For many large-breed dogs, start the conversation about senior wellness screening around 5–7 years of age. That range is an application of American Animal Hospital Association (AAHA) guidance that places middle age at about 7–8 years for most dogs, with large breeds often reaching it one or two years sooner. It is a planning window, not a universal “senior birthday”: your veterinarian should adjust it for expected lifespan, breed, medical history and current findings.
What “senior” means for a large-breed dog
There is no single age that makes every dog senior. AAHA defines senior status as the final 25% of a dog’s estimated lifespan, continuing through the end of life. Because lifespan and aging rate vary by breed and individual, two dogs of the same chronological age may need different monitoring plans. AAHA states: “Because of the variability in aging based on species and breed, there is no specific age for senior status.” (AAHA, Defining the Senior Patient.)
AAHA’s guideline text places middle age at approximately 7–8 years for most dogs and says large-breed dogs may reach middle age one or two years earlier (2023 AAHA Senior Care Guidelines). Subtracting one or two years produces a practical 5–7-year window for asking about baseline senior screening in many large dogs. The earlier end is not a diagnosis and does not replace an examination.

Wellness screening is preventive. A dog can appear energetic while blood, urine, blood-pressure or weight changes are developing. Testing during middle age gives the veterinarian baseline values for that individual dog. Later results can then be compared with the dog’s own history rather than only with a population reference range.

Starting the discussion in middle age also leaves time to establish a monitoring rhythm, review mobility and dental health, and identify breed-related risks before a problem becomes an emergency. Screening is different from diagnostic care: a new symptom should be evaluated when it appears, regardless of when the next routine check was scheduled.
A practical schedule by life stage
The intervals below summarize AAHA recommendations. They are planning guides; the veterinarian decides which tests are appropriate for a particular dog and whether results require earlier follow-up.
| Life stage | How to use the visit | AAHA timing guidance |
|---|---|---|
| Adult, before middle age | Routine preventive examination, vaccinations and risk review; establish normal weight, behavior and examination findings. | At least annually; AAHA’s canine life-stage checklist suggests considering visits every six months for mature, senior and geriatric dogs (AAHA Life Stage Checklists). |
| Middle age (often about 5–7 years for large breeds) | Discuss senior screening and obtain baseline data even if the dog seems well. | Retest at least annually during middle age (AAHA guideline). |
| Senior | Increase contact with the veterinary team and monitor trends in laboratory values, weight, mobility and behavior. | Routine health-care visits, including a minimum laboratory database, every six months are recommended in AAHA senior-care guidance. CBC, chemistry and urinalysis are listed every 6–12 months, depending on the individual plan. |
A six-month visit does not automatically mean every possible test is repeated each time. The interval is selected from the dog’s age, results, medications, risks and clinical signs.

History and physical examination
The appointment begins with questions about appetite, thirst, urination, stool, sleep, exercise tolerance, behavior and any changes noticed at home. The veterinarian reviews prior illnesses, surgeries, medications, supplements and laboratory trends. Weight and body-condition scoring help distinguish muscle loss from simple weight change.
The hands-on examination should include heart and lungs, abdomen, lymph nodes, eyes and ears, skin and any masses, oral and dental tissues, and an assessment of pain. Gait, posture, range of motion and orthopedic findings are especially important in large dogs, because subtle mobility changes may be easier to treat when addressed early. AAHA senior-care material also emphasizes reviewing quality of life and breed-specific risks.

Core laboratory screening
AAHA’s senior-dog table lists the following routine tests and frequencies (Diagnostic Tests and Recommended Frequencies):
| Test or assessment | Guideline frequency | What it can add |
|---|---|---|
| Complete blood count (CBC) | Every 6–12 months | Checks red and white blood cells and platelets for abnormalities that may be clinically silent. |
| Serum chemistry profile | Every 6–12 months | Assesses organ-related and metabolic markers; the veterinarian interprets values together, not as isolated numbers. |
| Urinalysis | Every 6–12 months | Adds information about urine concentration and other urinary findings that blood tests alone may miss. |
| Thyroxine (T4) | Annually | Provides thyroid screening when appropriate; abnormal results may require confirmatory testing. |
| Blood pressure | Annually | Detects hypertension that may not produce visible signs. |
| Heartworm and tick-borne disease testing | Annually | Follows preventive-care and regional exposure needs; the veterinarian selects the relevant tests. |
| Imaging and additional tests | When clinically indicated | Used when examination findings, history, breed risk or laboratory changes justify a closer look. |
These are guideline recommendations, not a requirement that every dog receive every test at every appointment. For example, an abnormal screening result, a new medication or a known chronic condition may justify testing sooner than the routine interval, while a stable dog may follow the longer end of a 6–12-month range.

How to interpret a baseline and later results
A baseline is most useful when it is tied to the dog’s age, weight, medications and clinical examination. One value outside a laboratory’s reference interval does not by itself establish disease, and a value inside the interval does not guarantee that nothing is wrong. The veterinarian looks for patterns across the CBC, chemistry profile and urinalysis, then compares future samples with both the laboratory range and the dog’s previous results.
Ask which findings are reassuring, which should be rechecked, and what change would prompt imaging or another targeted test. Keeping reports from different clinics together prevents an important trend from being missed when the laboratory or reference intervals change. If a sample needs confirmation, the follow-up timing may be days or weeks rather than the next routine six-month visit.
How your veterinarian individualizes the timing
Expected lifespan and breed
The “last 25% of expected lifespan” definition means the same chronological age can represent a different life stage in different breeds. Large-breed status is a reason to discuss screening earlier, but “large breed” still covers dogs with different aging patterns. Your veterinarian combines breed information with the dog’s actual condition rather than applying one cutoff to all large dogs.

