular describes deglycerolized units as containing 80% or more of the red cells from the original unit. That figure is a product-description and quality target, not a guarantee that every unit has identical recovery. Actual recovery can be influenced by the original component, the processing method, handling and the equipment.

A proposed 35th-edition standards draft also used a mean recovery of at least 80% following deglycerolization. Because that document was a proposal for comment, it should not be treated as operative standards text. For compliance decisions, the current paid AABB standards and the controlled Circular are the authoritative documents for the facility.

Open and closed systems determine the post-thaw clock

The time available after thawing depends on whether the deglycerolization process is open or closed. The distinction is clinically consequential because opening the system changes contamination control and storage conditions.

Processing system Post-thaw handling described by AABB What controls the final decision
Open system Transfuse within 24 hours after thawing. The controlled Circular, current standards, local procedure and product labeling.
Closed system Store at 1–6 °C and transfuse within a two-week interval, subject to manufacturer instructions. The validated closed pathway and the applicable device and component instructions.

The two-week interval is not a blanket expiration for every frozen unit. It applies to the closed-system description and remains subject to the manufacturer’s instructions and the facility’s validated process. Staff must record the thaw time, system status and assigned expiry rather than relying on memory or a generic label.

Why hospitals keep frozen units

Rare antigen phenotypes

Frozen storage is especially useful for red-cell units with unusual antigenic phenotypes. A blood center can preserve a rare donor unit for a patient who may otherwise have no compatible standard component when transfusion is urgently needed. This is an inventory and compatibility strategy, not a claim that frozen cells are superior for routine transfusion.

When standard components are unsuitable or unavailable

The Circular describes deglycerolized red cells as useful when standard components are inappropriate or unavailable. A thawed unit may therefore be selected for a particular compatibility problem, an uncommon phenotype or a carefully documented inventory shortage. The transfusion service still evaluates the patient’s clinical indication and compatibility requirements.

Removing plasma constituents

Washing removes much of the residual plasma along with glycerol. That can be helpful for a patient with a previous severe allergic transfusion reaction, when the clinical team determines that a plasma-reduced or washed component is appropriate. Washing is not a universal solution for every transfusion reaction and does not eliminate the need for investigation, product selection and monitoring.

Quality checks and regulatory boundaries

Blood centers use quality controls intended to minimize post-thaw hemolysis and confirm that the component remains suitable for transfusion. Depending on the validated product and applicable requirements, checks can include visual inspection, cell recovery, free hemoglobin, residual glycerol and other component specifications. The purpose is to detect processing injury rather than assume that a successful thaw means every cell is intact.

The FDA maintains limited exceptions and alternative procedures under 21 CFR 640.120(a). The cited exceptions include specific situations involving rare antigen-negative frozen units and may require inspection and tests such as recovery, free hemoglobin and residual glycerol. They are approvals for the described circumstances, not general permission to change routine storage, thawing or washing.

The current AABB Standards for Blood Banks and Transfusion Services are the 35th edition, effective April 1, 2026. A search result for a proposed draft does not replace that operative edition. The June 2024 Circular was formally recognized by the FDA on September 25, 2024 as an extension of labeling; AABB describes it as a controlled document that must not be revised. Facilities should use the current controlled Circular, operative standards, local procedures and product-specific directions.

Common failure modes and how laboratories avoid them

Assuming low temperature alone is protective

Uncontrolled freezing can create damaging concentration gradients and membrane stress. Laboratories validate the freezing and thawing pathway rather than treating the storage temperature as the only important variable.

Skipping or rushing glycerol removal

Residual glycerol can create an osmotic hazard when the cells are infused. Progressive washing, the final saline resuspension and an appropriate residual-glycerol check address that risk.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Using the wrong expiry for the system

An open-system unit and a closed-system unit do not share the same post-thaw time limit. Staff identify the system, document the thaw time and apply the correct interval, including the 24-hour open-system limit or the closed-system 1–6 °C, two-week description when applicable.

Substituting a generic procedure for validated instructions

Changing a washer program, solution concentration, container or hold time can change recovery and hemolysis. The validated local procedure and manufacturer instructions take precedence over an online summary.

What the evidence does—and does not—show

The evidence supports a physical and chemical explanation: ice formation concentrates solutes, cells dehydrate, membranes face osmotic and permeability stress, and cryoprotectant exposure and handling contribute to injury. It does not support the simpler claim that ice crystals alone account for all damage, nor that every thawed unit has the same recovery.

For patients and clinicians, the practical conclusion is that frozen red cells are specialized blood-bank components. They can preserve rare compatibility characteristics, but they require glycerol protection before freezing, controlled thawing, validated deglycerolization, quality checks and time-limited storage after thawing. Decisions about whether to use one belong to the transfusion service, which applies current standards and the component’s labeling to the individual clinical situation.
