Cleanup can defeat a retry limit if it erases the history that limit needs before the next recovery decision. In a Windows incident reported for Orca 1.4.198, the project traced repeated terminal remounts to a mismatch: remount eligibility and budget cleanup consulted different indexes. The fix aligned the cleanup check with the lookup that governed remounting; it did not lower the recovery cap or resolve every disagreement between the indexes.
How a retry limit depends on cleanup
A recovery limiter works only if it can count recent recoveries when deciding whether to allow another one. That requires both recording each recovery and retaining its timestamp until the next admission decision. If cleanup deletes those timestamps too early, a subsequent attempt can look like the first one, even though the cap itself has not changed.
That distinction is central to the Orca case: the issue was not that the documented cap was too high. The cleanup path could conclude that a still-remountable terminal was gone and release the history needed to enforce the cap.
What the Orca incident report says happened
Orca is a desktop tool for organizing terminals and agent work. The incident was associated with Orca 1.4.198 on Windows. The project’s pull request describes the user’s diagnostic bundle as showing 8,878 remounts across eight tabs in 122.4 seconds, with a median gap of 10 milliseconds. The documented limiter allowed three recoveries per tab per five minutes. The PR attributes the renderer failure to a Skia bitmap allocation abort. These are figures and an explanation reported by the project from the user’s diagnostic bundle, not measurements independently reproduced for this article. Orca pull request #19745

The author of the matching article says the original crash bundle was not obtained, and the desktop crash was not independently recreated. The incident is therefore best understood as a production failure reported by the project, rather than a crash reproduced here.
Why cleanup could reset the budget
The failure involved two indexes that normally corresponded but could diverge on the direct-SSH path. One represented terminal rows; the other represented unified UI tabs. Remount eligibility used terminal rows in tabsByWorktree. The older cleanup path called getTab, which read unifiedTabsByWorktree.
- A terminal row remained present, so the remount path still considered the terminal eligible.
- The corresponding unified UI entry was missing, so cleanup inferred that the tab had disappeared.
- Cleanup cleared that tab’s recovery timestamps.
- The next remount could proceed without the history that should have counted toward the per-tab limit.
In other words, the action being throttled and the action releasing its throttle history did not use the same test for whether the tab still existed. The recovery record could be discarded while the terminal remained eligible for another recovery.

What changed—and what did not
The merged change made budget release use the same terminal-row lookup that governed remounting. It also consolidated resolution through a shared lookup primitive, reducing the chance that admission and cleanup would drift apart again. The pull request’s author, OrcaWin, summarized the design principle this way: “The budget exists solely to throttle remountTerminalTabForRecovery, so its release condition must be the negation of that function’s success condition.” Pull request #19745

This was a targeted repair, not a general solution for disagreement between the two indexes. The pull request explicitly left broader index consistency out of scope. In the reverse-divergence case—where retained history remains but the UI entry is absent—the PR says that history expires with the configured recovery window rather than persisting indefinitely.
How the fix was checked
The project describes fixture tests that retained the original budget calculation, disposal, remount, and generation-update logic, while supplying fixtures for state plumbing, time, timers, PTY, and logging. Electron was not started, and the tests did not reproduce graphics-memory exhaustion. The pull request reports 43 tests passing in a targeted regression run. It also reports that restoring the old predicate caused 200 remounts in the storm regression, while the proposed fix passed that regression. These are test results reported by the project, not population-level measurements or independent test runs.

The PR also lists terminal-pane and store suites, type checking, and a changed-code-quality check. Its additional coverage addressed three important failure modes:
- Real closure: a test exercised the genuine tab-close path to check that closing a tab still releases its budget.
- Expiry: a recovery window test checked that history does not become permanent.
- Stale generations: coverage checked that requests tied to an old generation do not keep acting on a replaced display instance.
Together, these checks matter because simply suppressing repeated remounts could break legitimate recovery or leave stale state behind. The intended repair preserves normal release and expiry behavior while preventing cleanup from releasing a budget for a terminal that remains eligible.

What this case teaches about retry budgets
When reviewing a limiter, inspect the code that removes its history as carefully as the code that admits an action. For each cleanup path, ask:
- What exact action is being throttled?
- Which object identity or lookup makes that action eligible?
- Does cleanup use the same identity test before deleting its timestamps?
- Can a partially missing UI or index entry make cleanup disagree with the action path?
- Do genuine closure, time-based expiry, and stale-generation handling still work after the fix?
The practical principle is narrow but reusable: a limiter’s release condition should match the negation of the condition that permits the throttled action. Otherwise, cleanup can erase the evidence that the limiter needs to do its job.
