Short answer: An AI chatbot generates likely sequences of words, not verified facts. That statistical process can produce fluent and useful responses, but it can also create confident errors, invented explanations or citations, dangerous recommendations, privacy problems and biased or emotionally harmful interactions. Safeguards lower these risks; they cannot guarantee that every answer is safe or correct.

What a chatbot is actually doing when it answers

A generative-AI chatbot does not retrieve a checked answer from a universal database each time you ask a question. A language model approximates patterns in its training data and predicts what token (a word or word fragment) is likely to come next. It repeats that prediction many times to produce a response that reads like a coherent explanation.

This design explains both the usefulness and the uncertainty. Patterns learned from large amounts of text can help a model summarize, translate, draft code or explain a familiar concept. But the model’s objective is to produce a plausible continuation, not to establish that every statement is true, current, complete or appropriate for your circumstances. A polished paragraph is therefore evidence of fluent generation, not proof of accuracy.

Why open-ended prompts are especially difficult

Long-form questions, ambiguous instructions and prompts requiring specialized context give the model more opportunities to connect the wrong patterns. If several answers would sound reasonable, the system may select one without an underlying fact-checking step. Small changes in wording, conversation history or requested format can also change which continuation appears most likely. Two answers can consequently differ, or even contradict one another, while both sound confident.

Training data can contain errors, disagreements and outdated material. A model can reproduce those patterns or combine fragments that were never intended to fit together. It may also lack the private facts, local rules or professional context needed to answer a particular case. These are properties of probabilistic generation, not signs that the system is deliberately lying.

“Confabulation”: why AI gives confident but wrong answers

The National Institute of Standards and Technology (NIST) uses the term confabulation for “a phenomenon in which GAI systems generate and confidently present erroneous or false content in response to prompts.” “Hallucination” and “fabrication” are common informal descriptions of the same behavior.

What a confabulated answer can look like

  • A made-up fact stated with an exact date, number or quotation.
  • A plausible explanation for an event that did not happen.
  • A nonexistent paper, court case, product feature or web page.
  • A citation whose title and author look credible but whose source does not support the claim.
  • A response that gives two incompatible answers in different parts of the same conversation.

Generated reasoning can make the error harder to spot. The system may supply a chain of logic, calculations or references that appears to justify its conclusion. That supporting material is itself generated text. A citation displayed by a chatbot has not necessarily been checked against the underlying source, so you must open the source and confirm that it exists and says what the response claims.

Why confidence is not a reliability signal

Human readers often use clear prose, detail and a decisive tone as shortcuts for credibility. Chatbots can produce all three without possessing a corresponding level of certainty. NIST highlights the danger when people rely on such output for consequential decisions: a persuasive mistake can travel farther than an obviously incomplete answer.

There is no single consumer-chatbot percentage that describes how often these failures occur. Rates vary with the model, prompt, subject, access to external tools, safety settings and evaluation method. NIST’s 2024 profile is a risk-management document, not a randomized study of consumer outcomes.

How an answer can become harmful

Factual error is only one part of the risk. NIST’s Generative AI Profile describes several ways an output or interaction can damage people, organizations or information systems.

Risk area What can happen Why a user may miss it
Dangerous, violent or hateful content The system may generate threatening, inciting or radicalizing material, glorify violence, or provide dangerous, criminal or illegal recommendations. The response may be framed as an explanation, role-play or hypothetical scenario rather than an obvious instruction.
Self-harm and crisis advice A distressed person may receive an unsafe or unhelpful suggestion instead of appropriate human support. A conversational tone can feel supportive even when the content is unsuitable for a crisis.
Privacy leakage and inference Training-related leakage or generated combinations of information can expose sensitive details or infer sensitive attributes, even when an inference is wrong. An answer may look like a harmless summary while revealing more than the user intended.
Bias and homogenization Outputs can reproduce harmful stereotypes, treat groups unevenly or narrow the range of viewpoints and language. Fluent wording can conceal whose perspective is missing or being treated as the default.
Information-integrity failures False or manipulated content can be produced at scale, making it harder to distinguish reliable material from fabricated material. Repeated, consistent-sounding text can create an impression of independent confirmation.
Human over-reliance Automation bias can lead people to accept an answer without adequate checking, particularly under time pressure. The system is always available and responds immediately, which can make verification feel unnecessary.
Emotional entanglement People may disclose mental-health concerns or form unhealthy dependence on a conversational system; an unhelpful response can worsen distress. The system’s language is personal and responsive, although it has no human understanding or duty of care.

Safety filters are not a complete barrier

Providers can restrict some requests, add monitoring, or tune a model to refuse harmful instructions. Those controls reduce exposure to known patterns, but NIST notes that systems may still produce harmful recommendations when a request is novel, indirect or less explicit. Deliberate attempts to bypass controls are commonly called jailbreaking. A refusal in one wording does not establish that every equivalent wording will be refused.

