The documented health hazard from quartz (engineered-stone) countertops is inhaling respirable crystalline silica dust created when the material is cut, ground, drilled, polished, installed, modified, or cleaned up. An intact, installed countertop is not the same exposure scenario. Government health guidance focuses on workers who generate or disturb that dust, and government health guidance does not establish a household risk level for simply owning, touching, or living near an intact slab. That evidence gap is not a basis for claiming either that every countertop is dangerous or that every household circumstance is risk-free.
What the health concern actually is
Quartz is a common natural form of crystalline silica. Engineered stone marketed as quartz combines stone particles with binders and can contain a very high proportion of crystalline silica. The hazard is an exposure route: airborne particles small enough to travel deep into the lungs, not the ordinary act of touching a finished surface.
Respirable crystalline silica dust
Cutting, grinding, polishing and drilling can release respirable crystalline silica (RCS). Dry sweeping, compressed-air cleanup and other activities that disturb settled dust can also put particles back into the air. The risk rises when a worker repeatedly performs these tasks or works where controls are inadequate.

Why “toxic” is an incomplete description
Calling an intact countertop “toxic” skips the key question: how could silica enter someone’s lungs? A solid, undisturbed surface does not create the same airborne dust exposure as fabrication. Conversely, a countertop can become a serious occupational hazard if someone cuts or modifies it without effective controls. The material and the activity must be discussed together.
Diseases linked to excessive silica exposure
Silicosis
CDC/NIOSH states: “Silicosis is a lung disease caused by inhaling excessive amounts of respirable crystalline silica.” It also states, “It is not reversible, but it is preventable.” Silicosis is an occupational disease caused by excessive inhalation over time; CDC/NIOSH’s occupational-health material does not say that merely having a quartz counter in a home causes it.
Once established, silicosis cannot be reversed. Prevention therefore depends on stopping or controlling dust before it is inhaled. Anyone who has performed stone fabrication or installation should tell a clinician about that work history, especially when discussing respiratory concerns. A clinician can decide what evaluation is appropriate; this article cannot diagnose a condition or prescribe a screening schedule.

Other serious diseases
CDC/NIOSH identifies lung cancer, chronic obstructive pulmonary disease (COPD), certain autoimmune diseases and chronic renal failure among serious diseases associated with excessive RCS exposure. “Associated with excessive exposure” is important wording: these statements describe the health consequences recognized for substantial silica exposure, not proof that a particular countertop in a particular home caused an illness.
Who faces the clearest risk?
The strongest evidence concerns workers who manufacture, finish, install, repair or otherwise modify stone. Relevant tasks include:
- Measuring, cutting or shaping slabs.
- Grinding and polishing edges or surfaces.
- Drilling holes for sinks, taps or other fittings.
- Dry handling, sweeping or blowing dust after fabrication.
- Installing, removing or repairing pieces when cutting or grinding is required.
Exposure depends on the dust generated, the duration and frequency of the work, ventilation, wet methods, housekeeping and respiratory protection. A person who spends a shift performing these tasks has a fundamentally different exposure than a resident who uses a completed counter for food preparation.

What the reported numbers mean
Several government reports illustrate the seriousness of workplace exposure. Each number has a specific population, location and date; none is a national estimate of household risk.

The 2019 CDC report documented 18 cases among stone-fabrication workers in four states, many of whom principally worked with engineered stone. California’s later count shows how surveillance has grown in one jurisdiction, but the two reports cannot be combined into a single national trend.
Is an intact quartz countertop dangerous in a home?
Government health guidance does not quantify household exposure from an intact, installed countertop. It does not establish that touching, wiping or preparing food on an undamaged surface causes silicosis, nor does it provide a measured risk level for passive presence in a kitchen.

