An IUD (intrauterine device) is placed through the cervix into the uterus during a short office procedure. The insertion itself commonly takes less than five minutes, although the NHS advises allowing at least 30 minutes for the appointment. Cramping, pressure, or a brief sharper sensation are possible, but pain varies too widely to predict for one person. You can discuss pain-control choices, ask questions, and tell the clinician to pause if you need a break.

This guide explains the visit from preparation through the first days afterward, including what differs between copper and hormonal devices and which symptoms need prompt medical advice. It is general information, not a diagnosis or a substitute for advice from the clinician placing your IUD.

Before the appointment: checks and decisions

Your clinician first confirms that an IUD is appropriate and that placement can be done safely. The visit usually includes a review of your medical history and an examination of the vagina, cervix, and uterus. A pregnancy test may be offered or performed, and STI testing may be suggested when your sexual history or symptoms make it relevant. These tests are not automatically required in every situation; the clinician decides what is needed based on your health and the information available. Planned Parenthood and the American College of Obstetricians and Gynecologists (ACOG) describe this as an individualized eligibility assessment.

Tell the clinic what you need

When booking, mention prior difficult pelvic examinations, fainting with medical procedures, anxiety, trauma-related needs, or a preference for a slower explanation. Also say whether your priority is completing the procedure quickly, reducing acute pain, or having the ability to stop and reconsider. ACOG’s 2025 guidance says pain options should be discussed with and offered to every patient seeking an in-office gynecologic procedure, with the choice made through shared decision-making. Read the ACOG clinical consensus.

When placement can happen

ACOG says an IUD can be inserted at any point in the menstrual cycle when pregnancy has been reasonably excluded and there are no signs of infection. The appropriate timing and whether you need backup contraception depend on the specific device, when it is inserted, and local clinical guidance. Do not assume instructions for a hormonal IUS apply to a copper IUD; ask for the directions for the exact product being placed. The NHS provides separate fitting guidance for an IUD and a hormonal IUS.

Make a practical plan

Have your medicines, allergies, relevant conditions, and the date of your last menstrual period available. Ask whether test results need to be available before the appointment. If you have previously felt faint or anticipate needing recovery time, arrange transportation or leave space in your schedule. You can also ask how long the clinic expects the visit to take, where a support person may wait, and whom to contact if you need to change the appointment.

Plan for pain instead of guessing how it will feel

Many people experience cramping or pain during insertion, but intensity is highly individual. There is no reliable way for a clinician to predict one person’s pain in advance. Planned Parenthood notes that some people report more intense discomfort, including some who have not experienced childbirth or who have a history of trauma, depression, or anxiety. ACOG cautions that these characteristics cannot reliably forecast an individual experience. Use them to guide a conversation about support, not to assume how your insertion will go.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Some people describe pressure when the speculum is placed, a strong but brief cramp as the inserter passes through the cervix, or cramping after the device is released. Others report little discomfort. The procedure being quick does not make anyone’s pain less real, and a difficult prior examination does not guarantee that this insertion will be difficult. Tell the clinician what you are feeling in real time rather than comparing your experience with someone else’s.

Options to discuss before the procedure

  • Local anesthetic: A clinician may be able to use a local anesthetic technique to reduce pain. Whether it is appropriate and available depends on the practice and your medical situation.
  • Other comfort measures: Ask about positioning, pauses, verbal explanations, a support person where permitted, and a signal that means stop. The NHS says you can ask for the fitting to stop at any time.
  • Medication: ACOG’s patient guidance explains that nonsteroidal anti-inflammatory drugs (NSAIDs) taken before insertion do not reliably control immediate insertion pain by themselves. They may help cramps afterward. Ask your clinician or pharmacist whether a particular medicine is safe for you, considering allergies, pregnancy possibility, stomach or kidney problems, other medicines, and local instructions. This article does not provide a dose.

Clinics differ in what they can offer, so ask about the plan when you schedule or before you are positioned. ACOG quotes Kimberly Hoover, MD, FACOG, emphasizing that patients may prioritize speed, acute-pain reduction, or the ability to pause and try another intervention. ACOG’s May 15, 2025 release explains this approach.

What happens during insertion

1. You review the plan

The clinician confirms which IUD you chose, reviews relevant history, and answers last-minute questions. If pregnancy or infection has not been reasonably excluded, placement may be postponed while the issue is evaluated. Tell the clinician if you feel unwell, have new pelvic symptoms, or want to revisit pain control. You can ask what will happen next, what sensations are expected, and how you will communicate if you want a pause.

2. You get into position

You will be positioned for a pelvic examination. A speculum is inserted into the vagina so the cervix can be seen. You may feel pressure or stretching. You can ask the clinician to explain each step, slow down, or pause. Agree on a simple stop signal before the procedure begins, especially if speaking might be difficult while you are tense. You remain in control of communicating discomfort; the NHS specifically says you may ask for the fitting to stop at any time.

