Allen Brooks’s retrospective process found engineering issues, but its health report missed the most important failure: three proposed fixes had produced no implementations. In his account, the report measured observations and how many became automation or process—not whether proposed work was actually picked up. A passing or healthy-looking check is useful only if it measures the state you need to know.
What the retrospective was designed to do
In an August 24, 2026 DEV Community post, engineer Allen Brooks describes a post-merge retrospective that reviewed batches of merged stories through eight specialist lenses. Each lens could file findings or explicitly pass, then route findings to immediate work, the roadmap, or an out-of-scope decision. The process was generative rather than a merge gate: it could surface patterns, but it could not stop a merge.
Brooks gives examples of what the reviews surfaced: authorization handled differently across GraphQL and REST paths; route registration that depended on agreement among multiple registries; acceptance tests asserting against an unwired recorder; and a milestone that shipped early capabilities without its planned payment step. These are examples from his account, not an independent audit or a measure of how often the process finds defects.
Why the healthy report did not show the failure
After the second retrospective, Brooks says the health report showed nine standing observations across the charters and four observations that had graduated into automation or process. He considered that ratio healthy. But the report did not count how many proposed stubs had been ingested as work.

The first retrospective had proposed three stubs—scoped work proposals—and none had been built by the next retrospective. Brooks says two issue classes recurred in that next batch. One proposed reachability check was meant to replace existing route checks; instead, separate checks were added, recreating the duplication the proposal had warned about. In his view, seeing the pattern repeat made the follow-through failure visible in a way a single review could not.
The distinction is between measuring the life of observations and measuring what happens to recommendations. A healthy count of persistent or automated observations cannot answer whether proposals became owned work or were completed. Brooks summarizes the deeper problem this way: “I did not build a broken gauge. I built a correct gauge pointed at the wrong thing, which is worse, because a broken gauge eventually reads implausible and a correct one never does.”

How a missing retrospective looked like zero findings
A separate flaw meant some retrospectives never ran. Brooks says each fleet session used a fresh Git worktree, while the accumulator resolved its state relative to the current working directory. Because each new worktree appeared to have no state, three runs of two merged stories each—six stories in total—did not trigger a retrospective.
He reports moving state resolution to Git’s common directory, shared by linked worktrees in a clone, and adding a legacy-file adoption path that reports a migration rather than silently treating the state as absent. He also says he added 15 tests using real Git repositories and linked worktrees; nine failed against a mutant that restored the incorrect path resolution. Those implementation and test counts are Brooks’s report, not independently verified results.

This is a broader measurement trap: “no events observed” can mean either a measured zero or that the system never saw the relevant state. The storage scope must match the intended scope of the process, and the worktree setup where the tool actually runs should be part of its tests.
Why green CI checks can still hide a failure
Brooks also describes a pull request where every visible check was green, but a required test check was missing. The test fan-in job had never been created, so there was no failing check context to display. A system that only searches for failed checks could interpret the absence as success.

Required-check logic needs to establish both that the expected check exists and that it passed. “Absent” and “passing” are different states, just as “unmeasured” and “measured zero” are different states. Brooks’s general rule is: “any counter that can read zero has to be able to distinguish zero from unmeasured, and has to say which one it means.”

Brooks says he added a reconciliation script that parses the latest retrospective board, extracts stubs, looks for cards with matching identifiers, and reports stubs that have not been built. It runs before the fleet starts a batch and before another retrospective can open. The script reports the gap rather than automatically filing work, leaving the decision about what becomes a card with him.

He separately reports changing the accumulator’s state resolution and adding the legacy worktree-local file adoption path. He says that accumulator change was not itself a retrospective finding.
For the second retrospective’s five stubs, Brooks reported that one—the script for reporting unbuilt stubs—had been built five days later. The merge-gate script, mutation-evidence rule, brief-writing rule, and milestone verb report were still unimplemented when he wrote the post. That is a dated status from August 2026, not a current repository check. The retrospective had exposed a follow-through problem, but its own loop was not yet solved.
What to measure in your own process
Brooks’s account suggests checking the transitions between discovery and action, not just the health of the discovery mechanism. A lightweight audit can ask:
- Finding to proposal: Was an observation turned into a specific, scoped recommendation?
- Proposal to work: Was the recommendation accepted, assigned or added to a tracked work item—or explicitly declined with a reason?
- Work to completion: Was the item completed, and can the team point to the change?
- Expected signal to observed signal: Did the check, job, accumulator, or counter actually run and produce a result?
- State across environments: Does stored state survive the real execution pattern, including fresh or linked worktrees?
These checks separate reporting from gating. A retrospective can remain non-blocking and still report unowned proposals; a team can choose which recommendations to implement without letting them disappear from view. The important point is to label what the system knows: measured zero, not measured, absent, passing, proposed, accepted, or completed. As Brooks puts it, “The thing they share is that each of them collapsed two states that needed to stay apart.”
