A fever after vaccination usually reflects the immune system responding to vaccine antigens or other immune signals while it builds protection. It does not mean the vaccine gave you the disease, and a fever alone does not prove how much immunity you developed. Most vaccine side effects are mild and improve within a few days, but some symptoms require urgent or emergency care.
The right response depends on the person’s age, the vaccine received, when symptoms began, how high the temperature is, and whether breathing, heart, neurologic, or allergic symptoms are present.
Why a vaccine can cause a fever
Vaccines train the immune system to recognize and fight a germ. As immune cells respond to the vaccine, the body can produce temporary effects such as soreness, fatigue, chills, body aches, and fever. The Centers for Disease Control and Prevention (CDC) describes these as possible signs of the immune system building protection.
Some vaccines contain adjuvants, ingredients designed to produce a stronger immune response. CDC says adjuvanted vaccines can cause more local reactions and more systemic reactions, including fever, chills, and body aches, than non-adjuvanted vaccines. That general fact cannot identify which ingredient caused one person’s fever.

A post-vaccination fever is therefore a recognized side effect, not an infection caused by the vaccine. However, a symptom that starts after vaccination could also have another cause, such as an unrelated viral illness. Do not assume every symptom is harmless or automatically caused by the shot.
What the fever does and does not mean
Fever is one possible sign of immune activity, but it is not a test of vaccine effectiveness. Some people develop protection without a noticeable fever or other side effect, while others have several short-lived symptoms. A higher temperature does not establish that immunity is stronger, and no fever does not show that the vaccine failed.
The body’s immune response continues after symptoms fade. For COVID-19 vaccination, CDC explains that it can take a few weeks for the body to produce T- and B-lymphocytes. That timeline describes immune development, not how long a fever should last.

How long fever lasts and when it may start
There is no single schedule that applies to every vaccine. CDC says most vaccine side effects are minor and go away within a few days, but reactions differ by vaccine. Check the Vaccine Information Statement (VIS) for the specific product you received.
| Situation | What the evidence says about timing | How to interpret it |
|---|---|---|
| Many routine vaccines | Fever and other systemic effects are generally short-lived and usually resolve within a few days. | A mild fever that is improving fits the common pattern, but worsening or severe symptoms need medical advice. |
| Live attenuated vaccines | CDC general immunization guidance says fever or rash can occur later, often 3–21 days after administration, because the vaccine virus may replicate for several days. | This interval is specific to the live-attenuated context; do not apply it to every vaccine. |
| COVID-19 vaccination | The body may take weeks to produce T- and B-lymphocytes, but a fever does not have to last that long. | The duration of immune development is not a prediction of fever duration. |
The absence of fever does not mean a vaccine failed, and having a fever does not establish that immunity is complete.
Why timing can differ
Vaccine formulations and immune signals differ. An adjuvanted vaccine may produce more local or systemic reactions than a comparable non-adjuvanted product. With a live attenuated vaccine, the virus in the vaccine may replicate for several days, so fever or rash can appear later. These are broad patterns, not a way to diagnose the cause of an individual fever.

When a post-vaccine fever is an emergency
Call emergency services (911 in the United States) and go to the nearest hospital if a person develops signs of a severe allergic reaction after vaccination:

Do not wait to see whether throat or breathing symptoms pass. Severe allergic reactions can progress quickly and require emergency treatment.
Age-specific fever warning for infants
Any temperature of 100.4°F (38°C) or higher in a child younger than 12 weeks is an emergency warning sign. Contact emergency medical services or obtain urgent emergency evaluation rather than managing the fever at home. The CDC page providing this threshold addresses respiratory illness generally, not vaccination specifically; the age-based warning still matters when the fever follows a shot.
Use a thermometer to verify the temperature if one is available, but do not delay emergency care while repeatedly checking it when the infant appears seriously ill. The CDC respiratory guidance carrying this age threshold was updated September 10, 2026, so caregivers should also follow any newer instructions from the child’s clinician.

Fever and febrile seizures in children
A febrile seizure is a seizure associated with a fever. CDC says most occur between 6 and 59 months of age, and up to 5% of young children will have at least one febrile seizure (CDC, 2026). Febrile seizures can occur with many fever-causing illnesses, including a cold, influenza, or an ear infection, not only after vaccination.
CDC describes a small increased risk of febrile seizures during the 24 hours after a child receives an inactivated influenza vaccine at the same visit as PCV13 or a DTaP-containing vaccine. That finding applies to the studied combination and timing; it should not be generalized to every vaccine or schedule. Influenza illness itself can cause high fever and febrile seizures. CDC’s febrile-seizure page is dated September 1, 2026.
Any seizure is a reason to seek urgent medical care. Place the child on a safe surface on their side, remove nearby hard objects, do not restrain them, and do not put anything in the mouth. Call emergency services if the seizure lasts five minutes or more, breathing is difficult, another seizure follows, or the child does not recover normally.
What to do for a mild fever or soreness
- Measure the temperature. A digital thermometer can help you compare the reading with age-based guidance. A temperature reading does not determine the cause by itself.
- Observe the overall condition. Note alertness, breathing, fluid intake, urination, rash, pain, and whether symptoms are improving or worsening.
- Offer fluids and rest. Dress the person comfortably and avoid overheating. Encourage age-appropriate fluids.
- Follow the vaccine’s VIS. It lists expected reactions and when to contact a clinician for that product.
- Use fever medicine only when appropriate. CDC says evidence does not support taking antipyretics before or at the time of vaccination to prevent reactions. They may be used afterward to treat fever or local discomfort, but the medicine and dose must match the person’s age, weight, health conditions, and other medicines. Ask a clinician or follow the product label, especially for infants and children.
Do not give a child medication simply because a vaccine was administered, and do not use an adult product or dose for a child. A pharmacist or clinician can help when the label is unclear, the child has a chronic condition, or other medicines are being taken.
When to call a healthcare professional without calling 911
Contact a healthcare professional for advice when symptoms are severe, persistent, worsening, or unlike the expected effects listed for the vaccine. Also call when the person cannot keep fluids down, seems unusually difficult to wake, has a concerning rash, or you are uncertain how to interpret the temperature or symptoms.
There is no universal temperature cutoff for every age, vaccine, and situation in the guidance cited here. Clinicians may use the person’s age, medical history, vaccine type, timing, measured temperature, and accompanying symptoms together rather than relying on one number.