3. The IUD passes through the cervix

The clinician guides an inserter through the cervix and into the uterus, then releases the device. Cramping can occur as the cervix and uterus respond. Some people feel a short, intense cramp; others feel mainly pressure or little discomfort. Tell the clinician what you are feeling rather than trying to endure silently. The clinician may adjust the pace or stop to discuss another option. If your priority changes during the appointment, say so; shared decision-making can continue throughout the procedure.

4. The strings are left in place

After placement, flexible threads extend through the cervix into the upper vagina. They allow a clinician to check the device and make later removal possible. You may be shown how the threads feel, although you do not have to check them immediately if you are uncomfortable. If the threads later cannot be felt or seem noticeably different, use another contraceptive method until a clinician confirms the IUD’s position and follow the urgent-care advice below.

5. You have a chance to recover

Once the inserter and speculum are removed, the clinician may give you a moment to sit or lie down. Ask for help if you feel dizzy or unsteady. Before leaving, confirm the device name, insertion date, expected effectiveness, backup-contraception instructions, and the clinic’s contact number. Written instructions are useful because it can be hard to remember details immediately after a procedure.

How long the visit takes

Time estimates describe different parts of the appointment. Planned Parenthood says the placement process usually takes less than five minutes. The NHS describes fitting as about 5–10 minutes and recommends allowing at least 30 minutes for the appointment, which includes preparation and recovery. These are typical estimates, not a promise: a consultation, examination, pain-control discussion, difficult cervical access, or a decision to stop can make the visit longer.

Source and scope Published estimate How to use it
Planned Parenthood, placement process Usually less than five minutes Describes the insertion portion rather than the entire visit.
NHS, IUD fitting About 5–10 minutes for fitting; allow at least 30 minutes for the appointment Builds in time for preparation and post-procedure needs.

Immediately after placement

People respond differently. You may feel normal immediately, or you may have period-like cramps, a backache, dizziness, or light spotting. Sit or lie down until you feel steady, and tell the clinic if dizziness is persistent or severe. A heating pad can be a comfort measure for cramps. An over-the-counter pain reliever may also help if it is suitable for you; check with a clinician or pharmacist rather than following a generic dose.

Mild cramping can come in waves during the first hours. Light bleeding or spotting may be noticeable even if your usual periods are predictable. These symptoms should trend toward manageable discomfort rather than steadily intensify. If you have work, school, caregiving, or travel planned immediately afterward, leave flexibility in case you prefer to rest. A heating pad is optional comfort equipment, not a required purchase or a treatment for a serious complication.

Before leaving, ask the clinician to write down the IUD type, the date it was inserted, when it becomes effective, whether backup contraception is needed, and how follow-up works in your area. Keep the clinic’s contact number. Instructions vary by device and jurisdiction, so use the advice given for your specific IUD rather than a general online schedule.

What to expect over the next few days and weeks

Cramps and spotting

Period-like cramps and light bleeding can continue for a few days. They should gradually become manageable rather than steadily intensify. Rest, normal hydration, and a heating pad may make the period-like discomfort easier to tolerate. Ask a clinician or pharmacist whether an over-the-counter medicine is suitable for you; do not assume a pre-insertion NSAID plan will control insertion pain or that any medicine is safe with your health conditions.

Copper IUD bleeding pattern

A copper IUD contains no hormone. The NHS says it can make periods heavier, longer, or more painful, particularly as your body adjusts. Ask the prescriber what changes are expected for your particular model and what symptoms would be outside the expected range. A heavier period pattern is different from very heavy bleeding that makes you feel faint or unwell, which needs prompt medical advice.

Hormonal IUD or IUS bleeding pattern

A hormonal IUD, often called an IUS in NHS materials, can make periods lighter over time. Irregular bleeding or spotting may occur early on before the pattern settles. The timing and duration of these changes differ among products and people, so your device-specific information sheet is the best reference. Early irregular bleeding does not by itself tell you whether the device is positioned correctly; follow the clinic’s instructions and report concerning symptoms.

Device category Hormone present? Bleeding and cramping pattern described by guidance Contraception instructions
Copper IUD No Periods may become heavier, longer, or more painful. Follow the instructions for the exact copper model and insertion timing.
Hormonal IUD/IUS Yes Periods may become lighter over time; early irregular bleeding can occur. Follow the product- and timing-specific instructions from your clinician.

Effectiveness, backup contraception, and STI protection

An IUD’s contraceptive effect does not follow one universal rule for every device. Whether you need condoms or another method for a short period depends on the IUD type, the day of your cycle, and local guidance. Ask exactly when your device is effective and what to use until then. Record that date with the device name so you do not have to rely on memory.

IUDs do not protect against sexually transmitted infections. Use condoms or another barrier method when STI protection is needed, even when the IUD is providing contraception. STI testing and treatment decisions are separate from the contraceptive effect of the device.

Warning signs that need medical advice

Contact a clinician promptly for severe or worsening lower-abdominal or pelvic pain, fever, abnormal or foul-smelling discharge, very heavy bleeding, or a suspected pregnancy. These symptoms should not be dismissed as ordinary insertion cramps. The NHS lists these concerns in its IUD side-effects guidance.

