People who are pregnant, have pulmonary hypertension or Eisenmenger syndrome, or have known or suspected heart disease should not treat hot-water exposure as automatically safe. Pregnancy calls for a warm bath rather than a hot one; Cleveland Clinic recommends keeping bath water at or below 99°F (37°C). Mayo Clinic advises people with pulmonary hypertension or Eisenmenger syndrome to avoid hot tubs, saunas and long hot baths or showers. Cleveland Clinic recommends talking with a doctor before using a hot tub if you have known or suspected heart disease.

A home bath, hot tub, sauna and steam room create different exposures. No reviewed medical authority sets one temperature or time limit that is safe for every adult and every diagnosis. The right decision depends on the exposure, your condition, symptoms, medications and your clinician’s advice.

First, distinguish a bath from a hot tub, sauna and steam room

“Hot bath” can describe several very different situations. An ordinary bath is a tub of water at home. A hot tub keeps water continuously heated and is shared or recirculated, while a sauna heats the surrounding air and a steam room adds humid heat. These differences matter because heat can lower blood pressure, raise core temperature or stress the heart, and hot-tub water can carry infectious organisms.

Exposure Main concern discussed by medical guidance Who needs specific caution
Warm home bath Excessive heat can raise body temperature or cause dizziness and falls Pregnant people and anyone who becomes light-headed or overheated
Hot tub Heat-related blood-pressure drop, cardiac strain and contaminated water Pregnant people; people with pulmonary hypertension or Eisenmenger syndrome; people with known or suspected heart disease
Sauna or steam room High ambient heat can raise core temperature and lower blood pressure Pregnant people and people with pulmonary hypertension or Eisenmenger syndrome

A hot tub also maintains heat for the entire session, unlike a bath that gradually cools. Recirculation and shared surfaces add a sanitation issue that a private, clean home tub does not have. A sauna or steam room may feel different because the heat is in the air, but the body can still warm and blood pressure can still change. Because the risks are not identical, advice about avoiding a hot tub should not automatically be rewritten as a ban on every bath. Conversely, a normal-feeling bath is not proof that a hot tub or sauna is appropriate.

Pregnancy: keep baths warm, and avoid high-heat exposures

Recommended bath temperature

Cleveland Clinic advises a warm rather than hot bath during pregnancy and gives a ceiling of 99°F (37°C). Alison Stalzer, DO, an ob/gyn at Cleveland Clinic, explains: “Your bath should be no higher than 99 degrees Fahrenheit (37 degrees Celsius), which will prevent a rise in your body temperature.” The recommendation appears in Cleveland Clinic’s May 2, 2025 guidance on hot-tub safety during pregnancy: https://health.clevelandclinic.org/hot-tub-while-pregnant.

A reliable bath thermometer can help check the water instead of relying on touch. Test the water before getting in and recheck it if you add hot water. The thermometer is only a measuring aid; it does not make a hotter bath safe or replace prenatal advice. If you are unsure how hot the water is, choose cooler water rather than trying to estimate by hand.

Why high heat is discouraged

Excessive heat during pregnancy can raise core temperature. Cleveland Clinic also warns that overheating may cause dizziness, increasing the chance of slipping or falling while getting into or out of the tub. Its pregnancy-bath guidance is available at https://health.clevelandclinic.org/baths-while-pregnant.

For sauna or hot-tub use early in pregnancy, the American College of Obstetricians and Gynecologists (ACOG) gives the short answer, “It’s best not to.” Its September 2021 patient guidance explains that these exposures can raise core temperature: https://www.acog.org/womens-health/experts-and-stories/ask-acog/can-i-use-a-sauna-or-hot-tub-early-in-pregnancy. This is specifically early-pregnancy guidance about sauna and hot-tub heat, not a claim that a comfortably warm household bath is forbidden throughout pregnancy.

CDC likewise says, “If you are pregnant, talk to a healthcare provider before using a hot tub.” That advice is specific to hot tubs and does not mean that every warm home bath is prohibited: https://www.cdc.gov/healthy-swimming/safety/what-you-can-do-to-stay-healthy-in-hot-tubs.html.

