Recovery after laparoscopic endometrioma surgery usually includes abdominal soreness or bloating, tiredness, and sometimes shoulder pain caused by the gas used during laparoscopy. Many people can go home the same day or after a short stay, but the operation’s extent determines what happens next. Removing an ovarian endometrioma alone is not the same as treating extensive endometriosis involving other organs.
Plan for help with chores, rest during the first week, and short, gentle walks as tolerated. Use the written instructions from your surgical team for wound care, medicines, lifting, work, exercise, sex, driving, and follow-up. Fever, increasing pain or swelling, heavy bleeding, wound drainage, fainting, persistent vomiting, trouble urinating, chest pain, breathing difficulty, or a painful swollen leg need prompt medical assessment.
What recovery feels like in the first hours
Anesthesia commonly causes drowsiness, nausea, poor concentration, or a generally “washed out” feeling for the rest of the day. Your abdomen may feel tender, tight, or bloated around the small incisions. Pain should be managed with the medicines prescribed at discharge rather than by waiting until it becomes severe.

Movement and the recommended pain medicine may help. The different time ranges are general guidance, not a guarantee for an individual operation.

Before leaving, confirm who to call outside office hours, when your follow-up is due, how to take each medicine, and which restrictions apply to your specific procedure. If you are still very nauseated, cannot keep fluids down, feel faint when standing, or cannot pass urine, tell the hospital staff before discharge.
The first week at home
Rest, then gentle movement
Johns Hopkins advises planning to rest for about a week and arranging help with cooking, shopping, childcare, and strenuous housework. Rest does not mean remaining in bed all day. Walk around the home for short periods as you are able, increase activity gradually, and stop if pain, dizziness, or fatigue worsens. Gentle movement also helps reduce stiffness and may ease gas-related shoulder discomfort.
That guidance includes avoiding lifting more than 10 pounds and avoiding strenuous housework until your clinician says it is safe. Treat the 10-pound limit as a Johns Hopkins discharge instruction, not a universal rule: a larger cystectomy, additional endometriosis treatment, or a complication may require stricter limits, while another patient may receive different instructions.

Endometriosis UK says it is normal to feel weak and tired for about ten days after a laparoscopy, and longer when surgical treatment has been performed. Fatigue can fluctuate: a shower, a short walk, or an appointment may leave you needing a nap. Schedule essential tasks first, keep commonly used items within easy reach, and ask someone else to handle lifting and transport while you recover.

Eating, drinking, and bowel movements
Follow the discharge plan for fluids and food. Anesthesia, reduced activity, and opioid pain medicines can contribute to constipation. Johns Hopkins advises taking medicines as directed and preventing constipation; use a stool softener or laxative only if your clinician approves it for you. Contact the team if vomiting persists, abdominal swelling is worsening, or you cannot keep fluids down.
Incision care
Keep the laparoscopic wounds clean and follow the dressing instructions you were given. Johns Hopkins recommends washing an incision with mild soap and water, then patting it dry. Do not apply oils, powders, or lotions unless your surgical team specifically instructs you to do so. Do not pick at glue, strips, or scabs; allow the closure to come away according to the team’s directions.
Look at the incisions once or twice a day so you can notice a change. Mild tenderness and limited bruising can occur, but spreading redness, increasing swelling, pus-like drainage, wound separation, or pain that is becoming harder to control should be reported promptly. Ask before soaking in a bath or swimming, because the timing depends on how the wounds were closed and whether there are additional vaginal or abdominal procedures.
How long does recovery take?
There is no single endometrioma-surgery timetable. The relevant variables include the size and location of the cyst, whether an ovarian cystectomy was performed, whether endometriosis was removed elsewhere, blood loss, adhesions, complications, and your general health. A limited laparoscopy may have a shorter recovery than extensive surgery, which can involve a longer hospital stay or additional procedures.

Do not interpret a “week” or “ten days” as a promise that you will be fully recovered then. Feeling better and being cleared for strenuous activity are separate milestones. If recovery is not steadily improving, contact the operating team rather than trying to push through.
Returning to work, exercise, sex, and driving
Work
Desk work may be easier than a job involving standing, lifting, climbing, or long travel, but the sources do not establish a universal return-to-work date. Ask your surgeon for a plan based on the actual procedures performed and your progress. Build in shorter days or additional breaks if fatigue remains significant.
Exercise and lifting
Begin with walking as tolerated. Do not move directly from resting to running, abdominal workouts, heavy lifting, or strenuous housework. The Johns Hopkins 10-pound restriction applies until your clinician clears you; another service may set a different limit. Pain, bleeding, dizziness, or a feeling of pressure is a reason to stop and call for advice.
Sex, tampons, and douching
Johns Hopkins advises not having sex and not using tampons or douches until the doctor says it is safe. The appropriate timing depends on the operation, internal healing, bleeding, and any additional treatment, so substitute your surgeon’s clearance for a fixed number of days.

Driving
Driving advice is procedure- and medication-dependent. Ask the surgical team when you may drive, and do not drive while drowsy, impaired by pain medicine, or unable to sit comfortably and react quickly. Check any local or insurer requirements that apply to you.
When symptoms need urgent attention
Use the emergency contact number on your discharge paperwork. If symptoms are severe or rapidly worsening, follow local emergency instructions rather than waiting for a routine appointment.

UCLH states that serious complications at laparoscopy are uncommon, affecting approximately two in every 1,000 women. That institutional figure applies to the context of its laparoscopic surgical information and does not predict the risk of your particular endometrioma procedure.
Why the exact operation changes the recovery plan
“Laparoscopic endometrioma surgery” can describe more than one operation. An ovarian cystectomy removes the cyst wall; surgery may also treat endometriosis on the pelvic lining, bowel, bladder, or other structures, or release adhesions. The number and position of incisions may be similar while the internal work and recovery demands are very different.

NICE defines ovarian cystectomy as surgical excision of an ovarian endometriotic cyst and recommends discussing benefits, risks, effects on symptoms, and the possibility of future surgery. The European Society of Human Reproduction and Embryology’s 2022 guideline recommends cystectomy rather than drainage and coagulation when surgery is performed for an ovarian endometrioma, citing lower recurrence and less endometriosis-associated pain. Those are treatment-choice recommendations made before or during surgical planning; they do not replace postoperative instructions for an individual.
Ask at follow-up exactly what was done, whether tissue was sent for analysis, whether other endometriosis was treated, and whether the surgeon found anything that changes lifting, sex, work, or exercise restrictions. If your discharge sheet conflicts with a general website, the operating team’s instructions take priority.
Follow-up and the possibility of symptoms returning
Keep the postoperative appointment even if the wounds look good. The clinician can review pathology, check healing, adjust pain treatment, discuss menstrual or pelvic symptoms, and plan ongoing endometriosis care. Recovery from the incisions does not by itself show whether endometriosis symptoms will recur.

This is a long-term observation from that institution, not a prediction for one person and not a measure of immediate recovery. New or returning pelvic pain, painful periods, pain with sex, bowel or bladder symptoms, or fertility concerns deserve a planned discussion rather than self-treatment.

- When is my follow-up appointment, and will pathology results be discussed then?