Call for advice if you cannot feel the threads or they seem shorter, longer, or otherwise different. Until a clinician checks the device, use another contraceptive method. The ACOG IUD information also advises attention to severe pain, fever, unusual discharge, heavy bleeding, or possible pregnancy. Seek urgent care according to local emergency guidance if symptoms are severe, rapidly worsening, or accompanied by fainting or feeling seriously ill.

A practical appointment checklist

  • Write down your current medicines, allergies, relevant medical conditions, and the date of your last menstrual period.
  • Ask which IUD model is planned and how long it is approved to remain in place.
  • Ask what pregnancy or STI testing is recommended for you and whether results are needed before insertion.
  • Discuss pain-control choices, including local anesthetic availability, pauses, and a stop signal.
  • Arrange transportation or recovery time if you have previously felt faint or expect to need rest.
  • Confirm when contraception starts, whether backup is needed, and how to handle sex before that time.
  • Ask how and when to check the threads, whom to call with concerns, and what follow-up your clinic recommends.
  • Before leaving, record the insertion date and the device name in your health records or phone.

Questions worth asking the clinician

  1. Is this copper IUD or hormonal IUD/IUS, and what bleeding pattern is most relevant to this model?
  2. What pain-relief options can this clinic provide, and what should I avoid because of my medical history?
  3. What sensations should make me ask you to pause or stop?
  4. When does this specific device become effective, and do I need backup contraception?
  5. Which symptoms require a same-day call, an urgent visit, or emergency care?
  6. When should I contact you if I cannot feel the threads or they feel different?

Frequently asked questions

Does IUD insertion always hurt?

No. Cramping and pain are common possibilities, but intensity varies and cannot be predicted reliably for an individual. Some people feel pressure or a brief strong cramp; others feel little discomfort. A pain conversation before placement lets you discuss local anesthetic, pauses, explanations, and other options.

Can I ask the clinician to stop?

Yes. NHS guidance says you can ask for the fitting to stop at any time. Agree on a stop signal in advance if that will make communication easier. You can pause to reassess pain control or decide not to continue that day.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Will ibuprofen or another NSAID prevent insertion pain?

ACOG patient guidance says NSAIDs taken before insertion alone do not reliably control immediate insertion pain. They may help cramps afterward. Ask a clinician or pharmacist whether a particular medicine is safe for you and what instructions apply.

How long should I reserve for the appointment?

Planned Parenthood describes the placement itself as usually less than five minutes. The NHS describes fitting as about 5–10 minutes and recommends allowing at least 30 minutes for the full appointment, including preparation and recovery.

Can an IUD be inserted when I am not on my period?

ACOG says placement can occur at any point in the cycle when pregnancy has been reasonably excluded and there are no signs of infection. Your clinician will decide what testing and timing are appropriate.

When does my IUD start working?

There is no single answer for every IUD. Effectiveness and the need for backup contraception depend on the device, insertion timing, and local guidance. Ask for written instructions for the exact model.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Are cramps and spotting normal afterward?

Period-like cramps and light bleeding can occur for a few days. They should become manageable, not steadily worse. Severe or worsening pain, very heavy bleeding, fever, foul-smelling discharge, or suspected pregnancy requires prompt medical advice.

What if I cannot feel the strings?

Call a clinician for advice if the threads cannot be felt or seem different. Use another contraceptive method until the device has been checked, and follow local urgent-care guidance if you also have pain, bleeding, fever, or possible pregnancy.

Will my periods change?

A copper IUD may make periods heavier, longer, or more painful. A hormonal IUD or IUS may make periods lighter over time, although irregular bleeding can occur early. Ask about the expected pattern for your specific product.

Does an IUD protect against STIs?

No. An IUD provides contraception but not STI protection. Use condoms or another barrier method when protection from sexually transmitted infections is needed.

The key takeaway

IUD insertion is usually a brief procedure, but a short appointment does not mean every person has the same pain or recovery. Preparing a personalized pain plan, knowing that you can communicate discomfort and request a pause, and leaving with device-specific aftercare instructions gives you more control. Mild cramps and light bleeding can be part of the early adjustment; severe or worsening pain, fever, unusual discharge, very heavy bleeding, suspected pregnancy, or changed threads require clinical advice.

Frequently Asked Questions

Do I need backup contraception?

Possibly. Instructions depend on the exact IUD, insertion timing, and local guidance. Ask when your device becomes effective and what to use until then.

When should I seek help afterward?

Seek prompt advice for severe or worsening pelvic pain, fever, foul-smelling discharge, very heavy bleeding, suspected pregnancy, or threads that cannot be felt or seem different.

The Bottom Line

IUD placement is quick, but your comfort and control matter. Discuss pain options before the appointment, ask for pauses, follow device-specific contraception instructions, and seek care for severe or worsening symptoms.
