A decision made in a Claude Code conversation can guide the work without becoming a record your cofounder can find. Claude Code sessions, machine-local memory, and shared project documentation serve different purposes. The practical fix is to capture decisions that could change a teammate’s next action—along with the reason and source—in a team-accessible home.
Where Claude Code plans and decisions are stored
There is no single shared “plans” location that captures every decision made during a Claude Code session. A conversation is working context; Claude Code’s auto memory carries selected learnings between sessions on the same machine; and project instructions such as CLAUDE.md or AGENTS.md can be committed for a team. None of those, by itself, is necessarily a discoverable decision history.
Session conversation: useful working context
Anthropic says each Claude Code session starts with a fresh context window. That makes a session useful for the work underway, but it should not be treated as a permanent shared record. This does not mean a transcript was necessarily deleted or cannot be retrieved; it means the session itself is not the same thing as a team-maintained decision log. Anthropic’s memory documentation explains the session and memory distinction.

Auto memory: persistent for Claude, local to a machine
Claude Code can retain learnings through auto memory, but Anthropic states that “Auto memory is machine-local.” Its memory files are not shared across machines or cloud environments, although worktrees in the same repository share the memory directory. That can help one person resume work; it does not make the decision available to a collaborator using another environment.

Project instructions: shared context, not a decision log by default
A project-level CLAUDE.md or AGENTS.md can put conventions and architectural guidance in version control, where teammates can read and update them. Anthropic describes these files as context rather than enforced configuration. They are a reasonable home for durable guidance that should influence future work, but a collection of instructions may not preserve why a particular choice was made, what evidence informed it, or which later decision replaced it.
Why the human decision can disappear from team view
Claude Code conversations mix planning, implementation, and incidental discussion. Anthropic’s report, “How Claude Code is used in practice,” analyzed roughly 400,000 interactive sessions from roughly 235,000 people between October 2025 and April 2026. In the report’s classifier estimates, people made about 70% of planning decisions, while Claude made about 80% of execution decisions, averaged across sessions. Anthropic summarizes the pattern this way: “People decide what to build, and the agent decides how to build it.” Those are classifier results for the report’s analyzed interface and sample—not a universal measurement of all coding work.

- Versatile gift choice: these inspiring journal notebooks are a good choice for team gifts, office Christmas gifts, employee appreciation gifts, retirement gifts, or colleague holiday gifts, it is also suitable for Teacher's Day, graduation ceremonies, congratulations, and weddings
The distinction matters: an agent may help carry out a choice, but the reason for that choice may exist only in the conversation where a person made it. A cofounder who later needs to know why the team chose one API, migration path, or constraint needs more than the implementation result. No reviewed source quantifies how often Claude Code decisions are missed or the cost when that happens, so it is better to treat this as a workflow risk than as a measured rate.

Make a short decision-capture habit part of the session
At the end of a meaningful coding session, ask Claude to surface the decisions and their reasons. Then select only the decisions that could change a teammate’s next action; not every preference or temporary implementation detail merits a permanent entry. Put the selected items in the team’s established durable home and include enough context for someone who was not in the conversation.
- Ask for a decision list. Prompt Claude to identify choices made, the rationale discussed, alternatives rejected, and any supporting files, tests, or issue references. Treat its response as a draft to check, not an authoritative transcript.
- Choose what has future impact. Keep decisions that affect architecture, interfaces, data, rollout, operational behavior, or a teammate’s next task. Leave transient brainstorming in the session unless it changes a later choice.
- Record the choice and its reason. State what was selected, why, the important alternative or constraint, and the date or project context. Link to an issue, code change, test, or session reference where the team can access it.
- Use a location the intended readers can reach. Commit repository-wide engineering guidance to the project; use an ADR or issue when the decision needs a reasoned history and status; use an existing shared account or team knowledge system for cross-functional customer context.
- Update, do not silently overwrite. When circumstances change, make the new decision and its rationale visible, and mark the prior record as superseded or link the two entries.
This is a practical editorial recommendation, not a workflow benchmark. Tandem describes a similar human-selection pattern for moving Claude Code context into shared account memory: people choose what to retain rather than treating every conversation as automatically shared. See Tandem’s account of its workflow for the vendor’s description.

Choose the record by audience and decision lifetime
The best home is the one the people affected by the decision already use and can access. Before creating a new system, check four things: who can read it, whether the rationale is preserved, whether it points to the evidence or session, and how a later choice will supersede it without erasing history.
- Use
CLAUDE.mdorAGENTS.mdfor stable repo-level conventions and architectural guidance that should shape future coding sessions. Keep the file focused on usable context; add a link to a fuller rationale when the decision has a meaningful history. - Use an ADR or issue when engineers need to see the decision, its context, alternatives, consequences, and eventual status. This is usually a better fit than an instruction file for a choice likely to be revisited.
- Use shared account or team memory when the decision belongs to customer delivery or cross-functional work rather than one repository. Tandem’s product materials describe a shared account brain that can include Claude Code sessions and return source-linked context to a team; that is a vendor-described use case, not an independent comparison. See Tandem’s product overview.
Do not confuse availability with provenance: giving a teammate access to a summary is not enough if they cannot tell why it is trusted or what it supersedes. Likewise, preserving a transcript is not the same as maintaining a concise record that helps someone act.
Do not assume session saving means automatic team sync
Tandem’s August 2026 article describes its selection and sync workflow as human-triggered; it said checkpoint-triggered automatic resync had not shipped at publication. That is a time-sensitive statement about one vendor’s product, not a general Claude Code behavior. Check the current product documentation before depending on a particular sync trigger.
Claude Code’s own context-management advice addresses a different problem: Anthropic recommends clearing context between unrelated tasks and describes compaction as a way to retain a summary of important material, including decisions. Those practices help manage an agent’s working context, but neither makes a decision automatically discoverable to a teammate. See Anthropic’s compaction guidance.
