Constipation needs medical attention when it comes with rectal bleeding, constant or severe abdominal pain, vomiting, fever, inability to pass gas, lower-back pain, unexplained weight loss, or a major change in bowel habits. Severe pain with vomiting, abdominal swelling, or inability to pass stool or gas can indicate a potentially serious blockage and warrants immediate local emergency care.

This adult-focused guide separates emergency symptoms from reasons to arrange a prompt appointment. The symptoms listed cannot diagnose an obstruction, cancer, or another condition; they identify situations that need professional assessment.

What counts as constipation?

Constipation is not determined by bowel-movement frequency alone. NIDDK describes it as one or more of the following: fewer than three bowel movements in a week, hard, dry, or lumpy stools, painful or difficult passage, or a feeling that stool has not completely passed. People’s normal patterns differ, so a meaningful change from your usual bowel habit matters even if you still pass stool regularly.

Constipation is common. NIDDK, citing the American Gastroenterological Association’s 2013 medical position statement, reports symptoms in about 16 of every 100 adults and about 33 of every 100 adults aged 60 and older. Those figures are context from that source and year, not a newly collected 2026 estimate and not a threshold for deciding whether your symptoms are dangerous.

Get immediate medical care for possible obstruction symptoms

Seek immediate care through your local emergency pathway for severe or rapidly worsening abdominal pain, particularly when it occurs with other obstruction-type symptoms. Mayo Clinic lists constipation, vomiting, inability to have a bowel movement or pass gas, and abdominal swelling among possible symptoms of intestinal obstruction. These symptoms can have several causes, so do not assume you have an obstruction; the combination needs urgent assessment.

Symptoms that raise the urgency

  • Severe, persistent, or rapidly intensifying abdominal pain
  • Vomiting together with constipation or abdominal pain
  • Inability to pass gas or stool
  • A swollen, tight, or markedly distended abdomen
  • Collapse, confusion, or an inability to keep fluids down

Do not drive yourself if you are faint, severely ill, or in uncontrolled pain. Use the emergency number and service appropriate to your country. Avoid taking additional laxatives while waiting for assessment if severe pain, vomiting, or suspected blockage is present unless a clinician specifically instructs you to do so.

Contact a clinician promptly for these warning signs

NIDDK advises seeing a doctor right away when constipation occurs with rectal bleeding or blood in the stool, constant abdominal pain, inability to pass gas, vomiting, fever, lower-back pain, or unintentional weight loss. “Right away” is the source’s wording; it does not assign one universal response time for every person or country. Contact an urgent-care service, same-day clinician, or other local medical service to determine the appropriate route.

Bleeding or black stool

Bright-red blood on the paper or in the bowl, blood mixed with stool, and black or tar-like stool should not be written off as ordinary straining. Mayo Clinic includes rectal bleeding, blood in stool, and black stools among reasons to seek medical advice. Bleeding can arise from a local anal problem, but it can also reflect bleeding higher in the digestive tract. A clinician needs to determine the source.

Persistent pain, fever, or back pain

Constant abdominal pain is different from brief discomfort that improves after passing stool. Fever with constipation, or lower-back pain accompanying it, also warrants prompt assessment under NIDDK guidance. These combinations can occur with infections, inflammation, urinary or gynecologic problems, or other conditions; a symptom list cannot identify which one applies.

Unintentional weight loss

Weight loss that you did not intend, especially alongside a new bowel change, is a reason to arrange medical evaluation. Do not wait for a particular number of pounds or kilograms: the important feature is an unexplained trend rather than planned dieting or exercise.

Arrange an appointment when constipation persists, recurs, or changes

Book a routine or soonest-available appointment when self-care or treatment does not improve the problem, when episodes keep returning, or when symptoms progressively worsen. NIDDK recommends medical review when symptoms do not go away with self-care and when there is a family history of colon or rectal cancer.

NHS adult guidance (page reviewed 26 October 2023) advises seeing a GP when constipation is not improving with treatment, is regular or recurrent, or occurs with regular bloating, blood in the stool, unintended weight loss, persistent tiredness, a sudden change in bowel habits, or tummy pain. It also recommends discussing a medicine that may be causing constipation. Do not stop a prescribed medicine on your own; ask the prescriber about alternatives or dose changes.

Mayo Clinic adds consultation when constipation is more severe or lasts longer than usual, alternates with diarrhea, causes abdominal or rectal pain, produces unusual stool changes, interferes with daily activities, or continues for longer than three weeks. A fixed day count is not a universal rule: “longer than usual,” failure to improve, recurrence, and associated symptoms are more useful measures than one deadline.

