A cortisol test is not part of routine infertility or pregnancy screening. Ask a clinician about testing when your symptoms, examination, medicines, or an adrenal finding suggest either excess cortisol (Cushing syndrome) or too little cortisol (adrenal insufficiency). Pregnancy changes which tests are reliable, so an obstetric or reproductive clinician should coordinate with an endocrinologist when an adrenal disorder is suspected.
Trying to conceive, feeling stressed, or having a single abnormal home-kit result is not, by itself, an indication for a random cortisol test. The right test depends on the suspected disorder, your pregnancy status and trimester, medications, collection timing, and whether confirmation is needed.
Is cortisol testing part of a routine fertility workup?
No. The American Society for Reproductive Medicine (ASRM) 2021 fertility-evaluation guidance recommends a history-led, targeted endocrine assessment. Thyroid testing, prolactin measurement, androgen evaluation, and ovulation assessment are used when symptoms or examination findings indicate them; broad cortisol screening is not listed as routine testing.
That means a clinician normally investigates common causes of irregular ovulation, absent periods, semen factors, tubal disease, and other fertility issues before considering cortisol. Cortisol becomes relevant when the history points to an adrenal problem or when examination or imaging reveals a reason to investigate.

What cortisol can and cannot tell you
Cortisol testing can help diagnose disorders in which the adrenal glands or the brain–pituitary–adrenal system produce too much or too little cortisol. It cannot diagnose ordinary psychological stress, measure whether you are “too stressed to conceive,” or predict fertility from one result. A result outside a laboratory range needs clinical interpretation and, often, a different confirmatory test.
When testing for excess cortisol makes sense
The Endocrine Society’s 2008 Cushing syndrome guideline recommends testing people with multiple progressive features of cortisol excess, features that are unusual for their age, or an adrenal incidentaloma (an adrenal mass found on imaging done for another reason). It recommends against widespread testing in people without those indications.
Features that should prompt a discussion
- Easy or unusually extensive bruising.
- Facial fullness or other progressive changes in fat distribution.
- Proximal muscle weakness, such as difficulty rising from a chair or climbing stairs.
- Reddish-purple stretch marks that are broad or appear without a usual explanation.
- Osteoporosis at an unusually young age or fractures with little trauma.
- Hypertension that is early, severe, difficult to control, or otherwise unusual for you.
- An adrenal mass reported on CT, MRI, or another scan.
One isolated feature is not enough to diagnose Cushing syndrome. The pattern, progression, examination, and possible medication exposure determine whether biochemical testing is worthwhile.

Check medicines before ordering a test
Tell the clinician about every glucocorticoid exposure: tablets, injections, inhalers, creams, joint injections, nasal products, and medicines obtained from another clinic. The Endocrine Society specifically recommends a thorough drug history before biochemical testing because excessive prescribed or non-prescribed glucocorticoids can cause iatrogenic Cushing features and change the workup. Do not stop a steroid suddenly; tapering may need medical supervision.

When low cortisol or adrenal insufficiency needs urgent assessment
Adrenal insufficiency can be life-threatening, particularly if an adrenal crisis is developing. The Endocrine Society’s 2016 guidance lists otherwise unexplained volume depletion or dehydration, low blood pressure, low sodium, high potassium, fever, abdominal pain, skin darkening (hyperpigmentation), and low blood glucose as reasons to evaluate for primary adrenal insufficiency. Marked weakness, repeated vomiting, confusion, fainting, or collapse increases the urgency.
What to do in a possible adrenal crisis
If severe symptoms suggest adrenal crisis, seek emergency care immediately. Guideline-based management gives hydrocortisone before test results are available; waiting for a laboratory confirmation can be dangerous. Emergency clinicians can draw appropriate blood samples when feasible, but treatment should not be delayed for a result.
Symptoms that overlap with pregnancy
Fatigue, nausea, and vomiting are common in pregnancy, so they do not establish adrenal insufficiency. Persistent or severe symptoms, low blood pressure, dehydration, electrolyte abnormalities, hyperpigmentation, or hypoglycemia deserve medical assessment rather than attribution to normal pregnancy. A clinician may use a stimulation test when it is feasible and interpret the result in the context of pregnancy.

How pregnancy changes cortisol testing
Pregnancy raises cortisol-binding globulin and increases circulating cortisol concentrations approximately two- to three-fold. Those physiological changes can make tests designed for non-pregnant adults misleading, especially suppression tests that rely on a dexamethasone response.

