PSMA PET is used when a prostate-cancer team needs to locate disease that may change staging or treatment. Common, guideline-supported uses are selected initial staging, finding persistent or recurrent cancer after treatment, locating disease that appears nonmetastatic on conventional scans, and assessing PSMA expression before PSMA-directed radioligand therapy. It is not a universal screening test, a substitute for biopsy or PSA evaluation, or proof that cancer is absent when the scan is negative.
The value of a scan depends on the clinical question, prior treatment, PSA pattern, tracer, availability, and whether a result would alter management. A clinician must interpret the images with pathology and other imaging rather than treating a PET result as a stand-alone diagnosis.
What a PSMA PET scan shows
PSMA PET is molecular imaging. A radiotracer designed to bind prostate-specific membrane antigen (PSMA) is injected, and a PET scanner records where that tracer accumulates. The examination is commonly performed as PET/CT, so areas of tracer uptake can be assessed alongside anatomic information from CT.
Many prostate-cancer deposits express PSMA, which can make them visible even when they are difficult to identify on a routine CT or bone scan. That advantage has limits: uptake is not identical in every tumor, small deposits can be below the method’s detection capability, and benign or non-prostate conditions can sometimes produce findings that require correlation. The scan therefore answers a focused question—where PSMA-expressing disease might be—not the broader question of whether every cancer cell is present or absent.

When clinicians use PSMA PET routinely
The 2023 joint procedure standard from the European Association of Nuclear Medicine and the Society of Nuclear Medicine and Molecular Imaging lists four routine clinical uses. The exact decision still depends on local practice, the approved tracer, and the patient’s treatment plan.
Initial staging in selected patients
After a prostate-cancer diagnosis, imaging may be needed to determine whether disease has spread beyond the prostate. PSMA PET can be considered when the risk or clinical findings raise concern for metastatic disease and the result could change the choice between local treatment and a systemic or combined approach.
“Selected” matters. The guideline does not establish PSMA PET as an automatic scan for every newly diagnosed patient. The clinician weighs tumor features, examination, pathology, PSA context, existing imaging, and whether finding an involved lymph node or distant deposit would alter treatment.
Finding persistent or recurrent cancer after treatment
PSMA PET is used to look for disease that remains after surgery or radiation, or that has returned later. In the United States, the National Cancer Institute’s prostate-cancer PDQ describes FDA-approved Ga 68 gozetotide and F 18 piflufolastat PET/CT use for evaluating possible recurrence when PSA is elevated after treatment.
A scan can help distinguish a pattern that may be addressed with local salvage treatment from one that requires broader systemic management. The PSA trend, time since treatment, prior therapies, and other imaging remain part of the interpretation; the tracer image does not replace that clinical history.

Localizing disease called nonmetastatic on conventional imaging
When CT, MRI, bone scanning, or other conventional studies do not show metastases, PSMA PET may reveal sites that those tests did not identify. The purpose is not simply to obtain a more detailed picture. The scan is most useful when a different disease map would change radiation fields, surgery, systemic therapy, or the decision to pursue a potentially curative strategy.
Staging before PSMA-directed radioligand therapy
PSMA PET can establish whether tumors show enough PSMA expression for a PSMA-directed treatment pathway. This is a treatment-selection use, not a general statement that every person with prostate cancer should receive PET or radioligand therapy.

Regulatory wording is narrower than the broad idea of “better imaging.” The NCI PDQ describes FDA-approved indications for Ga 68 gozetotide and F 18 piflufolastat PET/CT as initial staging when metastatic disease is suspected and evaluation of possible recurrence based on elevated PSA after treatment. The specific product label, patient circumstances, and local availability control whether a particular scan is appropriate.
The FDA’s Pylarify approval announcement likewise describes imaging suspected metastasis in patients who might still be candidates for surgery or another potentially curative treatment, and imaging suspected recurrence associated with an elevated PSA. Those descriptions should not be expanded into a universal screening indication.
Potential uses that are less established
The joint EANM/SNMMI standard identifies biopsy guidance, imaging metastatic disease in particular situations, and monitoring systemic treatment as potential applications. Calling them “potential” distinguishes them from the routine uses above. Evidence, access, and clinical protocols vary, and a doctor must decide whether the information would be actionable in the individual case.

For example, repeating PSMA PET solely to monitor treatment makes sense only when the result has a defined role in a treatment decision or a protocol. A scan ordered without a plan for how a positive, negative, or indeterminate result would change care may add information without adding benefit.