Privacy: what can leak and what you should not assume

Privacy risk exists both in the data used to build a model and in what a system generates. NIST discusses possible leakage or generation of sensitive information from training data, as well as sensitive inferences assembled from otherwise separate sources. An inference can harm someone even when it is inaccurate, because people may act on it or repeat it.

Do not assume that every chatbot retains every conversation or uses every conversation for training. Data handling is service-specific. Before entering personal, confidential or regulated information, identify the service’s retention, review, training-use, deletion and access policies. If you cannot establish those terms, remove names, account numbers, precise locations, medical details, credentials and other identifying material, or do not submit the information.

How to judge whether an AI answer is trustworthy

Trust should be assigned to the claim, not to the chatbot’s tone. Use the following process whenever an answer could affect health, safety, money, rights, employment, education, security or another person’s privacy.

  1. Classify the consequence. If a wrong answer could cause material harm, treat the chatbot as a drafting or brainstorming aid, not as the final authority.
  2. Separate claims from wording. Copy out the dates, numbers, diagnoses, instructions, quotations and conclusions that would need to be true for the answer to be useful.
  3. Check primary or authoritative sources. Confirm laws with the relevant government source, medical information with a qualified clinician or recognized medical authority, financial rules with the responsible institution, and technical behavior with official documentation.
  4. Open every citation. Verify that the source exists, is the correct edition or date, and actually supports the specific statement. A plausible reference generated by the model is not verification.
  5. Look for missing conditions. Ask which country, date, software version, contract, diagnosis or user characteristic would change the answer. An answer that omits those conditions may be unusable even if its general description is correct.
  6. Seek an independent route. Compare the claim with a separate authoritative source or ask a qualified person. Getting the same generated answer twice is not independent confirmation.
  7. Test calculations and instructions safely. Recalculate important figures and use a non-production environment for code or configuration. Do not execute commands, change security settings or follow physical-safety instructions solely because a chatbot presents them confidently.
  8. Record uncertainty. If the evidence is incomplete or sources disagree, preserve that uncertainty rather than turning the chatbot’s best guess into a definite statement.

Warning signs that require a pause

  • Precise citations, quotations or statistics that you cannot locate.
  • Absolute language for a question with obvious exceptions.
  • Advice that changes when you rephrase the prompt.
  • Instructions involving medication, weapons, self-harm, illegal activity, credentials or irreversible system changes.
  • Requests for more personal information than the task appears to require.
  • A response that mirrors your emotional position and encourages you to withdraw from trusted people or professional help.

What organizations can do about the risks

NIST’s AI Risk Management Framework and its Generative AI Profile treat safeguards as a lifecycle responsibility. The framework is voluntary and asks organizations to consider trustworthiness during design, development, deployment, use and testing or evaluation. Its trustworthiness dimensions include validity and reliability, safety, security and resilience, accountability and transparency, explainability, privacy and fairness.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The 2024 Generative AI Profile identifies 12 generative-AI risk areas and just over 200 suggested management actions. Those figures describe the scope of the guidance, not a count of chatbot incidents or proof that every action works in every setting. NIST also emphasizes that no single trustworthiness characteristic guarantees that a system is trustworthy as a whole.

Safeguard question What a responsible program should specify
Which risk is addressed? Accuracy, harmful content, privacy, security, bias, information integrity, over-reliance or another defined risk.
Where does it operate? Model design, data preparation, evaluation, deployment controls, user interface, monitoring or incident response.
Who is accountable? The developer, deploying organization, administrator, trained reviewer or another named role.
What evidence supports it? Documented tests, representative scenarios, failure analysis, monitoring results and limits of the evaluation.
What happens after failure? A way to report the output, investigate it, correct or withdraw the system, and notify affected users when appropriate.

These questions prevent a common mistake: treating a safety label or a refusal demonstration as proof that the entire system is reliable. Controls must be evaluated against the uses, languages, populations and failure modes that matter in the real deployment.

Using chatbots safely in everyday work

Low-consequence uses

Brainstorming, rewriting a draft, generating alternative headings or explaining a familiar concept can be reasonable uses when you inspect the result and do not treat it as an authoritative source. Remove confidential material and keep a human responsible for the final text.

High-consequence uses

For medical, legal, financial, employment, educational-access, security or physical-safety decisions, use the chatbot only to prepare questions, organize information or identify topics for professional review. A qualified human and authoritative records must make or validate the decision.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

When someone is in distress

A chatbot should not be the sole support for a crisis, self-harm concern or urgent danger. Contact local emergency services, a crisis line or a trusted person who can provide immediate human help. The conversational style of a system does not give it clinical judgment, confidentiality or a duty of care.

What remains uncertain

Chatbots do not share one architecture, training set, privacy policy, safety configuration or level of reliability. Their behavior also changes with updates and with the surrounding application. Consequently, a safeguard or test result for one system cannot automatically be generalized to all others.

NIST’s framework is intended to evolve; its FAQ describes the AI RMF 1.0 as “a living document.” That continuing guidance is useful for managing risk, but it is not a guarantee that any individual answer will be correct or safe. The practical standard remains proportional caution: the greater the possible harm, the stronger the verification and human oversight required.
