Gabapentin and pregabalin are not routine treatments for osteoarthritis (OA). They are medicines used for epilepsy and for some forms of neuropathic (nerve) pain. UK guidance recommends them for neuropathic pain, but the NICE OA guideline does not recommend them for OA itself. Before starting, changing or stopping either medicine, ask a clinician to clarify what is causing your pain, whether nerve pain is also present, what benefit is expected and how treatment would be reviewed.

Are gabapentin and pregabalin osteoarthritis medicines?

Not routinely. Gabapentin and pregabalin are often described as epilepsy drugs because they are used in epilepsy care. They are also options for some neuropathic pain. That second use does not make either medicine an established treatment for the joint pain of osteoarthritis.

NICE guideline NG226, published in the United Kingdom on 19 October 2022, sets out OA care. It recommends tailored therapeutic exercise for everyone with OA and gives specific advice about topical non-steroidal anti-inflammatory drugs (NSAIDs), walking aids and other non-drug measures. It does not recommend routine gabapentin or pregabalin for OA.

NICE guideline CG173 addresses neuropathic pain, a different clinical category. For adults with neuropathic pain other than trigeminal neuralgia, it recommends a choice of amitriptyline, duloxetine, gabapentin or pregabalin as initial treatment options. If one is ineffective or not tolerated, another listed option can be offered. That recommendation is about the pain mechanism, not about an OA diagnosis.

The NHS describes pregabalin as a medicine used for epilepsy and nerve pain, among other uses. This general medicine information is not an OA recommendation. A prescription can be reasonable for a particular person only after a clinician has considered the suspected pain type, diagnosis, other conditions and current medicines.

Why identifying the pain type matters

Osteoarthritis pain

OA is a condition of a joint. A person may have pain, stiffness and reduced function related to that joint, but not every painful symptom in someone with OA is necessarily caused by OA. The existence of an OA diagnosis does not prove that every new or unusual symptom has the same cause.

Neuropathic pain

Neuropathic pain results from a problem involving the nervous system rather than solely from joint tissues. NICE treats it separately because medicines that can help nerve pain are not automatically useful for ordinary OA pain. A clinician may need to check whether the symptoms fit the existing OA diagnosis, suggest another condition, or indicate that more than one pain problem is present.

Chronic primary pain is another category

NICE chronic-pain guidance says not to start antiepileptic drugs, including gabapentinoids, to manage chronic primary pain, except when gabapentinoids are offered as part of a clinical trial for complex regional pain syndrome. It also says that when an underlying condition adequately explains the pain, management should follow guidance for that condition. This is another reason not to treat the word “pain” as a single indication.

What NICE recommends for osteoarthritis

Tailored therapeutic exercise

NG226 says to offer tailored therapeutic exercise to all people with OA. “Tailored” means the programme should be adapted to the person’s joint involvement, abilities and goals rather than treated as a one-size-fits-all routine. Exercise is part of OA care even when medicines are also used.

Topical NSAIDs

For knee OA, NICE’s exact recommendation is: “Offer a topical non-steroidal anti-inflammatory drug (NSAID) to people with knee osteoarthritis.” For OA in other joints, it says to consider a topical NSAID. A topical NSAID gel is therefore a guideline-backed category to discuss, including topical diclofenac for knee osteoarthritis, but suitability, product labelling and availability vary. The recommendation is not a direction for every individual to buy or use a particular product.

Medicines alongside non-drug care

When medicines are needed, NICE advises using them alongside non-pharmacological treatment and at the lowest effective dose for the shortest possible time. That principle applies to the overall OA plan; it does not turn gabapentin or pregabalin into routine OA medicines.

Walking aids and supports

For lower-limb OA, NICE says to consider walking aids such as walking sticks. It does not recommend routine braces, insoles, tape, splints or supports. Those aids may be considered when there is joint instability or abnormal biomechanical loading, exercise is ineffective or unsuitable without an aid, and the aid is likely to improve movement and function. A walking aid that improves mobility is a different decision from using a brace simply because a joint hurts.

How the options differ

Option Condition or situation addressed What the UK guidance says Important qualification
Tailored therapeutic exercise Osteoarthritis Offer to all people with OA Programme should be adapted to the person’s abilities and goals
Topical NSAID OA, especially knee OA Offer for knee OA; consider for OA in other joints Check product suitability, labelling and availability
Walking aid, such as a stick Lower-limb OA with mobility needs Consider when it may help Need and suitability are personal
Brace, insole, tape, splint or other support Selected OA circumstances Not for routine use Consider only with instability or abnormal loading, inadequate exercise without an aid, and likely functional benefit
Gabapentin or pregabalin Neuropathic pain; also used for epilepsy NICE CG173 lists either as an initial option for adult neuropathic pain (except trigeminal neuralgia) This does not establish either medicine as routine treatment for OA

Questions to settle before trying a prescribed medicine

A useful appointment is not simply a request for a particular tablet. It is a discussion about the diagnosis, the expected benefit and a safe review plan. Consider asking the following questions.

