Apache usually is not “ignoring” a rewrite rule at random. The selected VirtualHost may point at a different DocumentRoot, the matching <Directory> block may disable overrides, mod_rewrite may be off, or the rule may use server-context syntax inside a per-directory file. Start by proving which VirtualHost handled the request and whether that host permits .htaccess; only then debug the regular expression or substitution.
What happens to a request before .htaccess is read
Apache evaluates configuration in stages. DNS and the request’s Host header select a VirtualHost. That host supplies a DocumentRoot and directory policy. Apache then maps the URL to the filesystem and, during later request-fixup processing, reads eligible per-directory files such as .htaccess. Server- or VirtualHost-level rewrite directives therefore run before per-directory rules.
This order explains several otherwise confusing symptoms:
- A rule can work in the main configuration but never reach
.htaccessif the selected host’s directory policy disallows overrides. - An identical rule can work on one domain and fail on another because the domains select different VirtualHosts or different document roots.
- A rule that matches in a VirtualHost can fail in
.htaccessbecause Apache removes the local directory prefix before applying the per-directory pattern.
The first question is not “is my regular expression correct?” It is “which host and directory did Apache actually use, and was that directory allowed to read this file?”

The override policy that can make .htaccess completely invisible
In current Apache 2.4 behavior, AllowOverride defaults to None. When both AllowOverride None and AllowOverrideList None apply to a directory, Apache completely ignores .htaccess files there. No RewriteRule inside the file can run, regardless of its pattern.
The policy must be set in a non-regular-expression <Directory> section that matches the actual filesystem directory. A typical document-root configuration is:

</Directory>Do not place this policy only on a guessed path. The path must match the directory Apache uses after the chosen VirtualHost’s DocumentRoot, including any subdirectory in which the file resides. A policy for /var/www/site does not automatically describe a file under /srv/www/site.
Confirm the request selected the intended VirtualHost
A common “only one domain fails” case is a request reaching the default host or another host with a similar document root. Check all of the following before editing rewrite syntax:
- Verify that DNS for the hostname resolves to the server you are testing.
- Send the exact hostname in the HTTP
Hostheader. Testing by IP address commonly selects a different VirtualHost. - Inspect Apache’s active virtual-host map with the configuration-dump or virtual-host listing tool supplied by your deployment. Confirm the hostname, address/port, and file that define the selected host.
- Read that host’s
DocumentRoot. It must be the filesystem tree containing the.htaccessyou edited. - Check for a more specific host, port, or wildcard definition that wins before the one you intended.
Make a harmless, temporary change in the suspected document root—such as creating a uniquely named test file—and request that URL. If the file is not served, you are troubleshooting the wrong root or host, not a rewrite rule.

Make sure Apache can read the file
Use the correct filename
The conventional filename is exactly .htaccess: lowercase, with the leading dot, and no trailing extension. Apache can be configured with another AccessFileName, so check that setting if your deployment intentionally uses a different name. A file called htaccess, .htaccess.txt, or .HTACCESS is not the conventional file.
Check directory and file access
The Apache worker must be able to traverse every parent directory and read the file. An operating-system permission problem normally appears in the error log, while an override-policy problem can look like a perfectly ordinary request because the file is silently out of scope. Keep the two checks separate: filesystem permissions answer “can Apache read it?” and AllowOverride answers “is Apache permitted to use its directives?”
Reload after server-configuration edits
Changes to a VirtualHost, <Directory> policy, module loading, or AccessFileName require a configuration test followed by the reload or restart procedure used by your operating system. A stale worker configuration can make a correct edit appear ineffective. Changes made inside an eligible .htaccess are read per request, but a cache, proxy, or application response can still hide the result while you test.
Enable mod_rewrite in the scope that is actually used
RewriteEngine is off by default. Enable it in the applicable file:
RewriteEngine OnThe mod_rewrite module must also be loaded. On a server you administer, verify the loaded-module list through the platform’s Apache module tooling. On managed hosting, the provider must have enabled the module; an AllowOverride change cannot load a module that is absent.
Settings in the main-server context are not inherited by VirtualHosts by default. Therefore, enabling the engine globally does not prove that a particular host has the setting you expect. Put RewriteEngine On in that VirtualHost or in its eligible .htaccess, and do not assume another host’s configuration is shared.

