AI agents make mistakes with company data for a predictable reason: their answers are limited by the information they can retrieve, the permissions and freshness attached to it, and the controls governing what they may do next. An agent can produce a fluent answer from an incomplete policy, a stale customer record, or a maliciously edited page just as easily as from an authoritative source.
The practical fix is not to place the entire company database in a giant context window. Use targeted retrieval with access checks, provenance, freshness controls, and a small, explicit context; treat every retrieved passage as untrusted data; test retrieval separately from generation; and keep consequential actions behind deterministic limits and human approval.
What actually causes an agent to get company data wrong?
An agent is a synthesis system. It does not repair missing governance merely because the model is capable. Microsoft states the principle plainly in Data architecture for AI agents across your organization: “Because agents synthesize information rather than create it, their accuracy depends entirely on the quality and accessibility of underlying sources.” The failure can therefore be an ordinary quality problem, a security problem, or both.
Incomplete, fragmented, or stale source records
Policies may be split between a document repository, a ticketing system, a wiki, and individual spreadsheets. One system may contain the current approval limit while another still carries last year’s value. If the authoritative record is absent from the index, has not been refreshed, or is difficult to retrieve, the model can only combine what it sees. A polished answer can conceal that a required field was never available.

Ownership matters as much as content. Without a named owner, a freshness expectation, retention rules, and a process for retiring superseded documents, nobody is accountable for correcting a fact after a policy or product changes.
Retrieval returns the wrong evidence
A search layer can miss the system-of-record document, rank a plausible but unofficial page above it, or apply an access rule incorrectly. Similar terminology makes this especially difficult: a procedure for internal employees may resemble a customer-facing procedure but have different exceptions. Metadata filters, relevance controls, permission-aware indexing, and source citations reduce those errors, but they have to be designed and verified rather than assumed.
Grounding content can be poisoned
Retrieved material is not automatically trustworthy. An internal wiki page can be maliciously edited, a public page can be compromised, an embedding or vector index can be manipulated, and a document can contain instructions intended to redirect an agent. The resulting answer may be coherent because the model is faithfully synthesizing the poisoned passage.
This is a security failure, not merely a low-quality search result. The ingestion path, index, documents, and retrieval responses all need controls that are separate from the model’s ability to generate text.
Instructions and data get mixed together
Prompts, retrieved documents, memories, and tool responses can all contain imperative language. If the agent treats a sentence inside a retrieved document as higher-priority guidance than its system policy, an attacker can use prompt injection to influence the plan or tool calls. Retrieved text should be handled as untrusted input unless a separate, authorized control explicitly elevates it.
Too much authority magnifies a wrong answer
A mistaken explanation in a read-only assistant is inconvenient. The same mistake is dangerous when the agent can alter a record, send an external message, approve a payment, change access, or delete data. The blast radius is determined by the tools and operations available to the agent, not just by answer quality.

Unmanaged dependencies hide changes
Agents depend on models, indexes, connectors, APIs, documents, memories, and policies. If those dependencies have no owner or change record, a silent connector update or document edit can change behavior without an obvious deployment. Accountability becomes difficult because nobody can reconstruct what the agent saw or why it acted.

How to ground an agent without overflowing the context window
Grounding normally means selecting a small set of relevant, authorized material at request time instead of copying an entire corpus into the prompt. A typical flow is:
- Ingest: collect approved documents and records, attach ownership and security metadata, scan content, and create searchable representations.
- Retrieve: use the user’s request, task metadata, and permission context to find candidate passages. Apply document-level authorization or security trimming before the passages reach the model.
- Rerank and filter: prefer authoritative, current, task-relevant sources; remove duplicates and passages that fail policy or schema checks.
- Assemble context: pass only the excerpts, structured fields, and instructions needed for this task. Include source identifiers and dates so the answer can be traced.
- Generate and verify: require the model to stay within the supplied evidence, identify unsupported uncertainty, and run deterministic checks before any sensitive use.
This approach keeps the working context focused, but it does not make retrieval a trusted channel. Every retrieved page, memory, and tool result remains a trust-boundary crossing. Compact context is useful only when the selection process is authorized, current, and observable.
Use structured fields for decisions
For approvals, account status, limits, dates, and identifiers, retrieve typed fields from a system of record rather than relying on prose alone. Validate extracted values against a schema, reject missing or contradictory fields, and require the agent to ask for clarification when a required value is absent. Narrative passages can explain a rule; they should not silently replace the transaction record that enforces it.
Preserve a source trail
Attach a document or record identifier, owner, effective date, and retrieval timestamp to each context item. Showing citations to the user is useful, but the internal trace must also record which passages were actually supplied to the model. A citation to a page that was not in the model’s context does not prove what caused the answer.

