Eligibility for CAR T-cell therapy depends on four things that must fit together: your exact cancer and its target, the indication for a specific CAR T product or clinical-trial protocol, your previous treatments and responses, and your current health and treatment circumstances. There is no universal checklist that can determine eligibility at home. An oncology team familiar with the product and treatment center must review your complete record.
CAR T-cell therapy is a personalized immunotherapy. T cells are collected from your blood, modified in a laboratory to recognize a target on cancer cells, expanded, and infused back into you. The following guide explains the main eligibility questions, what the approved-use examples do and do not establish, how manufacturing affects timing, and what to ask at an oncology appointment.
What eligibility means in practice
A CAR T decision is not based on a single diagnosis code or a general statement such as “blood cancer.” Each product has its own approved indication, and a clinical trial has its own protocol. The team must match your disease and treatment history to those requirements, then decide whether the treatment can be given safely and practically.

The diagnosis and target
The first filter is the cancer itself. CAR T products are designed to recognize a particular target on cancer cells, so the diagnosis, disease subtype, and any required target or biomarker testing matter. A product approved for one lymphoma subtype is not automatically appropriate for another, even when the names sound similar.

The product or trial protocol
Approval applies to a defined use, not to every person with a related cancer. The current U.S. Food and Drug Administration label may specify age, disease subtype, previous treatment lines, response or relapse status, biomarker findings, or other conditions. A clinical trial can have different inclusion and exclusion criteria. Ask the center to show you the current product label or trial protocol that applies to your case.
Your treatment history
Previous medicines, the number of treatment lines, whether the cancer responded, and whether it returned or failed to improve can determine whether an indication fits. The same diagnosis may qualify for one product after a particular sequence of therapies but not for another product or treatment setting.
Your health and circumstances
The team also evaluates baseline health, the extent and pace of the cancer, other treatments, and the safety and monitoring resources available at the center. The National Cancer Institute (NCI) notes that the likelihood and severity of side effects depend on factors including baseline health, cancer type and extent, treatment type, and dose. The sources do not establish one universal age, organ-function, disease-burden, or performance-status threshold for every CAR T product.
Which cancers have approved CAR T uses?
NCI’s overview updated February 26, 2025, lists six CAR T products and broad cancer categories. This table is an orientation, not a self-screening tool. Exact indications can change and often contain additional conditions.

What about solid tumors?
NCI’s general T-cell transfer therapy page says CAR T has been studied in cancers such as breast and brain cancer, but treatment for solid tumors remains experimental. If you have a solid tumor, ask whether a relevant clinical trial exists and request the protocol’s exact eligibility criteria. Trial participation is a separate possibility from an FDA-approved use.
Why previous treatment can determine eligibility

NCI states that obecabtagene autoleucel (Aucatzyl) is approved for adults whose B-cell ALL has returned or did not get better during or after other treatment. Your oncologist must still confirm the precise disease definition, prior-treatment record, and all other current label requirements.

Abecma and multiple myeloma
NCI states that idecabtagene vicleucel (Abecma) is approved for adults with relapsed or refractory multiple myeloma after at least two prior lines that included an immunomodulating agent, a proteasome inhibitor, and an anti-CD38 monoclonal antibody. “At least two lines” and the named drug classes are specific to this example; they should not be applied to another product without checking its label.
Bring a treatment-history record
To help the team make a fast, accurate determination, bring dates and names of every cancer treatment, the reason it was stopped, the best response, evidence of relapse or refractory disease, pathology reports, imaging, and records from other treatment centers. Ask whether any treatment you are currently receiving could affect collection, manufacturing, infusion, or trial eligibility.
Health and disease factors the team may assess
There is no single NCI-published checklist that applies to all products. Instead, ask which factors the specific center and product require the team to assess.
- Current disease status: How extensive or fast-growing is the cancer, and is control needed while cells are being prepared?
- Target confirmation: Does the cancer express the target required by the product, and is a new biopsy or laboratory test needed?
- Baseline health: Which medical conditions, infections, previous therapies, or current medicines could change risk?
- Ability to complete monitoring: Can you remain near the treatment center for the required period, attend follow-up visits, and reach urgent care quickly?
- Support arrangements: Does the center require a caregiver, transportation plan, or local accommodation, and for how long?
These questions are about safety and logistics as well as formal eligibility. Only the treating team can interpret test results in the context of the exact product.