Practical pregnancy checklist

  • Set bath water at or below 99°F (37°C).
  • Ask your prenatal clinician before using a hot tub, sauna or steam room.
  • Use a nonslip mat or secure surface, keep necessities within reach and avoid climbing over a high tub wall if balance is difficult.
  • Stand up slowly, and have help nearby if you have felt faint or unsteady.
  • Leave the water if you feel dizzy, nauseated, weak or unusually hot.

Pulmonary hypertension: avoid hot tubs and long hot baths

Mayo Clinic advises people with pulmonary hypertension to avoid activities that substantially lower blood pressure, specifically including hot tubs, saunas and long hot baths or showers. Heat widens blood vessels; a sudden pressure drop can reduce blood flow to the brain and cause fainting or other complications. Read the condition-specific guidance at https://www.mayoclinic.org/diseases-conditions/pulmonary-hypertension/diagnosis-treatment/drc-20350702.

This is not a suggestion to experiment with a shorter or cooler hot-tub session on your own. A person may tolerate a shower but not prolonged immersion, or may react differently on a day with dehydration or illness. Ask the clinician managing your pulmonary hypertension what bathing and shower temperatures are appropriate for you, especially if you have previously fainted, become breathless or had chest discomfort in heat.

Eisenmenger syndrome: the same blood-pressure warning applies

Mayo Clinic gives a parallel warning for Eisenmenger syndrome: avoid hot tubs, saunas and long hot baths or showers because heat-related blood-pressure drops can lead to fainting or other complications. The diagnosis-and-treatment page is at https://www.mayoclinic.org/diseases-conditions/eisenmenger-syndrome/diagnosis-treatment/drc-20350584.

Eisenmenger syndrome involves complex heart and lung circulation, so a generic “safe soaking time” cannot be inferred from advice for healthy adults. Follow the individualized limits from your cardiology team and avoid bathing alone if your clinician has identified a fainting risk. Do not assume that lowering the thermostat slightly removes the risk; the appropriate limit depends on your circulation and treatment plan.

Known or suspected heart disease: ask before using a hot tub

Cleveland Clinic recommends a conversation with a doctor before hot-tub use when heart disease is known or suspected. Sudden or prolonged immersion in hot water can stress the cardiovascular system, and the combination of heat, blood-pressure changes and exertion getting in or out may be poorly tolerated by some people. The guidance, published January 24, 2020, is at https://health.clevelandclinic.org/hot-tubs-can-land-your-heart-in-hot-water.

This recommendation is deliberately narrower than “anyone with any heart diagnosis must never bathe.” It concerns hot tubs and the need for an individualized assessment. Before using one, tell your clinician about coronary disease, rhythm problems, valve disease, heart failure, unexplained chest pain, fainting or shortness of breath. Ask whether a warm home bath is acceptable, whether someone should be nearby, and which symptoms mean you should stop. Do not use a hot tub to test whether undiagnosed symptoms are harmless.

Hot-tub germs are a separate reason to be cautious

Even when heat is not the issue, contaminated hot-tub water can spread respiratory, skin and gastrointestinal infections. CDC’s hot-tub guidance describes exposure through inhaled mist, swallowing water or contact with contaminated water and surfaces: https://www.cdc.gov/healthy-swimming/safety/what-you-can-do-to-stay-healthy-in-hot-tubs.html.

  • Do not use a hot tub when you have diarrhea; germs can spread through the water.
  • Ask a healthcare provider before hot-tub use during pregnancy.
  • Choose a facility that maintains disinfectant and pH levels and provides inspection information.
  • Shower before entering and avoid swallowing the water.
  • Keep children and anyone who cannot follow these precautions under close supervision.

These sanitation precautions apply to hot tubs and do not turn an ordinary, properly maintained home bath into the same type of exposure. A clean-looking tub is not proof that water is free of organisms, so facility maintenance matters.

What about medicines or other medical conditions?