Who may get little or no useful information
No single category in the cited guidance can be described as a person who will never benefit. Instead, limited usefulness is most likely when the test cannot answer a defined management question or when its technical and biological limits are likely to matter.
There is no decision attached to the result
If surgery, radiation, systemic therapy, or observation would be the same regardless of the scan, the examination may not improve care. Before scheduling, ask: “What will we do if it shows disease here, and what will we do if it does not?” A clear answer indicates that the scan is being used to resolve a real decision rather than to collect an additional image.
The proposed use is outside an established indication
Guideline-supported scenarios do not cover every possible request. Screening people without a prostate-cancer diagnosis, replacing a needed biopsy, or ordering a scan simply because conventional imaging is inconvenient is not established by the cited sources. A clinician may still consider an unusual use, but it should be presented as individualized rather than routine.
Disease may be below the detection limit
Small-volume deposits, disease with low or absent PSMA expression, or lesions in locations where tracer interpretation is difficult may not be visible. A negative scan therefore lowers the likelihood of detectable PSMA-expressing disease at that time; it does not prove that prostate cancer is absent. PSA behavior, pathology, treatment history, examination, MRI, CT, bone imaging, or follow-up may still require attention.

The result could be misleading without clinical correlation
Tracer uptake must be interpreted in context. Benign processes and other conditions can mimic cancer, while lesions with little PSMA expression can appear less conspicuous. An unexpected focus may need comparison with prior images, another imaging method, or tissue sampling before a major treatment change.
The patient-specific restrictions are not known
The sources cited here do not provide one complete contraindication list that applies to every tracer and every country. Kidney function, pregnancy status, medication considerations, ability to cooperate with the examination, and other safety issues are handled under the specific product label and the imaging center’s protocol. A nuclear-medicine physician or prescribing clinician must review those factors rather than relying on a generic internet list.
Can PSMA PET miss prostate cancer?
Yes. A negative result can occur when a lesion is too small, has insufficient PSMA expression, is obscured by normal tracer distribution, or falls outside the method’s practical resolution. The scan also can produce indeterminate or false-positive findings. For that reason, a negative PET should not be communicated as “no cancer,” and a positive focus should not be treated as definitive without clinical correlation.
The FDA reported a specific, selected estimate for Pylarify: among cases with positive PET readings and available corroborating tissue pathology, conventional imaging, or serial PSA information, recurrence or metastasis was confirmed in an estimated 85% to 87% of cases, depending on the reader. This was not a universal accuracy rate, sensitivity, specificity, or personal probability. It came from the subset in which corroborating information was available, so applying the percentage to every patient would be misleading.

Different approved or studied radioligands are not interchangeable in every operational detail. The joint guideline describes differences in radionuclide, radiochemistry, physiologic biodistribution, and interpretation pitfalls. Those differences can affect image appearance, normal-organ uptake, and how an equivocal focus is assessed.

The 2023 joint guideline does not establish that one of the cited PSMA radioligands has superior diagnostic accuracy together with better clinical outcomes than the others. “Newer,” “more available,” or “higher uptake” should not be presented as proof that one tracer is universally best.
PSMA PET for selecting radioligand therapy
PSMA PET has a distinct role when a treatment requires PSMA expression. In its July 31, 2026 announcement, the FDA stated that patients with metastatic androgen-pathway-modulation-naive or -sensitive prostate cancer should be selected for Pluvicto using Locametz or another approved PSMA PET product based on PSMA expression in tumors. The FDA’s March 28, 2025 announcement also describes PSMA PET selection for the expanded metastatic castration-resistant prostate-cancer indication.
These are treatment-specific contexts. A scan showing PSMA uptake informs eligibility for a particular therapy; it does not guarantee that the therapy will work, establish that all tumor sites express PSMA equally, or mean that a patient outside the labeled setting should receive it. Current product labeling and the treating team’s assessment control eligibility.

Questions to ask before scheduling
- What precise clinical question is this scan intended to answer?
- Which possible findings would change surgery, radiation, systemic treatment, or follow-up?
- Which tracer will be used, and what indication does its local label cover?
- How will a negative, positive, or indeterminate result be reconciled with PSA, pathology, and existing imaging?
- Are there patient-specific restrictions or preparation requirements under this tracer’s label?
- If the scan is for radioligand-therapy selection, what expression threshold or imaging criteria does the treatment protocol require?
Bottom line for patients and families
PSMA PET is most useful when a defined prostate-cancer management decision depends on locating PSMA-expressing disease: selected staging, suspected persistent or recurrent cancer, disease hidden by conventional imaging, or eligibility assessment for PSMA-directed radioligand therapy. It may offer little benefit when no decision would change, when the requested use is unsupported, or when disease is likely to be below the test’s detection or expression limits. The scan is one component of care, interpreted with PSA, pathology, treatment history, and other imaging—not a universal screening test or a guarantee that cancer is absent.
