A clinical AI system receives an image, record, or diagnosis request that differs from the cases it was validated on. It could still return a polished answer—but a safer system may flag uncertainty and route the case for review. That choice is useful only if teams know how often the system abstains, how well it performs when it does answer, and what happens next to the patient.
What does it mean for a clinical model to say “I don’t know”?
Uncertainty is a signal, not a guarantee
A model’s uncertainty estimate is information about how dependable its output may be. An abstention rule uses that information to withhold an answer or request human review when a case may not be handled reliably. Neither an uncertainty score nor abstention proves that the model has identified every error. A confident answer can still be wrong, and an uncertain answer can still be right.
That distinction matters in clinical use: uncertainty is not a safety certificate. The estimate and the decision to defer both need evaluation in the task, population, and setting where the system is intended to operate.

Why a model may be uncertain
A case may differ from the inputs used in development and validation, or the model may not distinguish reliably among possible outputs. In either situation, the score needs to be interpreted in context. If the input is outside the system’s intended use or does not align with its validated data, that limitation should be visible to the people using the output—not hidden behind a numerical confidence value.

How should selective prediction be evaluated?
Selective prediction measures a model that can answer some cases and abstain on others. Reporting accuracy only among answered cases gives an incomplete picture: a system can appear more accurate by declining difficult cases, while transferring the unresolved work elsewhere. Evaluate answer quality and referral burden together.
- Performance among answered cases: How well does the model perform on cases it does answer?
- Coverage or rejection burden: What share of cases receives an answer, and what share is deferred?
- Cases deferred: Which kinds of cases are being sent for review, and how severe or time-sensitive are they?
- Subgroups and sites: Does performance or abstention differ across patient groups or clinical locations?
- Workflow outcome: Who reviews deferred cases, how quickly, and what happens if review is unavailable?
Compare systems at matched coverage or referral rates where possible. Otherwise, a method that defers far more cases may look better simply because it answers fewer challenging ones. These are useful evaluation dimensions, not a universal regulator-prescribed metric set.

What does the medical-text study show—and what does it not show?
A February 25, 2025 arXiv preprint by Vazhentsev and colleagues evaluates uncertainty methods across text-based mortality prediction, medical-code prediction, outpatient diagnosis classification, and mental-health-related datasets. The authors report that results vary by task, which is a reason not to assume one method or threshold will work across clinical applications. Read the preprint.

This is a result for that dataset and evaluation—not an expected gain in deployment, a recommended rejection rate, or evidence that HUQ-RDE is best for another task. The paper also reports that label-wise selective prediction can behave differently from instance-wise methods, and that poorly calibrated class probabilities can make common thresholds problematic.

What should clinicians see when the model is uncertain?
A useful interface should make the status of the output clear and help the user decide what to do. A score without context can invite overconfidence or be mistaken for a probability that the answer is correct.
- State the system’s intended use and show when an input is outside that use or differs from development and validation data.
- Explain the uncertainty signal in language suited to the user; distinguish a model-generated estimate from a verified diagnosis.
- Make limitations, known failure modes, relevant confidence intervals, and gaps in population representation accessible to the people who need them.
- For an abstained case, specify the next step and responsible person or service. Do not treat “refer for review” as a complete workflow unless the review route exists.
FDA, Health Canada, and MHRA transparency principles emphasize information about intended use, development, performance, and—when available—logic, as well as the performance of the human-AI team. They also call attention to known biases or failure modes, confidence intervals associated with outputs, data-characterization gaps, underrepresented populations, evaluation limitations, and inputs that do not align with development and validation data. See the FDA transparency principles.

How should a health system validate and monitor abstention?
Test locally before relying on the output
Performance measured during development does not establish performance at a particular hospital or for its patient population. Use site-specific acceptance testing to check the model and the human-AI workflow under intended local conditions. Review who is answered and who is deferred, including subgroup differences, and confirm that staff can manage the review workload. The appropriate operating threshold depends on the task, consequences of error, and available follow-up—not on a result from an unrelated dataset.

Clinical data and patterns of use can shift. Track operating characteristics over time using a valid sample of the relevant population and clinical circumstances, and establish a process for reviewing failures as well as successes. FDA Commissioner Robert M. Califf warned in March 2024: “The ability of algorithm to provide accurate assessments will drift if left untended, often in unpredictable, and sometimes dangerous ways.” Read Califf’s remarks.

Monitoring should be paired with change management: teams need a way to investigate a change in performance, decide whether use should be adjusted, and communicate changes to affected users. The World Health Organization’s 104-page framework, published November 17, 2021, covers evidence generation from development through post-market surveillance and includes cervical cancer screening as a use case. See the WHO publication record.
What makes abstention a useful safety measure?
Abstention helps only when it is evaluated as part of the full clinical service: the model’s answers, the cases it defers, differences across people and sites, and the human response to those referrals. If deferred cases have no reliable route to review, the system has shifted uncertainty rather than resolved it. A model should sometimes say “I don’t know”—but its users need to know why, how often, and what happens next.
