On Forest Island (Island 2) in How to Fish, the three leeches are pickups in the grass, not fish in the lake. Speak to the woman beside the lake to start her quest, collect all three on foot, and bring them back for bait that summons the Giant Piranha.
Where are the leeches in How to Fish?
Go to Forest Island, also called Island 2, and search the grassy area near the lake. The leeches are small ground pickups: walk through the grass and look for the interaction prompt. Do not cast into the water or search underwater for them.
Walk slowly and sweep the area rather than relying on a single exact spot. Walkthroughs describe the general location but do not establish fixed coordinates.

How to collect all three leeches
- Talk to the woman by the lake. Start the quest before searching. A walkthrough reports that doing so activates the quest tracker.
- Search the nearby grass on foot. Move through the grassy area and use the pickup prompt when it appears.
- Continue until you have three. Check the quest count, then return to the woman.
- Hand over the leeches. The reward is described as Modified Leech bait; item naming may vary, so follow the name shown in your current game.
What does the leech hand-in unlock?
The bait is used at the lake to summon the Giant Piranha. Defeat the boss, pick up its skeleton, and return the skeleton to the same quest giver. That later hand-in advances the island progression; turning in the leeches alone does not complete this quest stage.

How to handle the Giant Piranha fight
Use the quest bait at the lake to begin the encounter. A walkthrough recommends staying mobile and choosing weapons that let you deal with the boss and its smaller piranhas, but available guides disagree on the boss’s health. No single health figure is established, so use the encounter itself and your current equipment rather than relying on a quoted number.
