“T-cell immunity test” is not one standardized test or score. The report might show T-cell counts, a laboratory stimulation response, CD8-specific function, or a tuberculosis (TB) interferon-gamma release assay (IGRA). Those tests answer different questions, so interpretation starts with the exact assay name, its reference interval, and why it was ordered. A low value can indicate an abnormality in the function measured, but it does not by itself diagnose broad immunodeficiency; an in-range value does not guarantee protection from infection.
Start with the exact test name
Ask the ordering clinician or laboratory which test appears on the requisition and report. “T-cell immunity” may refer to several unrelated measurements:
| Test category | What the laboratory does | Typical output | What it can address | Important limitation |
|---|---|---|---|---|
| T-cell subsets or counts | Flow cytometry identifies populations such as CD3, CD4 and CD8. | Absolute counts and/or percentages | How many cells of selected types are present in the blood sample | A count does not show whether those cells function normally. |
| Lymphocyte proliferation | Patient cells are exposed to mitogens or specific antigens, and their growth response is measured. | Mitogen and antigen proliferation results | Whether circulating lymphocytes respond to selected stimulation | Severe T-cell lymphopenia, specimen timing and other clinical factors can make the result difficult to interpret. |
| Global CD8 functional assay | CD8 cells are assessed for interferon-gamma response and CD107a/b expression. | A value compared with the laboratory’s CD8 reference interval | Selected functional competence of CD8 T cells | It is specific to CD8 cells and does not establish overall T-cell competence. |
| TB IGRA | Blood is exposed to TB-related antigens and the interferon-gamma response is measured. | QFT-Plus: positive, negative or indeterminate; T-SPOT: positive, borderline, negative or invalid | Likelihood of infection with TB bacteria | It is not a general test of immune-system strength. |
Do not compare a CD4 count, a proliferation index, a CD8 functional value and an IGRA number as if they were different scales of one “immunity score.” They measure different biological questions and use different reference populations and reporting rules.
T-cell subset and count tests
What is measured
Flow cytometry labels cells by surface markers and reports populations such as total T cells (CD3), helper T cells (CD4) and cytotoxic T cells (CD8). A report can include both a percentage of lymphocytes and an absolute number of cells per volume of blood. The percentage and absolute count may tell different stories when the total lymphocyte count changes.

How to read the interval
Use the reference interval printed on that report, including its age category and units. Intervals are not universal: methods, instruments, calibration and reference groups differ between laboratories. Mayo Clinic Laboratories notes biological variation in lymphocyte populations and recommends consistent timing when results are being followed serially. A sample taken at a substantially different time of day can complicate an apparent trend.
What an abnormal count means
A low number can document lymphopenia in a particular subset. It does not prove that the remaining cells are dysfunctional, nor does a high percentage prove adequate protection. Clinicians generally interpret the pattern with the total white-cell count, medication history, underlying illness, recent infection and any prior results rather than from one isolated number.
Lymphocyte-proliferation assays
How the assay works
The laboratory incubates blood lymphocytes with mitogens, which broadly activate cells, or with selected antigens, which test a narrower response. It then measures how much the cells proliferate compared with an unstimulated control. A mitogen result asks whether cells can respond to a general trigger; an antigen result asks about responsiveness to that particular stimulus.

What a low or abnormal response can indicate
Mayo Clinic Laboratories states that abnormal mitogen or antigen proliferation can support concern about impaired T-cell function when the clinical context fits. The result is not a stand-alone diagnosis. Profound T-cell lymphopenia can leave too few cells for a meaningful response, and collection, transport or processing delays can affect cell viability. The laboratory also cautions that a proliferation response does not map perfectly to clinically significant immunodeficiency.

Why clinical correlation matters
Medications that suppress immunity, acute illness and the reason for testing all influence the meaning of a result. A clinician may confirm an unexpected finding, compare it with cell counts and immunoglobulin studies, or decide that a result is not interpretable because the specimen conditions were unsuitable.
CD8 functional-competence assays
A specialized CD8 assay measures two selected responses: interferon-gamma production and expression of the degranulation markers CD107a and CD107b. Mayo’s catalog describes values below its reference interval as consistent with impaired CD8 function, often in the setting of excessive immunosuppression. Values above the interval are described as unlikely to have clinical significance.
The scope is deliberately narrow. Mayo states: “This assay is specific only for CD8 T cells; it does not provide information for overall T-cell competence.” A result in range therefore means that the measured CD8 responses fall within that laboratory’s expected interval; it does not establish that every T-cell population, antibody response or infection defense is normal.
The published 95% confidence-interval reference values were derived from 102 healthy adult donors. That reference group does not automatically apply to children, older adults, people with major illnesses, transplant recipients or anyone whose sample was handled differently. The report’s own interval and interpretive comments take precedence.