1. Inventory data domains and name owners
Start with the tasks the agent must perform, not with a generic data lake. For each task, document:

- the data domain required, such as contracts, product specifications, support cases, or employee procedures;
- the system of record and any approved secondary sources;
- the business owner responsible for correctness;
- the expected update or freshness interval;
- retention and deletion requirements;
- which identities, roles, tenants, or projects may access it; and
- the operations the agent is allowed to perform using it.
Mark superseded documents and remove ambiguous duplicates from normal retrieval. Microsoft’s architecture guidance recommends documenting retrieval decisions and data domains; doing so creates an explicit basis for later tests and access reviews.
2. Enforce authorization during retrieval
Do not retrieve broadly and hope the model will redact what the caller should not see. Enforce document-level authorization or security trimming in the index and at query time. Use a managed identity or an equivalent service identity, pass the caller’s authorization context, and apply tenant, role, classification, and purpose metadata filters before context assembly.
Test both positive and negative cases: an authorized user should receive the permitted passage, while an otherwise similar unauthorized user should receive no passage or a safe denial. Check authorization again when a memory is written or reused; access granted at the time of one conversation may not remain valid later.
Recommended Free Tools
Permission-aware retrieval is available in some managed search services, including Azure AI Search, but the exact controls and integration must be checked against your identity, compliance, and auditing requirements. Microsoft’s recommendation to use built-in retrieval when those controls meet the requirement is vendor guidance, not an independent benchmark.
3. Keep instructions separate from data
Define a priority order in the agent policy: system and application controls outrank user content, retrieved documents, memories, and tool output. Delimit retrieved text and label it as evidence rather than instructions. Strip or quarantine content that attempts to change the agent’s rules, reveal secrets, bypass authorization, or call a tool.
For extracted values, use an explicit schema and reject fields that fail type, range, relationship, or signature checks. For example, a purchase request should not become an approved purchase merely because a retrieved paragraph says “approve immediately.” The approval state must come from an authorized workflow field and a policy check.

4. Protect ingestion, embeddings, and indexes
Limit who can write, modify, or delete source documents and vector content. Separate ingestion credentials from query credentials, require review for high-impact source changes, and preserve an immutable change record. Scan files before embedding, detect suspicious instructions and unexpected encoding, and quarantine content that fails inspection.

Monitor which sources are being retrieved, how often rankings change, and whether a new document suddenly displaces an established authoritative source. A sharp, unexplained change in retrieval distribution is an investigation signal, not proof that the new source is better.
5. Test retrieval independently from the generated answer
Build a test set from representative tasks and adversarial cases. For every case, inspect at least four outcomes:
- Was the authoritative source retrieved?
- Was every retrieved item authorized for the caller?
- Was the content current for the task’s effective date?
- Did the final answer stay supported by those items?
Include near-duplicate documents, conflicting policy versions, missing fields, revoked permissions, prompt-injection text, poisoned metadata, and tool failures. Run the harness before and after a material change to the prompt, model, retrieval configuration, connector, index, or tool. A good answer on a happy-path sample does not demonstrate safe retrieval.
6. Constrain tools and require review for impact
Grant the minimum data, tools, and operations required for the task. Microsoft’s risk guidance states: “Apply least privilege and least action. Allow only the minimum tools, data, and operations required. Deny everything else by default.” Use separate read-only and write-capable identities where possible.