Suspected Cushing syndrome during pregnancy
The Endocrine Society recommends urinary free cortisol (UFC) for the initial evaluation of a pregnant woman with suspected Cushing syndrome and recommends against dexamethasone testing initially. Pregnancy-related changes can produce misleading suppression-test results and false positives.
Urinary cortisol normally rises during pregnancy, so the trimester and the degree of elevation matter. The guideline notes that UFC values above three times the upper reference limit in the second or third trimester are concerning. A clinician may repeat collections and use additional testing to confirm the diagnosis; do not interpret a single urine value without the laboratory’s pregnancy-aware reference information.
Suspected adrenal insufficiency during pregnancy
For suspected adrenal insufficiency, the cosyntropin (corticotropin) stimulation test is the preferred confirmatory test for primary disease when it can be performed. Pregnancy and assay method affect interpretation, so an endocrinologist should select the protocol and cutoffs. If severe symptoms suggest crisis, treatment takes priority over testing.

If you are planning pregnancy and have signs of Cushing syndrome, adrenal insufficiency, an adrenal mass, or substantial glucocorticoid exposure, discuss evaluation before conception when possible. Coordinating reproductive care with endocrinology can clarify whether a condition is controlled, which medicines are appropriate, and what monitoring would be needed during pregnancy.

What happens when a clinician orders testing?
- Review the reason for testing. Explain your symptoms, their progression, menstrual and ovulation history, blood-pressure readings, fractures, skin changes, episodes of vomiting or fainting, and any adrenal imaging findings.
- List every steroid exposure. Include oral, injected, inhaled, topical, nasal, and joint medicines, plus recent courses. The clinician decides whether a medicine must be adjusted and whether testing should wait.
- Confirm pregnancy status and trimester. A test appropriate before pregnancy may not be the preferred first test once you are pregnant.
- Follow the collection instructions exactly. UFC involves collecting urine for a specified period, often with instructions about storage and missed samples. A stimulation test requires timed blood draws at baseline and after cosyntropin in a supervised setting.
- Complete confirmatory testing when indicated. A preliminary morning cortisol or an abnormal screening result does not automatically establish a diagnosis. Endocrinology may repeat the test or use a different method.
- Plan follow-up. Ask who will interpret the result, what reference range applies to your assay and trimester, and what happens if the result is borderline or abnormal.
How to understand the main numbers
For adrenal insufficiency, the 2016 Endocrine Society guideline identifies a peak cortisol below 500 nmol/L (18 micrograms/dL) at 30 or 60 minutes during a standard corticotropin stimulation test as indicating insufficiency, with assay dependence noted. Laboratories may use different modern assays, so the report’s method and reference range matter.
When stimulation testing is not feasible, a morning cortisol below 140 nmol/L (5 micrograms/dL) combined with ACTH is a preliminary indicator. It is not a stand-alone diagnosis, and a higher or intermediate value does not by itself rule out disease. Acute illness, recent steroid exposure, binding-protein changes, and timing can all affect interpretation.
For Cushing syndrome in pregnancy, UFC must be interpreted against pregnancy-appropriate expectations. Because normal urinary cortisol rises, a modest elevation is not equivalent to the marked elevation represented by more than three times the upper reference limit in the second or third trimester.
Can high cortisol cause infertility?
A cortisol result alone cannot establish that stress caused infertility. Routine fertility guidance does not recommend cortisol screening for people who are simply having difficulty conceiving. High cortisol may be a sign of an underlying endocrine disorder, and that disorder can affect health, menstrual function, blood pressure, or pregnancy planning; the clinical priority is identifying and treating the underlying condition rather than labeling ordinary stress as a cortisol disease.

If you have several progressive Cushing-like features, an adrenal incidentaloma, or significant glucocorticoid exposure, testing may be appropriate even when fertility is your main concern. If those features are absent, ask your clinician to focus the fertility evaluation on the history, ovulation, thyroid, prolactin, androgen findings, reproductive anatomy, and partner factors recommended by ASRM.
Is cortisol testing safe during pregnancy?
The safety question depends on the test. Urine collection and ordinary blood sampling do not expose the fetus to radiation or medication. A cosyntropin stimulation test is performed under clinical supervision when the information is needed. Dexamethasone suppression testing is avoided as the initial Cushing evaluation in pregnancy because it can be misleading, not because a home cortisol test is a safer substitute.
Do not self-order or self-interpret a consumer saliva or urine kit as an equivalent to guideline-directed laboratory evaluation. Such kits may use different collection times, reference ranges, and quality controls, and they cannot determine whether confirmatory testing or urgent treatment is needed.
When to seek immediate help
- Fainting, collapse, confusion, or inability to stay awake.
- Severe weakness with very low blood pressure or signs of shock.
- Repeated vomiting, severe abdominal pain, or inability to keep fluids down.
- Marked dehydration, low blood glucose symptoms, or a known adrenal disorder with sudden deterioration.
These symptoms warrant emergency assessment. Tell the team about pregnancy, adrenal disease, and any recent or current glucocorticoid medicine.
