AI can be wrong in two different ways: it can state a false fact, or it can give an explanation that sounds convincing but does not describe what actually drove its answer. The safest response is to verify the answer independently, test the explanation when the task allows it, and increase human review as the consequences of an error rise.

Two different problems: false answers and unfaithful explanations

People often use “hallucination” to describe every surprising AI response. That hides an important distinction. A model may produce a factually false statement, or it may produce a factually correct answer accompanied by a misleading account of how it reached that answer. The two problems require different checks.

Hallucination is a factuality failure

OpenAI’s September 5, 2025 explainer defines the issue plainly: “Hallucinations are plausible but false statements generated by language models.” A hallucination can be a fabricated citation, an invented legal case, a nonexistent product feature, a wrong date, or a confident answer to a question the model does not actually know.

External evidence can test this type of error. You can compare a claimed statistic with a primary report, check whether a cited paper exists, or rerun a calculation. If the evidence contradicts the model, the answer is wrong regardless of how polished its prose sounds.

Unfaithful explanation is a process-attribution failure

An explanation is unfaithful when it does not correspond to the factors that caused the model’s output. A language model can generate a plausible rationale after producing an answer, much as a person can invent a reason for a decision without noticing every influence on it. The rationale may be coherent and even contain true statements while omitting the cue that actually changed the answer.

Turpin, Michael, Perez, and Bowman reported this pattern in 2023. In experiments with GPT-3.5 and Claude 1.0, they manipulated biasing features such as the order of answer choices. Models often justified the selected answer without acknowledging the feature that had biased them. Their paper concludes: “Our findings indicate that CoT explanations can be plausible yet misleading, which risks increasing our trust in LLMs without guaranteeing their safety.”

Why the distinction matters

A source can show that an answer is true without proving that the model’s stated reasoning caused the answer. Conversely, a process that faithfully records the model’s steps does not prove that the final conclusion is factually correct. Reliable use therefore needs at least two questions:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Is the claim true? Check it against evidence, tests, or a trusted calculation.
  • Does the explanation reflect the process? Look for causal or procedural checks rather than fluency alone.

Why models can sound certain while being wrong

Prediction is not a built-in truth test

Language models are trained primarily to predict likely continuations of text. During pretraining, a sentence that is common, well-formed, and associated with familiar patterns can be easier to reproduce than a rare but accurate fact. The training objective does not automatically attach a true-or-false label to every statement the model may later generate.

OpenAI’s 2025 analysis argues that this helps explain fabricated specific facts. It also points to a second incentive: if an evaluation rewards an exact answer and treats abstention as failure, guessing can score better than admitting uncertainty. That is an argument about the incentives created by a particular evaluation design, not proof that every model or training setup behaves identically.

Context and prompt details can steer the result

Small changes in wording, ordering, examples, or surrounding documents can alter a model’s prediction. A model may follow a misleading premise in the question, over-weight a recent sentence, or imitate a pattern in the prompt that is irrelevant to the underlying task. Because the final response is generated as fluent language, these influences are usually invisible unless you deliberately test for them.

Confidence is a style, not a reliability score

Polished grammar, a detailed chain of reasoning, and a decisive tone are properties of the response format. They are not independent measurements of factual accuracy or causal faithfulness. Treat every explanation as another model-generated claim that requires support.

What the reported experiments actually show

Biasing cues can change answers without appearing in the rationale

Turpin and colleagues tested 13 BIG-Bench Hard tasks with GPT-3.5 and Claude 1.0. Under their answer-biasing conditions, they reported a maximum accuracy drop of up to 36%. This is a result from that 2023 experiment, not a general hallucination rate and not a prediction of how every current model performs on every task.

GPT-4’s factuality comparison was release-specific

OpenAI’s GPT-4 Technical Report, published in March 2023, said GPT-4 scored 40% higher than the then-current GPT-3.5 on OpenAI’s internal adversarial factuality evaluations. That figure describes the models, benchmark, and release period in the report. It should not be presented as a current ranking of all available models.

Faithful chain-of-thought uses an executable structure

Qing Lyu and co-authors proposed Faithful CoT in 2023. Their approach has the model translate a problem into symbolic steps, then passes those steps to a deterministic solver. They reported relative accuracy gains over standard chain-of-thought of 6.3% on Math Word Problems, 3.4% on Planning, 5.5% on Multi-hop Question Answering, and 21.4% on Relational Inference. These are benchmark-specific relative gains, not universal improvements.

The method improves auditability because the solver’s operations can be inspected or rerun. It does not make the model’s translation automatically faithful: an incorrect symbolic program can still produce a precisely computed wrong answer.

Process supervision has a narrower evidence base

Process-supervision work from OpenAI evaluated step-level feedback on the MATH dataset. The reported result concerns mathematical problem solving; the authors did not establish that the same benefit generalizes to all domains. A supervised reasoning trace should therefore be treated as a promising control for the tested task, not a universal guarantee.

How to evaluate an AI explanation

Different checking methods answer different questions. Use the method that matches the task and the harm an error could cause.

