Radiopharmaceutical therapy is a targeted treatment in which a radioactive atom is attached to a molecule that recognizes a disease feature. After the medicine is given, it travels to cells carrying that target and delivers radiation nearby. The visit, radiation-safety instructions, side effects and follow-up depend on the specific drug, disease, dose, kidney function, blood counts and local protocol.
Expect your team to explain preparation, the treatment sequence, time in the department, temporary precautions at home and the monitoring plan. Use those written instructions—not advice from another therapy—as the rule for your household.
What radiopharmaceutical therapy is and how it works
Radiopharmaceutical therapy (also called molecular radiotherapy, and in selected settings radioligand therapy) combines two parts: a radionuclide that emits therapeutic radiation and a carrier molecule that binds to a biological marker on or near disease. The carrier can concentrate radiation at target sites while limiting exposure to tissues that do not carry the marker, although healthy organs can still receive radiation and require monitoring.

Your clinician should be able to name the target, radionuclide, intended treatment course and how response will be measured. There is no single schedule or universal “safe distance” that applies to every radiopharmaceutical.
Before the appointment: what to confirm
Preparation can differ substantially. At the planning visit, ask the nuclear-medicine or radiation-safety team to write down the details that apply to your drug and dose.
- Whether you must fast, change food or fluid intake, or receive pre-treatment medicines.
- Which prescriptions, over-the-counter medicines, vitamins or supplements need to be continued, paused or discussed.
- Whether blood tests, kidney tests, pregnancy testing or a target-confirming scan are required before treatment.
- How long you should expect to be in the department and whether the appointment includes observation or imaging.
- Whether you need someone to drive you home, and whether a caregiver may remain nearby during the visit.
- What to arrange at home if you share a bathroom, live with children, are pregnant or could be pregnant, or have a job involving close contact.
- Which telephone number to use during clinic hours and overnight if a symptom or practical problem occurs.
Do not stop a medicine or supplement on your own. The general patient materials do not establish universal preparation rules, so the treating facility’s instructions take precedence.
What can happen during a treatment visit
A typical visit has several phases, but the order and duration vary by therapy. The team verifies your identity and treatment, checks relevant laboratory results and reviews precautions. Staff may place an intravenous line, give fluids or protective medicines, administer the radiopharmaceutical and monitor you before discharge. Some treatments include imaging to confirm where the medicine traveled.

A concrete Lutathera example
Memorial Sloan Kettering’s patient guide describes a Lutathera visit with an amino-acid infusion, administration of the radiopharmaceutical and a nuclear-medicine scan. In that guide, the infusion visit and scan take about six hours. This is a Lutathera-specific MSK example, not a standard duration for radiopharmaceutical therapy as a whole.

You may notice temporary discomfort from an intravenous line or nausea during an amino-acid infusion. Tell staff immediately about new symptoms rather than waiting until the end of the appointment. The team decides whether observation, medication or a change in the infusion rate is needed.
Why radioactivity can matter after you leave
After some therapies, a small amount of radioactivity remains in the body for a limited time. The body can eliminate part of it in urine or feces. The amount, duration and route of excretion depend on the radionuclide, administered activity, kidney function and the protocol. A facility’s radiation-safety staff will give instructions tailored to your treatment.

Household and contact precautions
Your written plan may address:
- How close you may sit or sleep next to another person and for how long.
- Extra precautions around pregnant people and infants.
- Whether travel, public transport, work or school should be delayed.
- How to use, flush and clean the toilet, and how to wash your hands.
- How to handle laundry, towels, spills and shared utensils if your team gives special instructions.
- When ordinary contact can resume.
Ask specifically about each person in your household. “Several days” is an example from general PRRT guidance, not a universal timetable. Do not substitute a rule from a friend, internet forum or another radionuclide for the instructions issued by your radiation-safety team.
Lu-177 PSMA-specific measures
The 2023 joint EANM/SNMMI guideline for Lu-177 PSMA-targeted radioligand therapy discusses hydration to support kidney clearance and frequent voiding to reduce bladder dose, subject to cardiovascular status and clinical judgment. It also discusses minimizing prolonged close contact with pregnant people or infants after outpatient treatment. Those measures are specific to Lu-177 PSMA therapy and its protocol; they should not be generalized to every radiopharmaceutical.
Side effects: what is known and what is not predictable
Side effects depend on the drug, target, dose, other medicines and your health. A symptom list for one therapy cannot predict what you will experience with another.

