For adults with nonspecific low-back pain or morning stiffness, a medium-firm mattress is the best-supported general starting point—not the firmest mattress you can find. The right fit still depends on sleep position, body shape, pressure sensitivity, and comfort. These five mattresses are a practical shortlist drawn from different 2026 product guides, not a medical ranking or a promise that any mattress will treat back pain.
What mattress firmness should you choose for back pain?
Start by considering a medium-firm feel. A 2026 review in the Journal of Orthopaedics and Traumatology examined 47 studies and found that medium-firm mattresses were most consistently favored over very firm or very soft alternatives. Its authors call medium-firm the most reasonable initial choice for adults with nonspecific low-back pain or morning stiffness. The review is a qualitative synthesis, not a guarantee for an individual sleeper.
A randomized, double-blind trial of 313 adults with chronic nonspecific low-back pain, published in 2003, found better pain and disability outcomes with medium-firm mattresses than with firm ones. That trial does not establish what will work for every cause of back pain or every body. See the 2026 review and the 2003 trial.
In practical terms, support means staying comfortably aligned while pressure is distributed—not simply sleeping on the hardest surface. A mattress that is too firm may press uncomfortably at the hips and shoulders; one that is too soft may let the body sink more than feels comfortable. Mattress materials alone do not establish that a bed will reduce pain: the review found no single material superior for all patients.

These picks bring together different use cases from three publishers’ guides. Their rankings and test methods are not directly comparable, and none of the recommendations establishes a clinical benefit.

How to choose among the five
Match the feel to your sleep position
- Side sleepers: Look for enough cushioning at the shoulders and hips to avoid uncomfortable pressure, while still feeling supported through the middle. Construction, cooling, motion isolation, and zoned support can matter to individual shoppers, but none is proof of pain relief. Compare features only in relation to your own needs—for example, whether you share a bed or tend to sleep hot—and treat performance claims as model-specific rather than universal.

What the evidence can—and cannot—tell you
The 2026 review searched studies published through December 31, 2023. Although it appeared in 2026, its evidence does not include studies published after that search cutoff. The authors describe generally low-to-moderate study quality, variation in mattress definitions and outcome measures, small samples or short follow-up in some studies, and no formal meta-analysis. Its conclusion supports a starting point, not a prescription.
“Orthopedic” is not a standardized label that guarantees biomechanical effectiveness or clinical benefit. Treat it as a marketing description unless a seller provides meaningful, specific evidence for the claim. Likewise, a mattress guide’s recommendation is an editorial assessment, not evidence that a bed treats a diagnosis.