Medical history, medication and prior results
Previous kidney, liver, heart, endocrine, orthopedic or cancer concerns can change both the tests and their frequency. Long-term medicines may require specific monitoring. Bring copies of outside laboratory reports and a complete medication and supplement list so the clinician can identify trends and avoid duplicating or misinterpreting tests.

Examination findings and symptoms
Weight loss, muscle loss, a new heart murmur, increased thirst, reduced stamina, stiffness or a mass can turn a wellness screen into a diagnostic evaluation. In that situation, the veterinarian may recommend targeted blood work, imaging or repeat testing sooner than the routine senior schedule.
Choosing between six-month and annual planning
When discussing options, compare the dog’s life stage and expected lifespan, breed-related risks, medical history and medications, examination findings, the specific tests proposed and how results will be used. An otherwise healthy middle-aged large dog may begin with at least annual screening and move toward six-month visits as senior status approaches. A dog with chronic disease, concerning findings or medication-monitoring needs may need a shorter interval. Conversely, the longer end of a 6–12-month testing range may be reasonable when results remain stable and the veterinarian sees no new risk.
Clarify whether “every six months” refers to a complete examination, a minimum laboratory database, or both. It is a contact schedule, not a promise that the same complete panel is repeated at every visit. The plan can be revised after each set of results.

- Progressive lethargy or a clear reduction in normal activity
- New or worsening behavior changes, confusion or altered interaction
- Changes in appetite or thirst
- Unintended weight loss
- Changes in gait, stairs, jumping, rising or general mobility
- Wounds that do not heal
Contact a veterinary practice promptly when one of these changes is new, progressive or severe. A preventive schedule is not a reason to wait; the appropriate appointment may be a same-week or urgent diagnostic visit rather than a routine wellness check.
How to prepare for the appointment
- Record changes at home. Note dates and patterns for drinking, urination, appetite, sleep, coughing, exercise tolerance, stiffness and behavior. Short videos of an unusual gait can help when a dog moves differently in the clinic.
- Bring health information. Provide medication and supplement names and doses, vaccination and parasite-prevention history, prior diagnoses, and laboratory or imaging reports from other practices.
- Ask for a life-stage plan. Ask whether the dog is considered middle-aged or senior based on expected lifespan, and what interval the veterinarian recommends for examinations and laboratory work.
- Clarify each proposed test. Ask what a CBC, chemistry panel, urinalysis, T4, blood-pressure reading or other test is intended to detect, how often it should be repeated, and what result would trigger follow-up.
- Agree on a follow-up method. Establish when results will be reviewed, whether a repeat sample is needed, and which symptoms should prompt an earlier call.
Common timing mistakes
Waiting for a “senior” birthday
Because senior status is based on estimated lifespan and not a universal age, waiting for a particular birthday can delay baseline testing. Begin the conversation during the large dog’s middle-age window instead.
Treating the guideline table as a mandatory package
The AAHA table supplies frequencies, not a one-size-fits-all prescription. A veterinarian may adjust tests for geography, exposure, breed risk, medications, previous results and the dog’s examination.
Confusing screening with diagnosis
Routine screening is for a dog that appears well. Progressive lethargy, appetite or thirst changes, weight loss, mobility changes or nonhealing wounds require an evaluation based on the symptom, even if the last wellness panel was normal.
Quick wins for a faster PC:

Normal results are a baseline, not a permanent clearance. Aging and disease risk change, which is why AAHA moves from at least annual middle-age retesting toward six-monthly senior health-care visits.

Bottom line for owners of large-breed dogs
Use roughly age 5–7 as a prompt to discuss senior wellness screening for a large-breed dog, while recognizing that the correct timing may be earlier or later. Plan at least annual monitoring during middle age and expect routine senior visits about every six months, with CBC, chemistry and urinalysis commonly scheduled every 6–12 months and annual T4 and blood-pressure checks when appropriate. The veterinarian should tailor the plan to lifespan, breed, history, medications, examination findings and symptoms.
Frequently Asked Questions
At what age should a large-breed dog start senior screening?
Discuss it around 5–7 years for many large-breed dogs. AAHA places middle age at about 7–8 years for most dogs and says large breeds may reach it one or two years earlier. The veterinarian should adjust the timing to breed, expected lifespan and health history.
Does every large dog need a full senior panel every six months?
No. AAHA recommends routine senior visits with a minimum laboratory database every six months, while its table lists CBC, chemistry and urinalysis every 6–12 months. The clinician chooses which tests and interval fit the individual dog.
What tests are commonly included in senior wellness screening?
The AAHA senior-dog table lists CBC, serum chemistry and urinalysis every 6–12 months, annual T4 and blood-pressure checks, and annual heartworm and tick-borne disease testing when appropriate. Imaging and additional tests depend on clinical indications and breed-specific risk.
Should a healthy-looking dog be screened before showing signs of aging?
Yes. Middle-age screening can establish personal baseline values and reveal clinically silent changes. It also gives the veterinary team a reference for interpreting future results.
What symptoms mean I should not wait for a routine wellness visit?
Progressive lethargy, behavior changes, appetite or thirst changes, unintended weight loss, altered mobility or nonhealing wounds warrant prompt veterinary contact. The appropriate visit may be diagnostic or urgent rather than a scheduled screening appointment.
How should I prepare for a large-breed senior check?
Track changes in appetite, drinking, urination, sleep, exercise, stiffness and behavior. Bring medication and supplement details, prior diagnoses, preventive-care records and earlier laboratory or imaging reports. Ask what each proposed test is intended to detect and when results will be reviewed.
