A detailed prompt can make an AI answer more relevant, but it cannot guarantee that the answer is true. Hallucinations—plausible-sounding false statements—can result from unclear instructions, missing or noisy source material, or a model that misuses even relevant context. The practical fix is to identify which part failed, then test the change rather than keep adding words to a static prompt.

What “stagnation” in prompt engineering really means

Prompt stagnation is a useful way to describe a team repeatedly patching the same instructions without checking whether the changes solve the actual failure. It is not established as an independent cause of hallucinations. A prompt can leave key requirements unstated, but improving it will not supply facts that the model never received or make the model infallible.

OpenAI’s developer guidance treats prompt engineering, retrieval-augmented generation (RAG), and fine-tuning as different ways to optimize a system—not steps that every project must follow in order. Evaluation helps identify whether the problem is insufficient context or inconsistent behavior. OpenAI’s guide to optimizing LLM accuracy also cautions that a model can receive useful context and still answer incorrectly.

Why detailed prompts can still produce false answers

The instruction leaves room for inference

A prompt may be long yet fail to specify the task, relevant boundaries, required evidence, or what to do when information is missing. The model then has to fill in gaps. More words help only if they clarify the requirement or supply useful context.

The answer depends on facts the model does not have

For current or specialized questions, a prompt alone may not provide reliable source material. RAG can retrieve domain-specific information and place it in the prompt, but it adds a retrieval failure point: irrelevant or incorrect results can mislead the model, and too much unrelated context can bury the useful evidence.

The model misuses correct context

Retrieval is not a guarantee either. Even when the system provides relevant evidence, the model can misread it, overlook a constraint, or make an unsupported claim. Check both what was retrieved and how the answer used it.

The evaluation rewards confident guessing

If an evaluation gives no credit for an unanswered question but rewards a lucky guess, it can favor confident answers over appropriate uncertainty. OpenAI’s September 5, 2025 explainer, “Why language models hallucinate”, reports this SimpleQA example:

Model in OpenAI’s example Abstention Accuracy Error
gpt-5-thinking-mini 52% 22% 26%
o4-mini 1% 24% 75%

These figures describe the named models in that evaluation example, not universal hallucination rates or a general model ranking. OpenAI’s point is that evaluations should account for confident errors and appropriate uncertainty, rather than treating every abstention as a failure.

How to diagnose and reduce hallucinations

  1. Save examples and classify the failure

Collect representative wrong, incomplete, or unsupported answers. Mark factual correctness separately from relevance, formatting, and tone. Look for the failure point: unclear instruction, absent or faulty evidence, or an answer that departs from evidence provided.

  2. Make the instruction explicit where the task is underspecified

State the task, relevant context, boundaries, and expected output. Specify what the system should do if the answer cannot be found—for example, say that the source does not provide it or ask for clarification. The UK Government Digital Service’s AI Insights guidance, updated August 3, 2026, likewise emphasizes constraints and instructions for cases where an answer is unavailable. Treat its explanation of model behavior as an accessible overview, not a complete technical theory.

  3. Ground source-dependent answers and inspect retrieval

When an answer depends on current or domain-specific facts, retrieve reliable material and instruct the model to use it. Check whether the retrieved passages actually answer the question, reduce irrelevant context, and verify that the generated answer reflects the evidence. If facts are missing or stale, improve the source material as well as the retrieval process.

  4. Allow uncertainty or clarification

Tell the system that it may say when the available source does not answer the question or ask for clarification when needed. In evaluation, distinguish a justified abstention from an avoidable omission; do not assume every question must receive a definitive answer.

  5. Retest the whole system after meaningful changes

Use a representative set of cases and track correct answers, unsupported claims, omissions, and suitable abstentions. For RAG, evaluate retrieval and answer generation separately. One impressive example does not establish that a prompt or system change improved reliability.

  6. Consider fine-tuning only for a diagnosed behavior problem

If the model repeatedly fails to follow a consistent behavior despite good instructions and relevant context, fine-tuning may be worth investigating. Use representative examples and keep a hold-out evaluation set to check whether the change generalizes rather than overfits. Prompting, retrieval, and fine-tuning can be combined when different parts of the system need different fixes.

Choose the fix that matches the failure

Observed problem First intervention to test What to verify
Instructions are ambiguous or omit constraints Clarify the prompt and define what to do when information is absent. Whether the revised instructions address the same failure across representative cases.
Answer needs current or specialized facts Improve source material and retrieval; provide relevant evidence in context. Whether retrieval returned the right evidence and the model used it correctly.
Correct evidence is present, but the model repeatedly behaves inconsistently Evaluate the behavior and consider fine-tuning if the pattern is repeatable. Whether performance holds on a separate set of examples.
The system guesses when it should not Permit abstention or clarification and evaluate those outcomes appropriately. Whether uncertainty is justified by the available evidence, not merely frequent.

These options have no universal cost or performance ranking in the cited guidance. Choose based on what failed, whether source material is needed, how often the failure recurs, whether abstention is acceptable, and whether training or added system complexity is justified.

What RAG can—and cannot—establish

RAG can give a model relevant domain-specific context, but it cannot guarantee a factual answer. A June 2024 ServiceNow AI Research summary of a NAACL paper describes a particular RAG system for generating structured workflows from natural-language requirements. Its authors report reduced hallucinations and improved generalization to out-of-domain settings in that system. That result is specific to the described task; it is not evidence that RAG prevents hallucinations in every application.