- SNMMI’s information on ligand-targeted prostate radiotherapy lists fatigue, dry mouth and decreased blood counts among common effects.
- SNMMI’s PRRT material discusses nausea associated with the amino-acid infusion.
- PRRT guidance also discusses possible longer-term effects involving blood counts or kidneys.
These are treatment-specific examples, not a forecast for every patient. Ask when a symptom usually begins, how long it may last, which self-care measures are acceptable and which symptoms require a call. Do not take an anti-nausea, pain or antidiarrheal medicine without checking whether it fits your treatment and other medicines.

Early, delayed and late effects
Some effects occur during the infusion or soon afterward; others appear over days or weeks as blood counts or organ function change. The National Cancer Institute’s general radiation-side-effect guidance notes that some late effects can occur months or years after radiation treatment. That information concerns radiation therapy broadly, so it provides context rather than a radiopharmaceutical-specific symptom schedule. Your team should explain which delayed effects are relevant to your radionuclide and what surveillance is planned.
Follow-up tests and monitoring
Follow-up is part of the treatment, not an optional check after it. Depending on the therapy, clinicians may monitor blood counts, kidney function, liver tests, symptoms, blood-pressure or fluid status, and disease-specific imaging or laboratory markers. The timing can change after each cycle or if results are abnormal.
Before leaving each visit, confirm:
- The date and location of the next blood test or scan.
- Which results must be available before another treatment cycle.
- How the team will judge response, stable disease or progression.
- Whether a dose or schedule could change because of blood-count or kidney results.
- Who reviews results and how you will receive them.
- What to do if you cannot keep fluids down, cannot urinate normally, develop an unexpected symptom or miss an appointment.
Keep a record of treatment dates, symptoms, fluid intake if requested, medicines and questions. Bring it to follow-up visits; it helps the team distinguish a treatment effect from an unrelated illness or medication effect.

When treatment options are being compared
If your clinicians are discussing more than one radiopharmaceutical or another type of treatment, compare the options using the same questions rather than a single headline benefit.

The available patient guidance does not establish a universal ranking of therapies. The appropriate choice depends on target expression, disease history, previous treatments, organ function, practical support and your goals.
A practical discharge checklist
Before you leave the treatment center, make sure you have written answers to these items:
- The exact name of the radiopharmaceutical and treatment date.
- Hydration and voiding instructions, if applicable to your protocol.
- Distance and contact limits, including rules for pregnant people, infants and caregivers.
- Bathroom, handwashing, laundry and waste instructions.
- How to manage medicines and expected symptoms.
- The date of the next test, scan or treatment.
- A daytime and out-of-hours contact number.
- Instructions for emergency care that tell outside clinicians you recently received a radiopharmaceutical.
Keep the instruction sheet accessible at home. If another healthcare service, airline, employer or caregiver needs confirmation of your treatment, ask the radiation-safety team for the appropriate documentation rather than improvising an explanation.
Questions worth asking your treatment team
- What target does this medicine recognize, and why is it suitable for me?
- How long should I expect to be at the center, and can the schedule change?
- Do I need a driver or someone at home afterward?
- Which symptoms are expected, which require a same-day call and which require urgent help?
- How will you monitor my blood counts and kidneys?
- When can I resume normal contact, work, exercise, travel and childcare?
- Who should answer questions outside clinic hours?
Radiopharmaceutical therapy can be highly targeted, but it is still whole-patient care: the medicine’s distribution, temporary radioactivity, side effects and follow-up all matter. The safest plan is the one tailored by your treating and radiation-safety teams to the exact drug, dose and circumstances.