Use this urgency guide

Situation Action Why
Severe abdominal pain, especially with vomiting, a swollen abdomen, or inability to pass stool or gas Seek immediate local emergency care These can occur with intestinal obstruction or another serious problem; assessment is needed rather than self-diagnosis.
Blood in stool or rectal bleeding, black stool, fever, constant pain, lower-back pain, or unexplained weight loss Contact a clinician promptly or use an urgent-care service NIDDK and Mayo Clinic identify these associated symptoms as reasons for medical evaluation.
Constipation that is not improving, keeps recurring, worsens, alternates with diarrhea, or follows a sudden bowel-habit change Arrange an appointment Persistent or changing symptoms may need a medication review, examination, or testing.
Mild, familiar constipation without warning signs Follow reasonable self-care and monitor the trend; seek advice if it fails to improve Normal bowel patterns vary, but ongoing or treatment-resistant symptoms should not be ignored.

This table is an organizing aid based on guidance from NIDDK, NHS, and Mayo Clinic, not a validated scoring system. If symptoms are severe or you feel acutely unwell, choose the more urgent route.

What to do while arranging care

Record the change from your normal pattern

Write down when the change began, how often you pass stool, stool consistency, straining, pain location, bloating, vomiting, fever, bleeding, and whether you can pass gas. Note whether diarrhea alternates with constipation and whether the problem affects eating, sleeping, work, or other daily activities. This information helps a clinician judge both urgency and possible causes.

Prepare a complete medication and health history

Bring the names and doses of prescription medicines, over-the-counter products, vitamins, and supplements. Tell the clinician about medicines started or changed before the constipation began, prior abdominal surgery, neurologic or metabolic conditions, pregnancy possibility, and any family history of colon or rectal cancer. NHS guidance specifically says to discuss medicines that might be causing constipation rather than stopping them without advice.

Avoid masking dangerous symptoms

Do not use repeated laxative doses to delay care when there is severe pain, vomiting, marked swelling, bleeding, or inability to pass gas. If you are waiting for advice and can drink safely, take small amounts of fluid; stop and seek urgent help if you cannot keep fluids down. Do not force a bowel movement or insert a rectal product when bleeding or severe pain is present without medical guidance.

What a clinician may assess

The appointment usually starts with questions about your baseline bowel pattern, timing, stool appearance, diet and fluid intake, activity, medicines, previous illnesses, surgeries, and family history. The clinician may examine the abdomen and, when appropriate, the rectal area. Depending on the findings, they may order blood tests, stool testing, imaging, or other investigations. The specific tests depend on age, examination findings, duration, and associated symptoms; there is no single constipation test for everyone.

Tell the clinician if symptoms began suddenly, are steadily worsening, alternate with diarrhea, or are accompanied by fatigue, bleeding, weight loss, pain, fever, or vomiting. These details can change the urgency and the choice of evaluation.

Why warning signs should not be self-diagnosed

Constipation with a red flag does not automatically mean cancer, obstruction, or another particular disease. For example, blood may come from an anal fissure or hemorrhoid, while abdominal pain may have digestive, urinary, reproductive, or muscular causes. Conversely, a serious problem can begin with symptoms that seem minor. The purpose of warning-sign guidance is to prompt an examination while treatment is more straightforward, not to label the cause from home.

Adults and children need different guidance

The NHS page cited here is specifically for adults and directs readers to separate advice for children. Do not apply the adult thresholds or medication advice to an infant or child without pediatric guidance. A child with severe pain, vomiting, a swollen abdomen, blood in the stool, or unusual sleepiness needs prompt medical assessment through a pediatric or emergency service.

Sources and dates behind this guidance

  • NIDDK constipation symptoms guidance lists rectal bleeding, blood in stool, constant abdominal pain, inability to pass gas, vomiting, fever, lower-back pain, and unintentional weight loss as reasons to see a doctor right away. The page was last reviewed in May 2018.
  • NIDDK’s definition and facts page explains that bowel patterns vary and reports the prevalence estimates based on the American Gastroenterological Association’s 2013 position statement; it was also last reviewed in May 2018.
  • NHS adult constipation guidance was last reviewed on 26 October 2023; its associated video or media was last reviewed on 12 May 2026.
  • Mayo Clinic’s adult constipation guidance includes unusually severe or prolonged symptoms, alternating diarrhea, pain, bleeding, black stools, unexplained weight loss, unusual stool changes, difficulty with daily activities, and symptoms lasting longer than three weeks. Its intestinal-obstruction guidance supports immediate assessment for severe pain or other obstruction symptoms.

These authorities use slightly different wording about urgency. Follow the service instructions where you live, and report any rapid deterioration rather than waiting for a routine appointment.
