Targeted therapy can stop controlling a cancer when resistant cells were present before treatment or when surviving cells adapt. Resistance may involve a changed drug target, an alternate growth pathway, wider changes inside the cancer cell, or the tumor’s surrounding environment. More than one mechanism can occur at the same time.
A scan or other finding that raises concern is the starting point for a careful discussion—not proof by itself of a particular resistance mechanism. Patients can ask what evidence shows loss of control, whether biomarker testing could clarify the situation, how a result would change choices, and how the next treatment will be monitored.
What targeted therapy is—and what resistance means
Targeted therapies are medicines designed to act on proteins or other features that help cancer cells grow, divide, or spread. Because the drug is aimed at a specific feature, clinicians generally test a tumor for a matching biomarker before selecting treatment. The match can make a treatment reasonable to try, but it cannot guarantee that every cancer cell will remain sensitive indefinitely. The National Cancer Institute (NCI) explains the basic principle in its overview of targeted therapy.

Resistance means that cancer cells survive despite exposure to a treatment that previously controlled them or was expected to control them. A treatment may never work well because resistant cells were already present, or it may work for a time and then lose effectiveness as surviving cells change. Doctors establish what is happening by looking at the whole clinical picture, including examinations, blood tests, imaging, symptoms and the timing of findings; NCI describes these types of regular assessments for monitoring response.

Two broad patterns: intrinsic and acquired resistance
Intrinsic resistance is present from the start
Intrinsic resistance means that resistant cells or molecular changes exist before the first dose. A tumor is not necessarily uniform: most cells may carry the feature that makes the drug a good match, while a smaller group already has another change that allows it to survive. The sensitive population can shrink, leaving the resistant population to become more apparent. A lack of an initial response can therefore reflect biology that was present before treatment rather than a change caused by the medicine.
Acquired resistance develops during treatment
Acquired resistance emerges after treatment has placed pressure on the tumor. Cells that survive can adapt, select for new alterations, or use pre-existing minor populations that become dominant. The NCI’s discussion of treatment resistance describes resistance as a process that can involve several types of change rather than one universal event.
How a cancer can evade a targeted drug
The drug’s target can change
A change in the protein or other molecular feature targeted by a medicine may prevent the drug from binding or otherwise interrupting the intended signal. If the target’s structure, amount, or location changes, the original treatment may no longer have the same effect. A new mutation is one possible explanation, but resistance is not always caused by a newly acquired mutation.

Cancer cells may activate another route to grow or survive. The original pathway can be blocked while a parallel pathway, a downstream signal, or a backup growth program carries on. In that situation, the medicine may still be hitting its intended target, but the cell has found a way around the blockade.

Changes can occur beyond the immediate target
Resistance can involve broader changes in how a cell behaves, how it interacts with a drug, or how it controls growth and survival. Altered cell states and other molecular changes can make a tumor less dependent on the feature that was originally targeted. The tumor microenvironment—the surrounding cells, blood vessels, signaling molecules and supporting tissue—can also influence whether cancer cells persist.
Several mechanisms may coexist
Different parts of one tumor can use different escape routes. One biopsy or test may identify an important change without explaining every resistant cell. This is why an oncology team interprets test results in the context of tumor type, prior treatment, disease sites and the specific question the test is intended to answer.
What evidence suggests that a treatment is losing control?
Patients often hear that a treatment is “not working” before they know exactly why. Ask the team to identify the evidence and its level of certainty. A change on imaging, a pattern in blood tests, findings on examination or new symptoms may prompt concern, but the meaning depends on the cancer and the treatment schedule. NCI notes that clinicians use regular visits, examinations, blood tests and imaging to assess response. The most useful question is not simply whether a scan is “good” or “bad,” but what trend the team sees and whether it meets their definition of progression.
Ask whether the finding could represent growth in one site while other sites remain controlled, and whether the team needs confirmation or another type of measurement. Do not assume that a single result identifies the resistance mechanism. The oncologist may recommend reviewing prior pathology, comparing scans over time, or testing a new sample before changing treatment.

How biomarker testing may clarify the next decision
Biomarker testing can look for genomic or other features that help select treatment and identify possible clinical trials. A result may point toward an FDA-approved therapy, an off-label use, or a trial, but its meaning depends on the cancer type, the exact result and the patient’s treatment history. Testing is not automatically useful in every situation, and a test should be tied to a decision the team might make.
Tissue testing: what a biopsy can provide
A tissue biopsy removes a sample from a tumor or a site of suspected progression. It can allow pathologists to examine cancer cells directly and perform molecular tests chosen for the clinical question. The procedure can involve preparation, local or general anesthesia, discomfort, bleeding, infection or other risks that vary with the body site and the patient’s health. Ask where the sample would come from, whether the site is safely accessible, what complications are possible, how much tissue is needed and how long results may take.
Blood-based or other liquid biopsy
A liquid biopsy analyzes biomarkers from cancer cells or cancer-related material in blood or another body fluid. It avoids a procedure aimed at removing tissue, but whether it can answer a useful question depends on the assay, the cancer and the clinical situation. Ask which biomarkers the test can detect, what a positive result would change, and what a negative or uninformative result would leave unresolved. NCI describes liquid biopsy as a clinical tool whose usefulness must be judged for the individual case.

