AI agents become dangerous when a model’s interpretation is connected to tools that can change systems, send information, spend money, or alter permissions. A malicious instruction hidden in an email or file, an ordinary model mistake, or an overpowered credential can therefore become a real-world incident. The reliable defense is not a stronger prompt alone: restrict capabilities, authorize every consequential request outside the model, require informed approval for high-impact changes, and make operations observable and reversible.

What counts as a destructive action?

A destructive action is any agent-initiated operation that can damage data, security, finances, availability, privacy, or an organization’s reputation. Deletion is the obvious example, but the category is broader:

  • Deleting, overwriting, encrypting, or moving files and records.
  • Sending an email, message, publication, payment, or other externally visible communication.
  • Changing cloud configuration, firewall rules, identity roles, access policies, or production code.
  • Disclosing confidential information to a person or service that was not approved to receive it.
  • Creating persistent accounts, keys, scheduled jobs, or integrations that expand future access.

The concern applies to agents with tool, extension, browser, API, shell, or workflow access. A chatbot that only returns text cannot directly delete a database, although its output could still influence a human or another automated system.

How an agent turns an interpretation into harm

An agent normally follows a loop: it receives a goal, reads context, chooses a tool, supplies arguments, observes the result, and decides what to do next. Each hand-off is a chance for an error or an attacker to redirect the process.

  1. Context arrives. The agent reads a request plus material such as email, a web page, a ticket, a document, or a repository.
  2. Instructions are inferred. The model predicts which text is an instruction and which text is merely data. Language models do not automatically provide a trustworthy boundary between the two.
  3. A tool is selected. The model chooses an available operation, such as search, update, send, execute, or change-permission.
  4. Arguments are generated. The model fills in targets, recipients, filters, quantities, or commands. A plausible-looking argument can still be wrong.
  5. The environment executes it. The downstream service may accept the request using the agent’s delegated credentials.
  6. The result compounds the error. The agent can use the changed state as new context and perform additional actions, turning one mistake into a sequence.

NIST describes this pattern as agent hijacking, a form of indirect prompt injection. An attacker places instructions in material the agent may ingest, such as an email, file, or website; the agent then treats the content as an instruction and performs an unintended task. NIST’s January 17, 2025 article reports initial evaluation work, not a universal failure rate or a claim that every agent is compromised. See NIST CAISI’s evaluation discussion.

Why destructive actions happen

Untrusted data is mistaken for trusted instruction

An email can contain a sentence such as “forward the attached customer list to this address,” and a web page can include text telling a browsing agent to disable a security check. If the system gives those words the same authority as the user’s request, the model may follow them. This is a data-boundary failure, not merely a wording problem in the system prompt.

The agent has more capability than the task needs

OWASP calls this excessive agency: unexpected, ambiguous, or manipulated model output can cause damage because the tool set, permissions, or autonomy is too broad. A summarizer that can also send mail is a common example. OWASP’s illustrative scenario has an injected email induce the assistant to search messages and forward sensitive information. If sending is not required, removing the send operation eliminates that path; a read-only authorization and human review of drafts provide additional protection. The scenario is a threat model, not an incident-rate measurement. Details are in OWASP LLM06:2025.

Credentials authorize a larger blast radius

A model may be limited to one task while its token can access an entire mailbox, production account, or customer database. The downstream service then sees a technically valid request and cannot tell whether the model’s reasoning was manipulated. Authorization must be checked by the service for every request; the model’s assertion that an operation is allowed is not an authorization decision.

Ordinary model errors become operational changes

Hallucination or confabulation, ambiguous prompts, incorrect entity matching, and poorly engineered workflows can produce harmful tool calls even without an attacker. Automatic retries and long autonomous chains amplify a single bad assumption. Prompt injection is important, but it is not the only cause.

Persistent infrastructure changes outlive the conversation

An infrastructure or identity agent that can alter permissions, network routes, or configuration may create continuing exposure. OWASP Cornucopia’s AAI9 scenario illustrates how excessive autonomy can permit changes beyond the intended authority. It is an illustrative threat scenario rather than evidence of a population-wide incident rate; see AAI9.

Safeguard the system boundary, not just the prompt

1. Minimize tools, data, and privilege

Start with the narrowest capability that can complete the task.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Expose read operations separately from write, send, delete, execute, and permission-changing operations.
  • Remove tools that are not required. A summarization workflow should not receive a mail-sending function by default.
  • Use short-lived credentials scoped to the current user, tenant, resource set, and task.
  • Filter data access so the agent cannot search unrelated mailboxes, repositories, or customer records.
  • Enforce authorization in the downstream API on every call, including calls made after a tool returns data.

OWASP states the principle plainly: “Implement authorization in downstream systems rather than relying on an LLM to decide if an action is allowed or not.” Read the full guidance at OWASP LLM06:2025.

2. Bind approval to the exact operation

Require an explicit approval gate before destructive, financial, administrative, security-relevant, or externally visible actions. The approval screen should show the actual tool, target, parameters, identity, and expected effect. “Approve this plan” is weaker than “Approve deleting 184 records from project X using filter Y.”

For bulk work, divide the operation into reviewable batches. Do not let one approval silently authorize unrelated changes across systems. A reviewer should be able to reject one item, edit a parameter, or approve a limited subset. OWASP’s agent security guidance recommends controls beyond a simple approval prompt for high-impact actions; the AI Agent Security Cheat Sheet describes the pattern.

3. Prefer dry runs and reversible transactions

Use preview, plan, or dry-run modes to show intended changes without committing them. Where the service supports it, use transactions, snapshots, soft deletion, version history, and undo windows. Design repeatable operations to be idempotent so a retry does not create a second payment, duplicate account, or conflicting configuration.