That limitation should be stated precisely. It means the available evidence is about occupational dust-generating work, not that every possible household circumstance has been proven harmless. A slab that is cracked, being cut for a new appliance, drilled for a fixture or ground during a repair is no longer an “intact, undisturbed” scenario. Dust-producing work should be treated as a professional job requiring appropriate controls.
What controls are expected during stone work?
The control hierarchy
NIOSH presents a layered approach:
- Elimination or substitution: Where feasible, choose a material containing little or no crystalline silica. NIOSH cautions that a substitute must not introduce a different danger.
- Engineering controls: Use methods and equipment that prevent dust from entering the breathing zone, such as properly designed wet processes and dust capture.
- Administrative controls: Organize work, maintenance, training and access so fewer people and fewer work hours are exposed.
- Personal protective equipment: Respiratory protection and other PPE are an additional layer, not a replacement for source control and safe housekeeping.
NIOSH’s OSHA/NIOSH hazard alert dated Feb. 26, 2026 addresses employees manufacturing, finishing and installing natural and engineered-stone countertops. It gives employers guidance intended to keep exposure below OSHA’s permissible exposure limit. The guidance is written for workplace programs, not as a do-it-yourself recipe for remodeling a kitchen.

California requirements
California’s resource dated Sept. 9, 2026 describes enhanced protections in the state’s general-industry silica rule for specified high-exposure trigger tasks on countertop slabs and panels. It identifies engineered or artificial stone containing more than 0.1% crystalline silica by weight as covered material for those tasks. Listed protections include effective wet methods, safe housekeeping, respiratory protection, medical surveillance, medical-removal protections and reporting.

Those are California requirements. They should not be presented as national rules; employers and workers elsewhere must check the applicable regulator and standard for their jurisdiction.

If your countertop is installed and undisturbed
- Use normal surface-care instructions from the installer or manufacturer.
- Do not assume the presence of quartz means you need a respirator, air purifier or home silica test. Government health guidance does not establish those purchases as necessary for an intact countertop.
- Keep the distinction between ordinary contact and activities that create airborne dust.
If cutting, drilling or grinding is needed
- Do not improvise a DIY cutting job. The task can create respirable dust even when the visible work area looks small.
- Hire a qualified stone professional and describe exactly what must be changed, such as a sink opening, cooktop cutout or plumbing hole.
- Ask how dust will be controlled. A competent contractor should be able to explain wet methods or other engineering controls, housekeeping, worker protection and disposal of dust and slurry.
- Keep occupants away from the work area and follow the contractor’s containment and cleanup directions. Do not sweep or blow visible dust yourself.
- Ask for the required local compliance information if the work is performed by a business, because rules differ by jurisdiction.
A respirator by itself is not a complete control plan. The cited guidance treats PPE as one layer after elimination or substitution, engineering controls and administrative measures.
Should you choose another countertop material?
For a remodel, ask suppliers and fabricators for the product’s crystalline-silica composition and for the work practices they use during fabrication. NIOSH places substitution with materials containing little or no crystalline silica at the top of its control hierarchy when the substitute is not itself dangerous.
Current government guidance does not provide a complete consumer ranking of quartz, granite, porcelain, natural stone and other surfaces. It therefore cannot support a blanket statement that one named countertop category is universally safe or universally safer. A meaningful comparison should consider both the material’s silica content and the dust created during fabrication, installation and any later modification.
When to discuss an occupational history with a clinician
If you have worked in countertop fabrication, installation or repair, tell your healthcare professional the type of stone, the tasks performed, the years of work and whether dust controls were used. That history helps place respiratory or other health concerns in context. The California and CDC case reports show that serious disease has occurred among workers, but they do not establish that every exposed worker will become ill or specify an individual’s risk.
Bottom line for homeowners and workers
Quartz countertops are not accurately summarized as simply “toxic.” The established hazard is inhaling respirable crystalline silica dust, especially during cutting, grinding, drilling, polishing and dusty cleanup. Engineered stone can contain more than 90% crystalline silica, and excessive exposure can cause irreversible silicosis and is associated with other serious diseases. An intact installed surface is a different situation, and current government sources do not quantify a household risk level for it. Treat any dust-generating modification as professional, regulated work with layered controls, and do not mistake a respirator or ordinary cleaning for a substitute for source control.