A test result is useful only if the team knows how it could change management. Ask whether the proposed test is intended to confirm resistance, find a new target, rule out an option, or locate a trial. Do not substitute consumer genetic testing for a clinically selected tumor test; the tests answer different questions.

Questions to take to your oncologist
NCI recommends preparing questions and bringing a written list to appointments. You can select the prompts that fit your situation and ask for plain-language explanations.
Clarifying what is happening
- “What evidence shows that my cancer is growing or that this treatment is no longer controlling it?”
- “What did the latest examination, blood test or scan show compared with earlier results?”
- “Could the finding involve only one area, or does it suggest a broader change?”
Discussing biomarkers and repeat testing
- “Could a change in the tumor’s biomarkers help explain what is happening?”
- “Would another tumor test be useful now?”
- “What would a tissue biopsy involve, and what are its risks in my case?”
- “Could a blood-based liquid biopsy answer a useful question for me, and what would it not tell us?”
- “How would a test result change the treatment options we consider?”
Comparing treatment paths
- “Are there approved options, off-label options or clinical trials that fit my cancer and test results?”
- “What benefits, side effects and uncertainties should I weigh for each option?”
- “Would the goal be to shrink the cancer, slow it, relieve symptoms or maintain control?”
- “What factors in my health or previous treatments make one option more or less suitable?”
Planning follow-up
- “How will we monitor whether the next treatment is working?”
- “When will we repeat imaging or other tests, and what result would lead us to change course?”
- “Which symptoms or side effects should prompt a call before the next appointment?”
How the answers affect treatment choices
Approved treatment
If testing identifies a feature with an approved therapy for the relevant cancer, the team can discuss how that option fits the patient’s prior treatment, overall health and goals. Approval applies to a defined indication; a biomarker match alone does not establish that every drug aimed at the feature is appropriate.
Off-label treatment
An off-label option may involve an approved medicine used in a different cancer, line of therapy or clinical circumstance. Ask what evidence supports the proposed use, how confident the team is, how it would be obtained and which uncertainties are greater than with an indicated treatment.
Clinical trial
A trial may test a new drug, a combination, a sequencing strategy or a way to delay or overcome resistance. Eligibility can depend on tumor type, biomarker results, prior therapies, organ function and other criteria. A trial is not a guarantee of benefit, and an experimental strategy should not be presented as standard care. Ask about the trial’s purpose, known and unknown risks, required visits, alternatives and what happens if the treatment does not help.

Benefits, harms and uncertainty
For every option, ask how likely and how quickly a benefit might appear, which side effects are common or serious, how they would be managed, and what is still unknown. The best choice can differ between people with the same biomarker because disease distribution, prior toxicities, other medical conditions and personal priorities differ.
A melanoma research example—and its limits
An NCI report dated March 17, 2023 described melanoma research into extrachromosomal DNA and complex genomic rearrangements as possible routes to genetic changes associated with resistance. Investigators were developing a clinical trial to test a proposed strategy. The report quoted Roger Lo, M.D., Ph.D., of UCLA Jonsson Comprehensive Cancer Center: “Inevitably, cancer cells capable of generating many versions of themselves will chance upon certain genetic variants that are able to resist a therapy.”
This example illustrates how researchers investigate resistance; it does not explain every patient’s cancer, establish a universal mechanism, or make the proposed strategy standard treatment. Ask whether evidence from a study applies to your cancer type and treatment history before drawing conclusions.

Preparing for a productive appointment
- Write down the treatment name, start date, doses or schedule, and any previous cancer treatments if you have those records.
- List new symptoms, when they began, how they affect daily activities, and any side effects that have changed.
- Bring imaging reports, pathology reports and prior biomarker results when available; ask the clinic which records it already has.
- Put your most important three questions first in case time is limited.
- Ask the clinician to define unfamiliar terms such as “progression,” “resistance,” “biomarker,” “off-label” and “trial.”
- Take notes or bring a support person, according to the clinic’s policy, so you can review the options later.
- Ask who to contact with follow-up questions and when the team expects to have test results.
What to expect from the conversation
There is no single resistance treatment that applies to every cancer. The appropriate next step may be additional observation, a new biopsy or liquid biopsy, a different approved medicine, an off-label discussion, a clinical trial or supportive care, depending on what the evidence shows and what matters most to the patient. The purpose of the questions is to connect each test and option to a specific decision, while making the expected benefits, harms and uncertainties clear.