The reviewed guidance does not establish a universal list of medicines that make baths unsafe. CDC notes that some medicines can increase dehydration or overheating risk during hot weather, but that general heat advice is not a bath-specific medication list: https://www.cdc.gov/extreme-heat/prevention/.

Ask a pharmacist or clinician about your own medicines rather than stopping or changing them. Individual advice is especially prudent if you have a condition affecting circulation, heart or lung function, temperature regulation or fainting risk. Tell the clinician about diuretics, blood-pressure treatment or other prescriptions if you take them, but do not assume any one drug is unsafe without professional guidance. This is a sensible clinical precaution, not a blanket prohibition established for every such condition.

How to make a warm bath safer

  1. Confirm the exposure. Decide whether you are planning a home bath, a hot tub, sauna or steam room; do not assume advice for one applies to another.
  2. Check with a clinician when indicated. Pregnancy, pulmonary hypertension, Eisenmenger syndrome and known or suspected heart disease warrant the specific precautions above.
  3. Measure pregnancy bath water. If you are pregnant, keep it at or below 99°F (37°C), using a reliable thermometer.
  4. Prepare before entering. Have water, a phone and a towel within reach; keep the floor dry and use a nonslip surface.
  5. Enter and exit slowly. Secure the floor and avoid standing quickly if you are prone to light-headedness.
  6. Do not combine heat with illness. Postpone a hot-tub visit if you have diarrhea or feel acutely unwell, and ask your clinician if you are dehydrated.
  7. Stop at warning symptoms. Get out carefully if you feel dizzy, faint, nauseated, weak, short of breath or overheated.
  8. Do not bathe alone when advised against it. If your clinician has identified a fainting or seizure risk, arrange appropriate supervision.

When to seek medical help

If dizziness, faintness, nausea or overheating resolves after you leave the water, contact your clinician for advice before trying the exposure again. Seek urgent medical care for severe or persistent symptoms, loss of consciousness, chest pain, significant breathing difficulty, confusion or symptoms that do not improve after cooling and resting. Hot water may be one cause, but these symptoms can also signal a serious heart, lung or neurologic problem. Leave the water safely rather than standing abruptly; call for help if you cannot exit without falling.

Frequently Asked Questions

Can I take a bath if I’m pregnant?

A warm home bath may be acceptable. Cleveland Clinic recommends keeping the water at or below 99°F (37°C), checking with a reliable thermometer and leaving if you feel dizzy or overheated. Ask your prenatal clinician before any hot-tub, sauna or steam-room use.

Can I use a sauna or hot tub early in pregnancy?

ACOG says, “It’s best not to,” because these exposures can raise core temperature. CDC also advises pregnant people to talk with a healthcare provider before using a hot tub.

Are hot tubs safe if I have heart disease?

Not automatically. Cleveland Clinic recommends discussing hot-tub use with a doctor when heart disease is known or suspected. This is not a universal ban on every warm bath.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Why are hot tubs risky with pulmonary hypertension or Eisenmenger syndrome?

Mayo Clinic advises avoiding hot tubs, saunas and long hot baths or showers because heat can lower blood pressure and lead to fainting or other complications.

Is a home bath the same as a hot tub?

No. A hot tub is continuously heated, often shared and recirculated, so it combines heat and sanitation concerns. Advice for a hot tub should not automatically be applied to a warm private bath.

What should I do if I feel faint in the bath?

Stop the exposure and get out carefully, using help if needed. Contact your clinician before trying again; seek urgent care for loss of consciousness, chest pain, major breathing difficulty, confusion or severe or persistent symptoms.

The Bottom Line

Pregnancy, pulmonary hypertension, Eisenmenger syndrome and known or suspected heart disease all call for specific caution, but the recommended action depends on the exposure. Keep a pregnancy bath warm and at or below 99°F (37°C); avoid hot tubs, saunas and long hot baths when Mayo Clinic advises it for your cardiopulmonary diagnosis; and ask a doctor before using a hot tub with heart disease. Stop immediately for dizziness, faintness, nausea, weakness, breathlessness or overheating.