How collection and manufacturing affect the decision
CAR T treatment is a multistep process rather than a same-day infusion. NCI describes collection of T cells, laboratory engineering to make them recognize a cancer target, growth of the modified cells, and infusion back into the patient.
- Collection: The center collects your T cells and sends them for manufacturing.
- Engineering: Laboratory staff modify the cells so they can recognize the required target.
- Expansion and quality checks: The modified cells are grown and prepared for release.
- Bridging or other disease-control treatment, if needed: Ask what treatment is possible while the product is being made and whether it changes eligibility.
- Infusion and monitoring: The finished cells are given at the authorized center, followed by close observation and scheduled follow-up.
One NCI overview estimates roughly three to five weeks from collection to infusion, while the broader NCI treatment page says cell growth can take two to eight weeks. These are general estimates from different NCI pages, not a guaranteed schedule. Ask your center for its actual collection-to-infusion interval, what could delay it, and what happens if the disease worsens while you wait.
Serious effects and the monitoring plan
NCI highlights two important categories of potentially serious complications. The team should explain the product-specific frequency, timing, grading, treatment, and location of monitoring.
Cytokine release syndrome (CRS)
CRS can include fever, low blood pressure, and breathing problems. Ask what symptoms require an immediate call, whether you should go directly to the treatment center or an emergency department, and who is available after hours. Clarify how long you must remain close to the center after infusion.

Immune effector cell-associated neurotoxicity syndrome (ICANS)
ICANS is a neurologic toxicity that can involve confusion, excessive sleepiness, or impaired speech. Ask whether a caregiver must observe you, what changes should be recorded, and how the team will evaluate and treat neurologic symptoms.
Make the emergency plan concrete
- Get a 24-hour telephone number and the name of the service to contact.
- Ask which symptoms mean “call now” and which mean “call emergency services.”
- Confirm the monitoring location, expected duration, and transportation rules.
- Ask what follow-up tests and visits are required after discharge.
- Write down medication instructions and restrictions in language your caregiver can follow.
Questions to ask your oncologist
Eligibility and testing
- Does my precise diagnosis and its target match an FDA-approved CAR T product?
- If not, is there a clinical trial for my cancer, and what are that protocol’s inclusion and exclusion criteria?
- Which prior treatments and responses does the indication require, and does my record meet them?
- Do I need target or biomarker testing, a new biopsy, imaging, or other laboratory tests?
- Which findings could make treatment unsafe or temporarily unavailable?
Benefits, uncertainty, and alternatives
- What benefit is realistic for someone with my diagnosis, treatment history, disease status, and health?
- What is known, and what remains uncertain, about the product in people most like me?
- Which standard treatments, other cellular therapies, medicines, radiation options, or trials should we compare?
- How do the alternatives differ in expected benefit, uncertainty, toxicity, monitoring, timing, and travel requirements?
Timing and practical requirements
- What is this center’s actual collection-to-infusion timeline?
- What treatment may be needed while my cells are manufactured?
- Where will collection, infusion, and follow-up occur?
- How long must I stay nearby, and is a caregiver required?
- What costs, travel, lodging, work, or family arrangements should I plan for?
Safety and follow-up
- How commonly does this product cause CRS or ICANS, and when do they usually appear?
- What fever, blood-pressure, breathing, confusion, sleepiness, or speech changes require urgent action?
- Who do I contact at night or on weekends?
- What follow-up visits, tests, and restrictions apply after infusion?
- Which current FDA label and patient-information materials should I read?
How to compare CAR T with a trial or another treatment
Ask the oncologist to compare options using the same five dimensions instead of focusing only on whether a treatment is called “approved.”

The cited NCI sources do not provide a patient-specific comparison of expected outcomes. Your oncologist needs to interpret the evidence in light of your pathology, treatment history, current scans, laboratory results, and overall health.
What published outcomes can—and cannot—tell you
NCI reports trial-level results that help explain why CAR T is being studied and used, but they are not personal predictions. In one trial involving people with advanced follicular lymphoma, NCI reported that axi-cel eliminated the cancer in nearly 80% of participants; a later update noted that many remained without recurrence at three years. In clinical trials involving people with large-cell lymphoma, NCI reported that more than 30% of participants were alive without evidence of cancer five years after treatment. Results can differ by product, cancer subtype, prior therapy, patient selection, follow-up time, and definition of response.
In the NCI article updated February 26, 2025, immunotherapy and CAR T-cell therapy pioneer Steven Rosenberg, M.D., Ph.D., is quoted as saying, “ [CAR T cells] have become a part of modern medicine.” That statement describes the field’s development; it does not determine whether a particular person qualifies or how that person will respond.
A practical appointment checklist
Before the consultation, prepare a one-page summary with your diagnosis and subtype, target-test results, every prior treatment and response, current medicines, allergies, major medical conditions, recent scans and laboratory results, and contact information for other treating centers. Bring a support person if possible and ask permission to take notes.
Leave the appointment with the product or trial name, the specific indication or protocol being considered, tests still needed, the expected manufacturing timeline, interim-treatment plan, monitoring location, emergency telephone number, caregiver and travel requirements, and the date of the next decision point. Eligibility can change as the disease or treatment history changes, so ask when the team will reassess if treatment cannot proceed immediately.