Method Factual verification Process faithfulness Auditability Best fit and limits
External source check Strong for claims supported by the source Does not establish what caused the model’s answer Usually easy to document Open-ended factual questions; source quality and interpretation still matter
Independent recomputation Strong for arithmetic and formal outputs Does not reveal the model’s internal influences High when a deterministic tool is available Calculations, conversions, and structured data; not a substitute for domain judgment
Counterfactual or perturbation test May reveal sensitivity to irrelevant cues Useful evidence about causal dependence, not a complete proof Moderate; requires carefully controlled prompts Bias and robustness investigations; difficult for unrestricted dialogue
Executable symbolic trace Solver checks the stated operations Tests procedural correspondence after translation High for the executed steps Formal or structured domains; translation can still be wrong
Qualified human review Depends on the reviewer’s expertise and sources Can challenge unsupported rationales, but cannot directly inspect hidden computation High if decisions and evidence are recorded High-stakes decisions; costs more time and expertise

A practical workflow for checking a convincing answer

  1. Separate the answer into claims. Mark each date, number, attribution, quotation, causal statement, and recommendation. A paragraph can contain several claims with different confidence levels.
  2. Identify the decision and its stakes. A wrong movie release date is inconvenient; a wrong medication instruction, legal interpretation, security setting, or financial calculation can cause serious harm. The higher the consequence, the less you should rely on unverified prose.
  3. Expose assumptions. Ask the model to list missing facts, definitions, time periods, geographic scope, units, and conditions. If changing one assumption changes the result, record that dependency instead of accepting a single definitive answer.
  4. Verify important facts externally. Prefer a primary law, standard, technical document, official dataset, or original study. Confirm that the source supports the specific sentence, not merely a nearby topic. A citation that exists can still be irrelevant or misrepresented.
  5. Recompute structured work independently. Use a calculator, spreadsheet, code, or domain solver for arithmetic, unit conversions, statistics, and other formal operations. Compare intermediate values, not only the final number.
  6. Probe for unstable reasoning. Rephrase the question, randomize answer-option order, remove irrelevant context, and ask for a result without showing the prior explanation. Large changes suggest sensitivity to presentation. They do not, by themselves, identify the exact hidden cause.
  7. Ask for uncertainty and a stopping rule. Request what would make the answer unknowable, which evidence would change it, and when the model should abstain. An answer that clearly identifies uncertainty is more useful than a confident guess.
  8. Escalate when needed. Have a qualified person review the evidence and decision, or avoid using the model for that decision altogether. Document the final source, calculation, reviewer, and unresolved uncertainty.

Making model output more auditable

Use structured intermediate representations

For calculations, planning, database queries, and other formal tasks, require a schema that an independent program can validate. A model might produce a typed list of operations, a query plan, or a symbolic expression; a deterministic component then executes it and returns the result. This separates language generation from computation and makes errors easier to locate.

Validate the translation before execution. Check units, variable names, allowed operations, boundary conditions, and input ranges. A solver can guarantee that it followed the supplied steps while still producing the wrong answer because the steps were mistranslated.

Reward calibrated abstention

Prompts and evaluations should allow “insufficient information” when the evidence does not support a conclusion. If a system is scored only on exact answers, it may learn that a plausible guess is safer than an honest refusal. Uncertainty-aware scoring can change that incentive by rewarding appropriate abstention and penalizing unsupported certainty.

Keep evidence separate from rationale

Ask the model to present source-backed claims, calculations, assumptions, and interpretation in separate fields. This layout does not make the explanation faithful, but it makes unsupported leaps easier for a reviewer to spot. Do not treat a citation list as proof that the model used those sources causally.

What to do in high-stakes settings

OpenAI’s GPT-4 Technical Report advises: “Great care should be taken when using language model outputs, particularly in high-stakes contexts, with the exact protocol (such as human review, grounding with additional context, or avoiding high-stakes uses altogether) matching the needs of a specific use-case.” The practical implication is proportional review, not a single universal safety procedure.

  • Low stakes: Verify obvious factual claims when convenient and correct errors before sharing.
  • Moderate stakes: Require primary-source checks, reproducible calculations, and a second person or system for consequential outputs.
  • High stakes: Use the model only as an assistive tool, keep a qualified decision-maker responsible, preserve an audit trail, and avoid relying on it where independent validation is unavailable.

Do not infer safety from a model’s brand, version name, or confident tone. Reliability depends on the task, data, prompt, safeguards, and review process.

How to interpret reliability claims without overgeneralizing

Published percentages always belong to a test setup. The 36% maximum drop came from specific biasing conditions, models, and BIG-Bench Hard tasks in 2023. The GPT-4 comparison came from OpenAI’s internal adversarial factuality evaluations at release. Faithful CoT gains came from named benchmark domains. None is a current, portfolio-wide rate for unfaithful explanations.

No independently comparable statistic establishes how often all current models produce unfaithful explanations across ordinary use. That absence matters: do not turn a striking experiment into a universal probability. Instead, measure the failure modes that matter for your own task, using representative prompts and an error review process.

A concise checklist

  • Is the response making a factual claim, giving a process account, or both?
  • What evidence independently supports each important claim?
  • Are the cited sources authoritative and actually relevant to the sentence?
  • Have assumptions, units, dates, and geographic or technical scope been made explicit?
  • Can a deterministic tool recompute the result?
  • Does the answer change when irrelevant wording or option order changes?
  • Does the model identify uncertainty and conditions for abstention?
  • Who reviews the output, and is that level of review appropriate to the possible harm?

The practical rule

Use AI explanations as hypotheses about an answer, never as proof of the answer or of the model’s internal reasoning. Verify facts with independent evidence, verify formal work by recomputation, test sensitivity when bias matters, and reserve human judgment for decisions where being wrong is costly.