  • What pain are we treating? Ask whether the clinician thinks the pain is OA-related, neuropathic, another condition, or a combination.
  • Why this medicine? Ask what feature of the assessment makes gabapentin or pregabalin appropriate and what other options fit the diagnosis.
  • What benefit should be expected? Agree on a practical goal, such as improved walking, sleep or daily activity, rather than assuming that any prescription will remove all pain.
  • How will benefit be reviewed? Ask when the response and tolerability will be assessed and what would count as insufficient benefit.
  • What conditions and medicines matter? Provide a complete list of prescriptions, over-the-counter products and supplements, and mention relevant medical conditions. The prescriber or pharmacist can then check for cautions and interactions.
  • What side effects should prompt contact? Ask which effects are expected, which require urgent advice and how activities such as driving should be handled under the prescribed treatment.
  • How would a reduction or switch work? Ask for the plan before treatment begins, particularly if the medicine is already being taken or might be used for an extended period.

These are discussion prompts, not an individualized treatment protocol. The cited guidelines do not determine whether a specific reader should receive either medicine.

If you already take gabapentin or pregabalin

Do not stop, restart or switch a prescribed regimen on your own. NICE neuropathic-pain guidance says withdrawal or switching should be tapered to take account of the dose and possible discontinuation symptoms. The exact schedule depends on the medicine, dose, duration and the reason it was prescribed, so it must come from the prescriber or another qualified clinician managing the treatment.

  1. Contact the prescriber or pharmacist before making a change.
  2. Explain why you are considering a change: no improvement, troublesome effects, a new diagnosis, pregnancy planning, another medicine, or concern that the pain is not neuropathic.
  3. Ask whether the diagnosis and treatment goal should be reassessed before any reduction.
  4. Obtain a written or clearly explained tapering or switching plan, including what to do if symptoms or discontinuation effects occur.
  5. Arrange the review that the clinician recommends rather than judging the medicine from a single difficult day.

A taper is not the same as an abrupt stop, and a switch is not automatically safe simply because the replacement medicine is intended for pain.

How to prepare for a clinician or pharmacist review

Record the pattern, not just a pain score

Before the appointment, note which joint is affected, when symptoms occur, what activities are limited and whether the current pattern differs from your established OA symptoms. Include the timing of any other symptoms and any recent change in function. This information helps the clinician decide whether the problem still fits OA or needs another assessment.

Bring an accurate medicine list

Include prescribed medicines, non-prescription products and supplements, with the way you actually take them. Mention previous problems with medicines and any condition that could affect the choice. Do not assume that a medicine used for another diagnosis is irrelevant to a pain decision.

Define a review point

Ask when the treatment will be reviewed, which functional improvement matters, and what happens if the target is not met. A review point prevents an ineffective medicine from continuing indefinitely without a clear reason.

When the OA diagnosis may need another look

Seek clinical reassessment when symptoms do not fit the OA explanation, change substantially, or remain disabling despite an appropriate OA plan. The guidelines cited here do not diagnose an individual reader, and they cannot determine from a webpage whether a symptom comes from OA, neuropathic pain or another condition. A clinician may need to revisit the joint diagnosis, consider coexisting pain mechanisms and align treatment with the condition that best accounts for the symptoms.

UK guidance, dates and local differences

The recommendations in this article come from UK NICE and NHS sources. NG226 was published on 19 October 2022. NICE CG173 on neuropathic pain is dated 20 November 2013 and was last updated 22 September 2020 in the source record. Chronic-pain guidance adds the restriction on starting antiepileptic drugs for chronic primary pain outside the stated clinical-trial exception.

Guidance, licensing, formularies, product labels and availability can differ outside the UK. If you live elsewhere, check the guidance and medicine information that apply in your jurisdiction. Even within the UK, the product supplied and the monitoring plan may depend on the prescriber, local formulary and your medical history.

The practical takeaway

If someone with OA is offered gabapentin or pregabalin, the key question is not “Which epilepsy drug is best for arthritis?” It is “What type of pain is being treated, and why is this medicine appropriate for that type?” NICE supports exercise and, particularly for knee OA, a topical NSAID as part of an OA plan. Gabapentin and pregabalin belong in a discussion about neuropathic pain or another specific indication, not as automatic treatments for OA. Clarify the diagnosis, agree on a measurable goal and review plan, and taper any existing prescription only with professional guidance.