How to decide between monitoring, a call, and emergency care
| Pattern | Best next step |
|---|---|
| Mild fever or soreness, person is alert, breathing normally, drinking, and symptoms are improving within the expected period | Monitor, provide comfort, and follow the product-specific VIS. |
| Symptoms are persistent, worsening, unusually severe, or concerning but there are no immediate breathing or circulation problems | Call a healthcare professional for individualized advice. |
| Hives with facial or throat swelling, breathing difficulty, faintness, collapse, or other severe allergic signs | Call 911 and go to a hospital immediately. |
| Fever at least 100.4°F (38°C) in a child younger than 12 weeks | Seek emergency evaluation immediately. |
| Chest pain, shortness of breath, or a fast, fluttering, or pounding heartbeat after a COVID-19 vaccine | Seek medical attention right away. |
| Any seizure, especially one lasting five minutes or more or followed by poor recovery | Call emergency services and protect the child from injury while waiting. |
Common misunderstandings
“The fever means the vaccine caused the disease.”
Vaccination can trigger immune symptoms without causing the disease it is designed to prevent. A fever after a shot is not proof of infection.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitches“A stronger fever means stronger immunity.”
There is no basis in CDC guidance for using fever intensity as a measure of protection. People can develop immunity with or without noticeable side effects.
“All vaccine fevers should start the same day.”
Timing varies. Some live attenuated vaccines can produce fever or rash later, and the 3–21-day interval described by CDC should not be imposed on other vaccine types.
“I should take a fever reducer before every shot.”
CDC says evidence does not support taking antipyretics before or at vaccination to prevent reactions. If fever or soreness develops afterward, ask a clinician or follow the label about whether treatment is appropriate.

Frequently Asked Questions
Is a fever after a vaccine normal?
A short-lived fever is a recognized possible side effect because the immune system is responding to vaccine antigens or other signals. Most vaccine side effects are minor and improve within a few days, but fever is not automatically harmless. Consider the person’s age, symptoms, timing, and the vaccine’s VIS.

Can a vaccine give me the disease it prevents?
A fever after vaccination does not mean the vaccine gave you the disease. It can reflect temporary immune activity. However, an unrelated infection can begin around the same time, so severe, persistent, worsening, or unusual symptoms still warrant medical advice.
When is fever in a baby an emergency?
Any temperature of 100.4°F (38°C) or higher in a child younger than 12 weeks is an emergency warning sign, even when it follows vaccination. Seek emergency evaluation rather than relying on home observation.
Should I give acetaminophen or another fever medicine before vaccination?
CDC says evidence does not support taking antipyretics before or at the time of vaccination to prevent reactions. Medicine may be used afterward for fever or discomfort when appropriate, but the product and dose must fit the person’s age, weight, conditions, and other medicines.
What symptoms after a COVID-19 vaccine need immediate attention?
Seek medical attention right away for chest pain, shortness of breath, or a fast-beating, fluttering, or pounding heartbeat. These symptoms need assessment even if a fever is mild or has resolved.
How common are febrile seizures in young children?
CDC says most febrile seizures occur from 6 through 59 months, and up to 5% of young children have at least one. A small increased risk was described during the 24 hours after certain simultaneous inactivated influenza and PCV13 or DTaP-containing vaccinations; that finding does not apply to every vaccine combination.
Do all vaccine fevers start right away?
No. Effects vary by vaccine. CDC notes that fever or rash after live attenuated vaccines may occur later, often 3–21 days after administration. Do not apply that interval to every vaccine.
Should I buy a thermometer?
A digital thermometer can help you measure temperature and compare it with age-based guidance, particularly the 100.4°F (38°C) threshold for infants younger than 12 weeks. It does not diagnose the cause of a fever, and measuring should not delay emergency care.

A mild fever after vaccination is often a temporary immune response and improves within a few days. Do not treat every fever as automatically benign: call 911 for severe allergic symptoms, seek emergency care for a fever of 100.4°F (38°C) or higher in a child younger than 12 weeks, get prompt assessment for chest or heart symptoms after a COVID-19 vaccine, and seek urgent help for seizures or any rapidly worsening condition.