Provide a reliable pause or stop mechanism. A kill switch that depends on the same failing agent is not a sufficient control; operators need an independent way to suspend credentials, queues, or connectors.
7. Make behavior auditable and observable
Keep an event trail for the request, identity, retrieved source identifiers and versions, model and prompt policy versions, tool calls, approvals, outcomes, and errors. Protect logs from alteration and restrict who can read sensitive contents. Where privacy rules prevent storing full text, retain stable identifiers, hashes, classifications, and the minimum evidence needed to reconstruct the decision.
Monitor source changes, connector health, authorization denials, retrieval quality, citation coverage, blocked tool calls, approval overrides, and unusual action sequences. Alert on patterns such as repeated attempts to access denied documents or a sudden concentration of answers on one newly added source.
Choosing a retrieval architecture
Built-in retrieval, custom retrieval-augmented generation, and agentic retrieval can all be appropriate. Compare them against the controls your task requires rather than assuming that a more autonomous design is more capable.
| Evaluation axis | Built-in retrieval | Custom RAG | Agentic retrieval |
|---|---|---|---|
| Authoritative source coverage | Strong when the managed connectors cover the systems of record and their ownership model is understood. | Can cover specialized or proprietary stores, but the team owns connector and mapping correctness. | Can search several sources dynamically, increasing coverage while increasing the number of paths to govern. |
| Freshness | Depends on the service’s connector and refresh behavior; verify the interval for each source. | Controlled by your ingestion and re-indexing jobs. | May combine live tools with indexes; each path needs its own freshness check. |
| Permission enforcement | Use it when its identity, document-level controls, and audit features meet the requirement. | You must implement and test security trimming, identity propagation, and memory access. | Every search and tool hop needs authorization; a missed hop can expose data. |
| Provenance and citations | Often available through managed source identifiers; confirm what is logged and shown. | Fully configurable, but provenance can be lost if custom transforms omit identifiers. | Must preserve provenance across each intermediate agent and tool result. |
| Relevance and answer quality | Managed ranking and metadata controls reduce maintenance, but may not fit unusual domains. | Allows domain-specific chunking, ranking, and schemas at the cost of engineering work. | Can plan multiple searches, but may amplify a misleading result through later steps. |
| Auditability and change monitoring | Use the provider’s logs and change controls, and verify retention and export needs. | You own end-to-end logs, alerts, and index change tracking. | Requires correlated traces across planning, retrieval, and action services. |
| Integration and maintenance burden | Usually lower when existing connectors and identity systems fit. | Higher engineering and operational responsibility. | Highest orchestration and testing burden because the agent chooses paths dynamically. |
| Read-only versus write access | Keep retrieval identities separate from write tools and add approvals for impact. | Implement separate credentials and deterministic gates yourself. | Constrain every tool and add approval checkpoints before irreversible operations. |
Microsoft recommends managed, built-in retrieval as a default when its accuracy, compliance, identity, and auditing controls satisfy the requirement; document the exception when they do not. That recommendation describes Microsoft’s platform guidance, not a universal independent comparison. NIST’s 2025 workshop summary offers a complementary lens—functionality, access patterns, risk, reliability, modality, and monitoring—and illustrates read-only, constrained-write, and write patterns. Its taxonomy work is described as ongoing, not as a final industry standard.
Failure scenarios and targeted fixes
| What you observe | Likely cause | First corrective action |
|---|---|---|
| The answer cites an old policy after a recent change. | Stale index, duplicate source, or missing effective-date filter. | Mark the system of record, refresh the index after changes, filter by effective date, and test conflicting versions. |
| A user receives details from another team or tenant. | Authorization was applied after generation or not applied to retrieval. | Move security trimming to query time, propagate the caller identity, and add unauthorized test cases. |
| The agent follows an instruction embedded in a document. | Retrieved data was treated as higher-priority policy. | Delimit and label evidence, inspect for instruction-like content, and enforce a fixed instruction hierarchy. |
| One newly edited page dominates answers. | Poisoned content, manipulated metadata, or ranking drift. | Review the change record and author, quarantine the page, compare retrieval distributions, and restore the last trusted index if needed. |
| A plausible answer triggers an unwanted external action. | Overbroad tool permissions or missing approval and parameter checks. | Disable the write credential, inspect the trace, separate read and write paths, and add deterministic gates and human approval. |
| Teams cannot explain why behavior changed. | Unmanaged dependencies or missing provenance and version logs. | Assign owners, version prompts and indexes, and record sources, tools, outcomes, and policy changes. |
A practical control checklist
- Source governance: every data domain has a system of record, owner, freshness expectation, retention rule, and access classification.
- Retrieval security: authorization and tenant filters run before context assembly; memory reads and writes are checked too.
- Trust boundaries: documents, memories, prompts supplied by users, and tool results are treated as untrusted input.
- Index hygiene: ingestion is restricted, files are scanned, changes are tracked, and ranking anomalies are monitored.
- Validation: retrieval tests cover authority, authorization, freshness, conflicts, injection, and missing data; answer tests verify support.
- Least action: read-only access is the default; write tools have narrow scopes, parameter validation, rate limits, and independent stop controls.
- Human review: sensitive, high-impact, external, or irreversible actions require approval with visible evidence.
- Auditability: logs correlate identity, sources, versions, model policy, tool calls, approvals, and outcomes.
What the evidence can—and cannot—tell you
The guidance behind these controls describes failure modes and design practices; it does not establish a representative enterprise error rate or a percentage of agents that fail. The approximately 140 experts reported for NIST’s January workshop indicate the scale of that discussion, not the reliability of deployed agents. Treat claims about a particular prevalence or benchmark as requiring separate, directly relevant measurement.
The central design decision is therefore not whether a model is generally intelligent. It is whether each answer and action can be traced to current, authorized evidence and constrained by controls that remain effective when the evidence is wrong or hostile.