Is a TB IGRA the same as a T-cell immunity test?
No. A TB IGRA uses T cells as the responding cells, but its purpose is to detect an immune response to TB-specific antigens. It does not measure total T-cell numbers or general cellular immune competence.
QFT-Plus and T-SPOT categories
- QFT-Plus: The Centers for Disease Control and Prevention (CDC) describes results as positive, negative or indeterminate.
- T-SPOT: Results are positive, borderline, negative or invalid.
A positive IGRA requires medical evaluation for TB disease; the blood test alone cannot determine whether infection is latent or active. A negative result must be considered alongside exposure risk, symptoms and other clinical findings. CDC says that a borderline, indeterminate or invalid result did not provide useful information about the likelihood of TB infection.
IGRA reports include antigen and control responses, but CDC states that specific guidance is not available for interpreting the numerical values by themselves. Do not apply a universal cutoff to a number from one assay or use it to estimate overall immune strength.
How to interpret the result step by step
- Identify the assay and specimen. Record the complete test name, specimen type, collection date and laboratory. “T-cell test” is not specific enough.
- Read the laboratory’s reference interval. Check units, age or sex categories, and whether the value is absolute, a percentage, a ratio or a stimulation response.
- Classify the laboratory flag. Note whether the report says low, high, positive, negative, borderline, indeterminate, invalid or uninterpretable. A numerical value without its category can be misleading.
- Check controls and quality comments. For proliferation tests and IGRAs, control failures or inadequate cellular responses may make the result non-informative rather than genuinely negative.
- Match the result to the test’s purpose. A subset count concerns cell quantity; proliferation concerns response to stimulation; a CD8 assay concerns selected CD8 functions; an IGRA concerns TB-antigen response.
- Review timing and treatment. Note recent infections, vaccination, corticosteroids, transplant or cancer medicines and other immunosuppressive therapy. Ask whether collection time and transport conditions were appropriate.
- Look for corroboration. The clinician may compare prior results, repeat an unexpected finding or order other immune studies. No single laboratory test defines impaired cellular immunity.
- Apply the clinical question. For an IGRA, discuss TB exposure and symptoms. For a cellular-function test, discuss recurrent or unusual infections, the reason for testing and whether the measured abnormality changes management.
What a low result can—and cannot—tell you
The word “low” has meaning only within the assay that produced it. A low CD4 or CD8 count means fewer cells were measured in that sample. A low proliferation response means reduced growth after the tested stimulus. A low CD8 functional value means the measured interferon-gamma or CD107a/b response was below that laboratory’s interval. These are not interchangeable diagnoses.

Potential influences include immunosuppressive medicines, an underlying disease, a recent or ongoing infection, biological variation, specimen delay and an unsuitable control response. The same numerical value can have different implications in a person being monitored after transplantation than in someone evaluated for recurrent infections. The ordering clinician decides whether the finding is reproducible and clinically important.
Mayo Clinic Laboratories emphasizes that “No single laboratory test can identify or define impaired cellular immunity on its own.” A low result can justify further evaluation; it cannot, by itself, label someone as globally immunodeficient or predict exactly how often that person will become ill.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. Why a normal result is not a guarantee of protection
An in-range subset count shows that the measured population falls within the laboratory’s expected interval. A normal proliferation result shows an adequate response to the particular mitogen or antigen used. A normal CD8 assay shows that two selected CD8 functions were within range. None evaluates every arm of immunity, every pathogen or the quality of protection after vaccination.

Mayo notes that further studies are needed to connect in-range values from its CD8 assay with different infection risks. A person can therefore have a normal report and still need ordinary preventive care, vaccination decisions based on clinical guidance and evaluation of symptoms or recurrent infections.

Timing, specimen handling and repeat testing
For serial lymphocyte subset monitoring, collect samples at a consistent time when possible. Counts vary biologically, and changing the collection schedule can create an apparent rise or fall unrelated to a true change in immune status.
Functional assays depend on living, responsive cells. Prolonged transport, incorrect temperature, insufficient volume or delayed processing can produce a weak or invalid response. Ask the laboratory whether the sample met its acceptance criteria before treating an unexpected result as a change in immunity.
Repeat testing is not automatic. It is most useful when the result conflicts with the clinical picture, a control failed, the specimen was compromised or a treatment decision depends on confirmation. The clinician should choose the timing so that the repeat answers a defined question rather than simply generating another isolated number.
Questions to take to the ordering clinician
- What exact cells or response did this assay measure?
- Is the value outside the laboratory’s age-appropriate reference interval?
- Was the result valid, and did all required controls pass?
- Could medication, recent illness, specimen transport or collection timing have affected it?
- Should the test be repeated or compared with an earlier result?
- Which additional findings would establish whether the result matters clinically?
- If this is an IGRA, how do my TB exposure risk, symptoms and examination change the interpretation?
Bring the complete report rather than a transcribed number; the method, units, interval and interpretive comments are part of the result.
When an IGRA category needs particular attention
A positive IGRA should be followed by medical assessment for TB disease and a decision about the appropriate next evaluation. A negative IGRA is less reassuring when exposure risk or symptoms are substantial. Borderline, indeterminate and invalid categories are non-informative rather than reassuring negatives; the clinician may consider repeat testing or another approach based on the person’s risk and circumstances.
For any non-TB T-cell assay, urgent decisions should be based on the person’s condition and treating team, not on a web interpretation of one value. The report can identify a measured abnormality, but diagnosis and treatment require the complete clinical picture.
Key points
- There is no universal T-cell immunity score.
- Counts measure quantity; proliferation and CD8 assays measure selected functions; an IGRA addresses TB-antigen response.
- Interpret every value against the reporting laboratory’s interval, method and reference population.
- A low result warrants context and sometimes confirmation, not an automatic diagnosis of immunodeficiency.
- A normal result does not guarantee complete immune protection.