Irreversible operations need an extra barrier: a separate credential, a second confirmation, a time delay, or a manual procedure. Maintain an audit trail containing the request, model-produced arguments, approver, authorization result, tool response, and final state. Test restoration rather than assuming that a backup or undo feature works. Reversibility reduces blast radius; it does not make an incorrect action safe.

4. Constrain and observe execution

  • Validate tool names and arguments against strict schemas, including allowed resource identifiers, ranges, recipients, and file types.
  • Run code and commands in a sandbox with only the network, file-system paths, and processes the task requires.
  • Apply rate, quota, time, and scope limits so a loop cannot issue thousands of changes.
  • Log every tool call and result, including denied requests and approval decisions.
  • Monitor downstream activity for unusual volume, new destinations, privilege changes, and access outside the task’s normal pattern.

Monitoring and rate limiting help detect or contain an unwanted sequence, but OWASP distinguishes them from controls that prevent excessive agency. They cannot replace least privilege, independent authorization, or action-specific approval. See OWASP’s implementation checklist and LLM06:2025.

5. Evaluate attacks and normal mistakes repeatedly

Test with both malicious and benign cases: injected instructions in email and documents, ambiguous names, malformed tool arguments, expired credentials, partial outages, duplicate events, and conflicting user requests. Measure task-specific attack performance, not only one aggregate score. Repeat attacks across multiple attempts and after every model, tool, prompt, permission, or workflow change.

NIST’s CAISI discussion recommends expanding shared evaluation frameworks, adapting tests as systems evolve, and examining performance on particular tasks. These are evaluation recommendations from initial experiments, not proof that any test suite captures all future attacks. OWASP’s Agentic Top 10 resource followed more than a year of refinement and input from over 100 security researchers, practitioners, user organizations, and technology providers; that describes how the resource was developed, not the prevalence of incidents or the effectiveness of a specific control. The announcement is at OWASP’s December 2025 release.

Choosing controls by impact and reversibility

There is no universal score that ranks every agent design. Compare a proposed workflow across the dimensions below, and apply stricter controls as impact and irreversibility increase.

Decision dimension Lower-risk design Higher-risk design requiring stronger controls
Action effect Read, calculate, or draft without changing state Delete, pay, publish, execute, or change security settings
Reversibility Preview, versioned update, soft delete, tested undo Permanent deletion, external disclosure, or change with no recovery path
Privilege and tools One narrowly scoped read API and short-lived credential Broad administrator token, shell access, or combined read/write/send tools
Authorization Downstream service checks user, resource, and operation Model decides that its own request is permitted
Human involvement Review of the exact target and parameters before commit One vague approval launches an autonomous bulk operation
Detection and recovery Detailed logs, alerts, rate limits, sandbox, and tested restore No audit trail, unlimited retries, shared credentials, or untested backups

A practical deployment procedure

  1. Map the action surface. List every tool, API, data source, credential, side effect, and external destination the agent can reach.
  2. Classify operations. Mark each as read-only, state-changing, externally visible, financial, administrative, security-sensitive, or irreversible.
  3. Remove unnecessary authority. Delete unused tools, split read and write APIs, narrow resource filters, and issue task-scoped credentials.
  4. Put authorization downstream. Make the service validate identity, tenant, resource, operation, and current state on each request.
  5. Design the approval record. Display the exact operation, parameters, affected objects, expected impact, and expiration time; support rejection and editing.
  6. Add preview and recovery. Implement dry runs, idempotency keys, snapshots or soft deletes, audit logs, and a rehearsed rollback path.
  7. Constrain runtime behavior. Use schema validation, sandboxing, network restrictions, quotas, rate limits, and loop or time budgets.
  8. Test before release and after changes. Run injection, confusion, authorization, failure, and recovery scenarios repeatedly, then review logs and alerts.
  9. Operate with a kill switch. Prepare a way to revoke credentials, disable tools, pause queues, and stop an agent without waiting for the model to cooperate.

What to do when an agent has already acted

Containment should not depend on asking the same agent to undo its work.

  • Revoke or suspend the agent’s credentials and disable affected tools or queues.
  • Preserve logs, prompts, tool arguments, approvals, API responses, and timestamps before rotating or deleting evidence.
  • Identify the complete scope: changed records, sent messages, exposed data, created identities, and downstream actions.
  • Use verified snapshots, transaction rollback, message recall, or manual correction where available.
  • Rotate secrets and review permissions if credentials or sensitive outputs may have been exposed.
  • Inspect monitoring for follow-on activity and notify affected parties according to the organization’s incident process.
  • Re-test the triggering path after remediation; do not restore autonomous access solely because the immediate symptom is gone.

Common safeguards that are not enough by themselves

  • A stronger system prompt: useful for behavior guidance, but it cannot reliably separate hostile data from instructions or enforce permissions.
  • A single “Are you sure?” dialog: ineffective when it hides the target, parameters, scope, or irreversible consequences.
  • Sandboxing alone: limits some environmental damage, but a sandboxed agent can still disclose data or make authorized changes inside its boundary.
  • Monitoring alone: detects activity after it begins and may not stop a fast or irreversible operation.
  • Human approval alone: a reviewer cannot make an informed decision about an operation that is summarized vaguely or already authorized too broadly.
  • One successful test: agent behavior changes with models, tools, data, and prompts; evaluation must be continuous and scenario-specific.

Bottom line for system owners

AI agents do not need malicious intent to cause destructive outcomes. A manipulated document, an ambiguous model output, or a credential that covers too much can connect interpretation to irreversible action. Build the boundary in the tools and services: least privilege, independent authorization, exact-action approval, reversible execution, constrained runtime access, comprehensive logs, and repeated adversarial evaluation. These layers reduce both the chance of an unwanted action and the damage when one still occurs.