A minimal root-level file might look like this:

RewriteRule ^old-page/?$ /new-page [R=301,L]Use a temporary redirect status while testing so that a browser does not permanently cache an incorrect destination. Once behavior is verified, choose the permanent status appropriate to your migration.

Remove the leading slash in an ordinary document-root file
This server-context rule is appropriate when the pattern includes the URL’s initial slash:
RewriteRule ^/old-page$ /new-page [R=302,L]In a document-root .htaccess, write the equivalent pattern without that slash:
RewriteRule ^old-page$ /new-page [R=302,L]Inside /var/www/example/public/app/.htaccess, a request for /app/index.php is normally matched as index.php, not /app/index.php. A pattern beginning ^/ therefore fails in ordinary per-directory use.
Account for the local directory
Rules in nested files are evaluated relative to their directory. If a rule is moved from the document root into an app subdirectory, remove the app/ portion from the pattern rather than copying the root rule unchanged. Keep conditions and substitutions consistent with that new context.

When RewriteBase helps—and when it cannot
RewriteBase does not turn rewriting on, load mod_rewrite, or permit an otherwise ignored file. It supplies the public URL prefix used for relative substitutions in per-directory context.
Most document-root installations using URL paths that mirror the filesystem do not need it. Consider it when an application is exposed through an Alias or another mapping in which the filesystem directory and public URL prefix differ. For example:

</Directory>If a relative substitution in that directory must be generated beneath the public /app/ prefix, an applicable file can declare:
RewriteBase /app/Use an explicit base only after confirming that the rule is being read and that the mapping is the problem. Adding it to every file can obscure the real cause and does not repair a wrong VirtualHost, disabled overrides, or a bad pattern.

Change one variable at a time and test one known URL. This prevents a corrected rule from being confused with a host, cache, or application change.
- Choose a deterministic test URL. Use a path whose expected result is unambiguous, and record the status code and
Locationheader if a redirect is expected. - Prove host selection. Confirm DNS, hostname, port, and the active VirtualHost map. Do not use an IP-only request as your domain test.
- Prove the document root. Verify that the selected host’s
DocumentRootcontains the exact file and a harmless marker resource. - Prove the filename. Check
.htaccessspelling or the configuredAccessFileName. - Prove override permission. In the matching non-regex
<Directory>, set eitherAllowOverride FileInfoor anAllowOverrideListcontaining every rewrite directive used. - Prove module and engine state. Confirm
mod_rewriteis loaded and placeRewriteEngine Onin the host or eligible file. - Simplify the pattern. In
.htaccess, remove the leading slash and test a literal path before adding captures, conditions, or complex flags. - Check ordering. Earlier rules, a broad catch-all, or the
Lflag can prevent a later rule from being reached. Conditions are evaluated before their associated rule. - Check the substitution. Decide whether it is a local path, a URL-path, or an external URL and whether relative-path handling requires
RewriteBase. - Reload and inspect logs. After server edits, run the deployment’s configuration test and reload procedure. Review the Apache error log; where permitted, temporarily enable rewrite trace logging to see rule matching, condition results, and substitutions, then return logging to its normal level.

If you control the server, keep host-wide rules in a dedicated VirtualHost. For a straightforward host-wide redirect, Apache’s configuration guidance favors a simple Redirect rather than a more elaborate rewrite. Use .htaccess when you lack main-configuration access or when behavior genuinely belongs to one directory and the provider permits it.

How to keep a working rule working
- Document which hostname, port, and document root each rule belongs to.
- Keep the
<Directory>policy beside the correspondingDocumentRootso a future move does not leave overrides enabled on the wrong path. - Prefer the smallest override permission that supports the file’s directives.
- Write separate tests for root-level and nested
.htaccessfiles; their match strings are different. - Use temporary redirects while developing and clear intermediary caches before judging a result.
- Remove temporary trace logging after diagnosis because verbose rewrite logs can expose request details and generate substantial volume.
