Seek medical advice promptly for diarrhea with blood or pus in the stool, severe abdominal or rectal pain, repeated vomiting, dehydration, or changes in alertness. Adults should also contact a doctor if diarrhea lasts more than two days, they have a high fever, or they have six or more loose stools in a day. These are U.S. thresholds from the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK); children have different thresholds, and urgent-care routes vary by country.

When should an adult contact a doctor?

Diarrhea can become dangerous when it causes severe dehydration. NIDDK advises adults to talk with a doctor right away if they have any of these symptoms:

  • A change in mental state, such as confusion or unusual difficulty staying alert.
  • Frequent vomiting, or being unable to drink enough fluids or oral rehydration solution.
  • Severe abdominal or rectal pain.
  • Black, tarry stool, red blood in the stool, or pus in the stool.
  • Signs of dehydration.

NIDDK’s additional adult thresholds are diarrhea lasting more than two days, a high fever, or six or more loose stools in one day. These are reasons to seek medical advice, not a requirement to wait until a threshold is reached: bleeding, severe pain, dehydration, or trouble keeping fluids down can call for help sooner. See NIDDK’s symptoms and causes of diarrhea.

Older adults, pregnant people, people taking antibiotics, and people with weakened immunity are more likely to have problems and should keep in touch with a doctor. People with diabetes, kidney disease, or other chronic conditions should also ask a clinician what fluid replacement is appropriate for them.

When does a child need medical advice?

Children can become dehydrated quickly, so use the child-specific guidance rather than the adult time limits. NIDDK advises contacting a doctor promptly for diarrhea lasting more than a day, frequent loose stools, frequent vomiting, severe pain, dehydration signs, or black, bloody, or pus-containing stool. Also seek advice for a high fever in an older child.

  • For an infant, contact a doctor for any fever or diarrhea lasting more than a day.
  • For an infant, refusal to eat or drink for more than a few hours is a reason to seek advice.
  • Seek prompt advice for an infant under 12 months, a child born prematurely, or a child with other medical conditions.
  • Contact a clinician if a child cannot drink enough or does not improve with oral rehydration.

NIDDK lists no wet diaper for three hours or more as a possible dehydration sign in infants and young children. Its guidance on viral gastroenteritis also explains when children need medical care.

How to recognize dehydration

Watch for a combination of symptoms and changes from the person’s usual pattern. NIDDK and the NHS identify these possible signs:

  • Strong thirst or a dry mouth.
  • Urinating less than usual or having dark urine.
  • Tiredness, dizziness, or persistent dizziness when standing.
  • Sunken eyes.
  • In a young child, fewer wet diapers, no tears when crying, or a sunken soft spot (fontanelle).
  • Unusual drowsiness or difficulty waking.

NIDDK says anyone with signs or symptoms of dehydration should see a doctor right away. The NHS also advises urgent medical help for symptoms such as reduced or dark urine, persistent dizziness on standing, unusual tiredness or drowsiness, fast breathing or heart rate, few or no tears, or a sunken fontanelle. See the NHS page on dehydration.

When is it an emergency?

Severe dehydration can become life-threatening. Under the NHS pathway in the UK, call 999 or go to A&E if dehydration is accompanied by blue, grey, pale, or blotchy skin; cold skin; difficulty breathing or rapid breathing; confusion; or unusual sleepiness or difficulty waking. These can be signs of shock.

Those service numbers and locations apply to the UK only. Elsewhere, use your local emergency number or emergency department. Sudden severe abdominal pain, serious breathing difficulty, or not responding normally also warrant emergency action. Do not delay emergency care to try home rehydration first.

What to do while monitoring symptoms or arranging care

Replace fluids and electrolytes

For mild diarrhea without warning signs, drink fluids to replace what is lost. Water and electrolyte-containing fluids, including oral rehydration solution, can help replace fluid and salts. Oral rehydration sachets are mixed with water according to their instructions. If nausea makes drinking difficult, take small, frequent sips and increase the amount as tolerated. The NHS recommends continuing breastfeeding or bottle-feeding babies; do not dilute or weaken infant formula. Its diarrhoea and vomiting guidance covers home care and when to seek help.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Older adults and people with weak immunity, diabetes, kidney disease, or other medical conditions should ask a clinician whether oral rehydration solution is suitable and how much to use. If you cannot drink enough, or symptoms do not improve with oral rehydration, seek medical advice.

Eat as appetite returns; be cautious with medicines

When appetite returns, NIDDK says most people can usually return to their normal diet. Do not treat an anti-diarrheal medicine as a substitute for fluids or medical assessment. NIDDK says clinicians typically advise against over-the-counter antidiarrheals for infants, children, and people with bloody stool or fever. The NHS says not to give children under 12 medicine to stop diarrhea. If diarrhea worsens or continues for more than two days while taking an over-the-counter medicine, NIDDK advises contacting a doctor. Ask a clinician or pharmacist if you are unsure what is appropriate.

How long can diarrhea last?

The NHS says diarrhea usually stops within five to seven days and vomiting within one or two days. These are descriptions of what usually happens, not guarantees or instructions to wait. Seek care sooner for blood or pus, severe pain, dehydration, inability to keep fluids down, altered alertness, or other warning signs. NIDDK’s adult advice to seek medical input after more than two days uses a different threshold; follow the guidance relevant to your location and circumstances.

Which care pathway should you use?

Situation Next step
Mild diarrhea, no warning signs, and fluids can be kept down Replace fluids and electrolytes, and monitor symptoms.
Adult meets an NIDDK threshold, has a warning sign, or cannot maintain hydration Contact a doctor promptly; do not wait for a duration threshold if a serious symptom appears.
Child meets a child-specific threshold or shows dehydration signs Contact a doctor promptly; use a lower threshold for infants and children with higher-risk conditions.
Severe dehydration signs, difficulty breathing, confusion, or difficulty waking Use emergency services. In the UK, the NHS directs people to call 999 or go to A&E for the listed signs of shock; elsewhere, use the local emergency route.

For non-emergency advice, use your local clinician or medical advice line. NHS 111, 999, and A&E are UK services and should not be treated as universal care instructions.
